B.1 STATEMENT OF WORK

STATEMENT OF WORK

FOR SAN DIEGO VA HEALTHCARE SYSTEM,

SAN DIEGO, CA

1. Introduction The purpose of this statement of work (SOW) is to outline the tasks and responsibilities of the successful offeror in relation to providing a Slit Lamp with applanation tonometer on a motorized slit lamp table to be used in our emergency room by ophthalmology doctors and ophthalmology residents. Also, a slit lamp imaging system for installation on an existing Haag Streit BQ-900 slit lamp that is in our ophthalmology clinic. This is a different location than where the emergency room slit lamp is located. There is also a requirement for a wireless indirect ophthalmoscope to be used in the emergency room by ophthalmologists and residents who will move between hospital wards, the ophthalmology clinic and the emergency room.

2. Background The San Diego VA Healthcare Hospital currently needs a slit lamp with tonometer mounted on a wheelchair accessible, adjustable height motorized slit lamp table as well as a binocular indirect ophthalmoscope to be used in the emergency department. In addition, the ophthalmology department needs a slit lamp image capture system to be used in the ophthalmology clinic installed on an existing Haag Streit BQ-900 slit lamp.

3. Scope This procurement is to provide our ophthalmology clinic with a new Slit Lamp imaging system capable of capturing, editing, storing and uploading Digital Imaging and Communications in Medicine Standard interface (DICOM) compatible images to our existing electronic medical record and to be installed on our existing Haag Streit BQ-900 slit lamp.

The Slit Lamp imaging system requested has a variety of features that allows the ophthalmic photographer to capture images of eye pathology, analyze, edit, store and upload those images to our electronic medical record via a DICOM interface. Training onsite is required for the imaging system.

This procurement is also for diagnostic equipment to be used in our emergency department.

The emergency department requires a slit lamp with tonometer that is fully compatible with our existing accessories for Haag Streit Slit lamps and a wheelchair accessible motorized height adjustable slit lamp table. No training is required for a Haag Streit slit lamp as the VA San Diego Healthcare System currently has several of this make and model already in use in the hospital.

Additional diagnostic equipment required for the emergency room includes a binocular indirect ophthalmoscope which is compatible with the accessories the VA San Diego has for an existing Keeler LED Slimline Vantage Indirect Wireless Ophthalmoscope.

4. General Requirements Vendor shall provide to San Diego VA Hospital, A “NEW” LED Slit Lamp with tonometer and NEW wheelchair accessible motorized adjustable height slit lamp table as well as a NEW wireless binocular indirect ophthalmoscope for use in the emergency room and a NEW slit lamp Imaging Systems to be installed on our existing Haag Streit BQ-900 slit lamp in the ophthalmology department.

5. Mandatory Tasks

5.1. The Vendor shall ship a Slit Lamp with applanation tonometer and wheelchair accessible motorized adjustable height slit lamp table, a wireless binocular indirect ophthalmoscope and a slit lamp Imaging Systems to the San Diego VA Hospital.

5.2. Once received at San Diego VA Hospital, the vendor shall physically assist Biomed in assembling the LED Slit Lamp with tonometer and mounting it on the wheelchair accessible motorized slit lamp table.

5.3. The vendor shall install the slit lamp Imaging System with capture and analysis system on an existing Haag Streit BQ-900 tungsten slit lamp, convert the existing tungsten Haag Streit slit lamp to a LED illumination, and assist the biomed department with interfacing the system to the VA network in order to upload images via a DICOM interface to the existing Vista Imaging System and existing Zeiss Forum system.

5.4. The vendor shall provide a onetime training at the VA San Diego Medical Center for up to 12 staff members on the use of the Imaging System.

5.5. The vendor shall contact the VA POC to coordinate the assembling, installation and training.

6. Period of Performance/Acceptance:

The Period of Performance (PoP) is 2 Months after receipt of order, this PoP includes delivering, assembling and training on the equipment.

9. Product Features for Haag Streit (Brand Name Only)

9.1. Brand Name for Slit Lamp

All salient characteristic requirements stated below must be met to accommodate the needs of the Ophthalmologists when treating patients, to make use of existing slit lamp accessories and attachments and to make sure that accessories and attachments can be used interchangeably on existing Haag Streit BQ-900 Slit Lamps

9.1.1. The slit lamp should have 5 adjustable magnification levels so that the ophthalmologist can examine the eye in more detail

9.1.2. The slit lamp should be LED illuminated as this provides illumination superior to other illumination methods.

9.1.3. The slit lamp should include an attached and fully integrated applanation tonometer.

9.1.4. The Slit lamp must be fully integrated/compatible with existing Haag Streit BQ-900 accessories and attachments. The hospital currently owns 12 Haag Streit BQ-900 Slip Lamps with accessories and attachments. The slit lamp for the emergency room needs to be compatible with a Haag Streit BQ-900 beam splitter and long observation tube. As the VA san Diego is an academic institution, it frequently has medical students and residents in the emergency room who need to be able to observe what the ophthalmologist is viewing through the beam splitter and observation tube.

Additionally, the VA San Diego is a multispecialty referral center for eye disease, with a glaucoma specialist, the slit lamp needs to be compatible with a stereo variator which allows the doctor to use the slit lamp to view through a small pupil. The slip lamp needs to be fully compatible with a Haag Streit inclined optics to meet the ergonomic needs of some of the doctors.

