Pest Control Services Ft. Yuma Healthcare Center
Winterhaven, CA
STATEMENT OF WORK
1. SCOPE:
This Statement of Work (SOW) is for the provision of pest control services at the Indian Health Service, Fort Yuma Healthcare Center (FYHC). The FYHC is located in Winterhaven, CA. The FYHC facility is approximately 76,000 gross square feet, which is located across from Paradise Casino, the Quechan Senior Center, and Pipa Market. Pest Control is to be provided every month at the following location:
1.1 Around the exterior and interior of building
1.1.1 Fort Yuma Health Center
401 Picacho Road Winterhaven, CA 92283
1.2 Additionally, pest control shall be provided around storage buildings, ground mounted equipment (such as emergency generators, cooling towers, and storage containers) and miscellaneous structures.
2. GENERAL
The Contractor shall provide all supervision, labor, materials, tools, traps, equipment and transportation necessary to perform the work as required by the SOW and supporting documents.
2.1 A site map and overall floor plan are included with the Integrated Pest Management Plan, which is attached to the FYHC Pest Control Policy (see Attachment A).
2.2 The Contractor shall be responsible for the extermination and removal of vermin, pests, and rodents such as mice and rats, insects such as spiders, roaches, cockroaches, bedbugs, ticks, ants, silverfish, flies, fleas, moths, wasps, bees, hornets, centipedes, scorpions and other insects common to the Winterhaven Area.
Ticks, and bees typically regarded as specialty pests, are to be included in the general monthly pricing.
2.2.1 The facility construction included a 5-year warranty on termite control that began in February of 2017; annual termite inspections shall be included along with termite re-treatment to provide 5 years of protection commencing February 2022.
2.2.2 Flies are a frequent pest due to the close proximity of agricultural fields. The contractor shall include sufficient UV fly traps indoors and traps with non-toxic attractants outdoors.
2.2.3 The close proximity of agricultural fields to the Southwest of the property raises the concerns of mice on the grounds and in the facility.
Trapping of large vertebrae such as dogs and cats is excluded from the scope (to be performed by Quechan Fish and Game), but sufficient traps for rodents shall be provided to ensure safe and sanitary conditions at the clinic.
Contractor is responsible to survey the building for appropriate locations and provide proper trap quantities.
2.2.4 General pest control inside the building is required; the facility does not include a food service area although a demonstration kitchen with a residential stove is included for diabetes education and nutrition demonstration. Cockroaches are a particular concern: the property and supply warehouse are especially susceptible to cockroaches due to the nature of receiving outside shipping containers and having a roll-up door; the contractor shall take special precautions to ensure that cockroaches are baited and eliminated around the warehouse section of the building.
2.2.5 Bird control shall be included in the scope of services.
2.2.5.1 The contractor shall inspect premises and remove nests, dead birds, droppings, feathers, etc. on the grounds on a monthly basis. The rooftop, penthouse walls, solar photovoltaic collectors and associated components, solar water heater collectors, air handling units, and shade structures are a particular concern for ensuring that birds do not nest.
2.2.5.2 The contractor shall install and maintain bird netting to the underside of the facility’s five carports. Spikes and optical gels have been found to be ineffective at keeping birds away from these areas.
2.2.5.3 The contractor shall disinfect with an EPA-approved
disinfectant after removal of all bird matter on a monthly basis.
2.2.5.4 Use of pressurized water and firearms are prohibited.
Carport Area (sq.ft.)

A schedule shall be agreed upon between the Facilities Engineer and the Contractor prior to commencement of services. Rescheduling must be submitted by the Contractor electronically and approved by the Facilities Engineer at least one week prior to work being performed. 2.4 Services shall be in accordance with Federal, State, and Tribal Government regulations, Environmental Protection Agency (EPA), Occupational Safety and Health Association (OSHA), US Department of Agriculture and other applicable regulations. This intent is to ensure patients, staff and visitors at the FYHC are not harmed by pest control activities, as well as protection of the Contractor’s employees.
2.4.1 The patient population at the health center is generally a higher-risk population than the American public at-large; the use of the least toxicity and least amount of chemicals is preferred to ensure the overall health of the patients. Non-chemical pest prevention such as air curtains (not provided during construction) and traps are preferred to the use of chemical sprays.
2.5 The Contractor is required to follow the regulations of the Federal Insecticide, Fungicide and Rodenticide Act (FIFRA). Information on FIFRA can be found at 7 U.S.C. 136 et seq. and implementing regulations at 40 CFR 165 et seq.
