PERFORMANCE OF WORK STATEMENT (PWS)

FOR

PLANT INSTALLATION/MAINTENANCE SERVICE

BLDG 116

AIR FORCE MORTUARY AFFAIRS OPERATIONS (AFMAO)

AT

DOVER AIR FORCE BASE

DELAWARE

1. Description of Service. Contractor shall provide a tranquil environment for grieving family members of fallen service members. Evaluate current plants and design. If needed, remove all existing plants, containers, mulch, and dirt. Sweep and wash floors, stone, and walls. Replace/plant all new plants, containers mulch/stone and dirt. This includes all external areas in the front of building 116, the internal courtyard and atrium as designated in the diagrams attached. Additionally, remove existing plants, mulch/stone and dirt in the brick circle in the front of the main entrance. The Japanese Maple that is currently planted, if healthy, will remain in the center of the outdoor circle at the entrance of 116. If unhealthy, the Japanese Maple must be replaced with the same type of tree. The two large trees within the courtyard as well as the trees in the front of the facility will remain and not be replaced unless plants are determined to be unhealthy. Replace/plant all stand-alone plants with new plants, containers mulch/stone and dirt. Vendor/Maintainer must have corporate horticulture experience (2 yrs. min) and have knowledge of the plants used and their care. Initiate full service and preventative maintenance (watering, pruning, replacement of sick or dead plants/trees) agreement for plants at Air Force Mortuary Affairs Operations (AFMAO), Dover AFB, DE. A site visit for potential bidders is requested to ensure the requirement is truly captured and the most accurate bid is provided. Vendor will provide the following:

1.1. Evaluate condition and remove (if necessary) all existing plants, containers, mulch and stone and dirt in the front of the facility, the atrium and interior courtyard of building 116. Once removed, sweep and wash floors, stone, and walls to remove all residue from insect repellant, dirt, rust, and water stains. Please see 1.1 PWS Attachment identifying all areas and measurements.

1.2. Configure and install new plantscaping in the atrium of building 116. The plants shall include various indoor plants throughout. The plants shall be healthy and a mixture of seasonal flowering and green plants in containers with adequate drainage and which complement the décor of the facility. Plants used will be indigenous to the eastern shore area and sustainable with available light sources. Stone or similar product will be used in the plant beds to hide pots and create a more natural appearance. Please see 1.2 PWS Attachment. This includes multiple photos of the current areas with measurements.

1.3 Configure and install new plantscaping in brick circle in front of building 116, beds extending from the front of building 116 (10ft). All trees shall remain, if healthy. The plants shall include various outdoor plants throughout the circle. The plants shall be healthy and a mixture of seasonal flowering and green plants with containers that drain properly, complement the facility and are indigenous to the area. Stone or similar product will be used in the plant beds to hide pots (if used) and create a more natural appearance. Please see 1.3 PWS Attachment. This includes multiple photos of the current areas and measurements. Additionally, we would like to see stepping-stones of the multiple branches of services, to include Army, Marines, Navy, Air Force, Space Force and Coast Guard. See 1.3 Stones PWS Attachment.

1.4 Configure and install new plantscaping internal courtyard in center of building 116. There are 4 beds of plants/trees. The trees shall remain, if healthy. The plants shall include various outdoor plants and greenery. The plants shall be healthy and a mixture of seasonal flowering and green plants in containers with adequate drainage, complement the facility and are indigenous to the area.

Stone or similar product will be used in the plant beds to hide pots and create a more natural appearance. Please see 1.4 PWS Attachment.

1. 5. The outside front of the building and interior courtyard will be planted with low maintenance and variegated colored plants. Some evergreen should be used to maintain appearance during the winter months. The plant bed will extend out 10 feet in the front of the building. Please see 1.5 PWS Attachment. This includes multiple photos of the current area and measurements.

1. 6. Maintenance of the plantscaping must be completed on weekly basis by member with a horticultural background (2 yrs. min). All dead leaves/plants must be removed and replaced with new. Remaining plants must be manicured to maintain a neat and professional appearance.

2. Specifications:

2.1. Site Visit: Potential vendors are strongly encouraged to perform a site visit to accurately determine project scope and submit a competitive proposal with drawings.

2.2. Materials: Successful vendor to supply and install all plants and materials required to perform the removal and installation.

2.3. Labor: Successful vendor to provide all labor necessary to complete work within the specified timeline of the contract.

2.4. Transportation and Lodging: Successful vendor will be responsible for all transportation and lodging for the entire duration of the contract performance period.

2.5. Period of Performance: Successful vendor will complete initial removal/replacement of plants no later than 90 days after contract award date.

2.6. Option Year Maintenance Coverage: The contract will contain a maintenance service agreement of four (4) option years to be exercised at the government’s discretion to provide complete maintenance and/or replacement of all plants, dirt, mulch, and containers.

3. Records Management. Records Management. The contractor understands that when creating, handling, and maintaining records for the Air Force, either electronic or paper, you must meet the requirements established in AFRIMS Records Disposition Schedule (RDS), AFI 33-322, Records Management Program and AFMAN 33-363, Management of Records. To meet the requirement established IAW the above-mentioned regulations contact the Base Records Management Office (436 CS/SCXK, Bldg 310) at (302) 677-5157 for guidance and training.

4. Security:

4.1. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the government installation shall abide by all security instructions and directives of Dover AFB. Employees are responsible for safeguarding all government property provided for contractor use.