9.1.5. Accessories and attachments listed in 9.1.4 for the emergency room slit lamp need to be fully compatible with existing Haag Streit BQ-900 Slit Lamps already in the hospital so that separate attachments can be shared between the emergency room and the ophthalmology clinic.

9.1.6. The slit lamp shall have a star fixation system to allow our doctors to better visualize the retina.

9.2. Brand Name for the Imagine System

9.2.1. The imaging system must be fully functional and integrated on the existing Haag

Streit BQ-900 Slit Lamp.

9.2.2. The imaging system shall be an integrated, “turn key” system and must include a beam splitter/camera able to mount on our existing Haag Streit BQ-900 slit lamp , integrated computer and high resolution monitor with EyeSuite image capture, editing and storage software and must have a DICOM interface so that the images can be uploaded via the VA San Diego network to the existing VA Vista Imaging System and to the existing Zeiss Forum software. The software must be approved by VA Office of Information and Technology (OIT) since it will contain private patient information.

9.2.3. The beam splitter/camera must allow a fully adjustable aperture setting so that the photographer can vary the amount of light that is directed to the camera. This allows for superior image composition as the photographer can control and compose the exposure with the control that only a fully adjustable aperture setting will allow. As an academic referral center, the VA San Diego has some unique requirements. The VA San Diego needs the capability to produce high quality images suitable for publication, teaching, research and patient care. Fixed beam splitters (without a fully adjustable aperture) would not produce the images needed as an academic institution to conduct research and provide training to fellows, residents and medical students.

9.2.4. The image capture system should include a depth of focus control to allow the photographer to compose an image of the ocular structure of interest to be in sharp focus with the ability to vary the depth of focus so that structures not of interest to the photographer can be defocused. Again, as the VA San Diego is an academic referral center, it sees patients with rare and infrequent ocular pathology that requires additional features to image ocular structures for teaching, research, publication and patient care

9.2.5. The imaging capture system and EyeSuite Software shall include a feature that allows the capture of multiple images at once with varied exposures. This feature allows the photographer to obtain high quality images without the need to repeat multiple exposures. The history trigger functionality will allow the multiple images (30 frames per second) to be captured and analyzed by the photographer so that any patient movement or patient blinking will not create artifact on the image. This will allow the photographer to obtain high quality images of the correct exposure without the need to repeat the pictures multiple times. As the patient population is older, frail and often have mobility and cooperation issues, this will allow the system to capture quality images that other systems may not be able to capture at all.

9.2.6. The imaging system should have controls that are fully integrated with the existing Haag Streit BQ-900 slit lamp and controls should be mounted within finger length of the slit lamp joystick. This feature is required so that the photographer does not need to divert their attention from the slit lamp oculars while composing and capturing the image. By having integrated controls, the photographer can capture images that they might not be able to if they needed to look away from the slit lamp.

9.2.7. The imaging system shall include a computer and monitor and EyeSuite imaging software able to capture, edit, store and upload images to our existing electronic medical record through a DICOM interface. The computer should be able to efficiently run the software and should be: Intel 5th generation or higher with 4 cores, 16GB RAM, 500 GB (NTFS) hard drive, dedicated graphic card with at least 2 GB of memory, PCI express card chipset of renesas/NEC. Monitor shall be greater than 20 inches but less than 28 inches with a resolution of 1920x1080 or 1920x1200 pixels.

9.2.8. The Eyesuite software must VA Office of Information Technology (OIT) approved in accordance with Directive 6550.

9.3. Brand Name for Slit Lamp Table

9.3.1. The slit lamp must be mounted on a Slit Lamp table that is powered with 120VAC input.

9.3.2. The table must be wheelchair accessible.

9.3.3. The table must have a motorized height adjustment.

9.3.4. The table must be on wheels so it can be moved about and stored in the emergency room.

9.3.5. The table wheels must be lockable to provide stability when the slit lamp is in use.

9.3.6. The table should be designed for use with an ophthalmic slit lamp.

10. Salient Characteristics for Keeler LED Slimline Vantage Indirect Wireless and Ophthalmoscopy or Equal

10.1.1. The Binocular Indirect Ophthalmoscope shall be LED illumination in order to allow the doctor superior illumination of the eye.

10.1.2. The Binocular Indirect Ophthalmoscope should be wireless and shall include a charger and 2 battery pack.

10.1.3. The battery packs should be fully compatible with our existing Keeler LED Slimline Vantage Indirect Wireless Ophthalmoscope so the doctor can move between the emergency room, the wards and the eye clinic and interchangeably swap drained battery packs in compatible chargers. This will save valuable time of the doctors from moving from location to location in the hospital and being able to swap drained for fully charged battery packs.

10.1.4. The wireless binocular indirect ophthalmoscope should be fully compatible with existing safety shields that the VA San Diego Healthcare System has custom produced on a 3D printer to protect our doctors from exposure to coronavirus during the COVID-19 epidemic. Ophthalmologists are the third most at risk medical specialty for COVID-19. To protect the doctors from coronavirus the VA San Diego has had existing Keeler Indirect Ophthalmoscopes retrofitted with custom made, 3D printer face shields made specifically for these Keeler Ophthalmoscopes.