4.2. At the close of each work period, government facilities, equipment and materials shall be secured, lights, heat and water turned off and all doors and windows secured.

4.3. The contractor and as applicable, subcontractor shall not employ persons for work on this contract if such employees is identified as a potential threat to the health, safety, security, and/or general well-being or operational mission of the installation and its population. Nor shall the contractor or subcontractor employ persons under this contract who have an outstanding criminal

Page 4 of 5 Pages warrant as identified during the Criminal Background Check (CBI).

4.4. Contractors shall ensure their employees and those of their subcontracts have the proper credentials allowing them to work in the United States. Employees and subcontractors later found to be undocumented or illegal aliens will be remanded to the proper authorities.

4.5. All contractors and subcontractors when working in a controlled, restricted or other sensitive areas must always be escorted. The military agency or unit responsible for the project or work is responsible for providing the escorts. The Contractor shall follow existing procedures and instructions for obtaining entrance to restricted or controlled.

4.6. All personnel employed by the contractor must sign a non-disclosure statement prior to performing under this contract.

5. SECURITY AND SAFETY REQUIREMENTS. In performing any work under this contract on premises which are under the direct control of the Government, the Contractor shall conform to all safety rules and requirements prescribed in Air Force Pamphlet 91-210, Contract Safety and Public Law 91-596, The Occupational Safety and Health Act of 1970. Effective on the date of this contract, the contractor shall take such additional precautions as the Contracting Officer may reasonably require for security, safety, traffic fire, personnel clearances, and accident prevention purposes. The Contractor agrees to take all reasonable steps and precautions to prevent accidents and preserve the life and health of the Contractor, Government Property and Government personnel performing or in any way encountering the performance of this contract on such premises. These requirements are additional to and do not replace the standards promulgated by the Department of Labor under the Occupational Safety and Health (OSHA) Act. In the event of a conflict between the OSHA Standards and these requirements, the most stringent shall apply. Any violation of such rules and requirements, unless promptly corrected, as directed by the Contracting Officer, may be considered grounds for termination of this contract in accordance with the default provisions hereof.

5.1. CONTRACTOR BADGE POLICIES. All requests for contractor badges will be submitted through the Base Contracting Squadron Office and/or the requesting agencies base point of contact.

The Contracting Squadron Office or base point of contact will then fill out an Operational Risk Management Assessment Form (ORM) for the requesting agency and submit it to the Pass and Registration section for approval IAW O Plan 31-101. As a minimum the ORM will be submitted 10-days prior to the requested date of employment.

5.1.1. All contractor employees who'll be granted unescorted access to the installation are required to consent to a Criminal Background Investigation (CBI) prior to being granted entry to the installation. Continued employment is contingent upon successful completion and favorable reporting of the criminal background check.

5.1.2. DAFB Contractor badges will be issued for a maximum period of one year. Prior to reissuing new badges, all old badges must be returned to 436 SFS, Pass and Registration for destruction. Also, before a new badge will be issued an ORM must be resubmitted to the Pass and Registration section for approval again. In the event a badge is lost or stolen, immediately contact the 436 SFS, Pass and Registration office. The badge holder must accomplish the Loss/Theft of Identification Worksheet and provide it to his/her supervisor. The supervisor of the contractor will investigate the loss and report in writing the circumstances in which the badge was lost to the 436th Contracting Squadron and 436 SFS, Pass and Registration office and submit a new request for badge before a new badge can be reissued.

5.1.3. Base Contracting Office or base point of contact will immediately notify Security Forces, Pass and Registration when a contractor's employment has been terminated. The Site Supervisors are responsible for notifying and returning the contractor's badge to Pass and Registration when this occurs. At no time will a contractor contact Security Forces directly regarding badge denial; they need to contact the Contracting Office.

5.2. INSTALLATION ACCESS. The contractor shall obtain personal contractor identification badge for all employees and vehicle passes for all contractor and personal vehicles requiring entry onto DAFB from Pass and Registration, for the duration of the contract. Employees are only permitted to enter the installation during the date and time periods indicated on their contractor badge.

Vehicle registration, proof of insurance and a valid driver's license must be presented for all vehicles while operating on the installation. All vehicles entering the installation or sensitive areas are subject to search. Any refusal or non-consent by an employee will result in termination of their base access and immediate confiscation of this access badge.

5.2.1. During Force Protection Condition (FPCON) Normal and Alpha, personnel without base issued badges must be sponsored onto the installation.

5.2.2. During FPCON Bravo, Charlie and Delta, personnel without base issued identification shall be physically escorted onto the installation. 5.3.3. During Higher FPCONs (Charlie and Delta) the base will curtail non-essential operations/functions; access by Non-Essential Contractors will be suspended as the direction of the Installation Commander.

5.3. FLIGHTLINE DRIVING. Contractors will have to adhere to the provisions in DAFBI 13- 202 if operating on the airfield.

NOTE: The successful vendor and all employees who would have any interaction with the AFMAO or CFF facilities would be required to sign a non-disclosure agreement which prohibits the signatory from discussing anything they see, hear or smell while in the facility. Dover AFB will take every action possible to protect the dignity of the honored dead, and the sensitivity of contractor personnel;

however, employees must be made aware of the fact that they could be subjected to sights, noises or smells that could be very disturbing.
