SECTION B - CONTINUATION OF SF 1449 BLOCKS

ACKNOWLEDGMENT OF AMENDMENTS: The quoter acknowledges receipt of amendments to the Solicitation numbered and dated as follows:

AMENDMENT NUMBER DATE

INSTRUCTIONS TO QUOTERS

The purpose of this Solicitation is to obtain quotes for the services set forth in Section B.3 Performance

Work Statement. The Government, in its sole discretion, may award a contract from the quotes received in response to this Solicitation, issue a new Solicitation, or cancel this Solicitation without reissue.

ACQUISITION OF COMMERCIAL ITEMS: This Request for Quote (RFQ) solicitation is issued under the authority of FAR Part 12 Acquisition of Commercial Items and FAR Part 13 Simplified

Acquisition Procedures. FAR Part 15 will not be used. The procedures in Part 12 used in conjunction with Part 13 allow the Government to procure services resembling those in the commercial marketplace and streamline the procurement process. From this RFQ, the Government intends to award a firm-fixed-price contract where the contractor provides a price that is not subject to any adjustment on the basis of the contractor’s cost experience in performing the contract. (Note: The terms “offeror” and “quoter” are both used in this Solicitation. For the purposes of this Solicitation, these terms are interchangeable; there is no difference in meaning between the two.)

The Government, in its sole discretion, may award a contract from the quotes received in response to this Solicitation, issue a new Solicitation, or cancel this Solicitation without reissue.

SMALL BUSINESS SET-ASIDE: This requirement has been set aside for Small Businesses pursuant to

FAR Subpart 19.5.

THE SITE VISIT. It is strongly suggested and expected that the potential quoter inspect the campus to be mailto:Antoinette.Byrd@va.gov serviced to fully understand the nature of the work and the conditions under which the work is to be performed, but attending the site visit is not required to submit a quote. In no case shall failure to attend a site visit or otherwise inspect the campus constitute grounds for a claim after contract award.

RFQ QUESTIONS: In order to maintain the integrity of this solicitation and subsequent award, all potential quoters shall submit questions regarding this solicitation via e-mail to Contracting Officer Rob

Knight at Robert.Knight1@va.gov. Answers will be issued via a consolidated response by solicitation amendment posted on Contract Opportunities, available at https://sam.gov/content/opportunities.

SUBMISSION OF QUOTE: Quoters shall submit their quote by e-mail to Contracting Officer Rob

Knight at Robert.Knight1@va.gov. Please limit graphics or excessively large files during quote submission. The VA has a maximum allowable incoming email size limitation, including attachments, of seven (7) megabytes. Quoters submitting responses via email that exceed 7 megabytes shall split their response into multiple email messages so as to not exceed the maximum allowable email size limitation. The VA may not consider late responses due to undelivered e-mails that exceed 7 megabytes.

CONTENTS OF QUOTE: The purpose of this Solicitation is to obtain a high-performing contractor that provides the services set forth in Section B.3 during the Period(s) of Performance set forth in Section B.2.

A high-performing contractor provides exemplary service with minimal oversight. The Government, in its sole discretion, will award a contract to the responsible contractor whose quotation, conforming to the solicitation, is most advantageous to the Government, price and other factors considered.

The following factors will be used to evaluate conforming quotations. The order of the factors listed does not imply the relative importance of each factor: (1) Price; (2) Technical Capability; (3) Prior Experience;

(4) Administrative Information; and (5) Representations & Certifications.

The Government will evaluate quotations using the comparative evaluation process outlined in FAR

13.106-2(b)(3), where quotations will be compared to one another to determine which provides the best benefit to the Government based on the factors listed above. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s). Quotations may exceed minimum requirements of the solicitation. The Government reserves the right to select a quotation that provides benefit to the Government that exceeds the minimum requirements of the solicitation, even if that quote is not the lowest priced quote submitted, but is not required to do so. Each response must meet the minimum requirements of the solicitation. The Government is not requesting or accepting alternate quotations

Quotes shall include the following information to be evaluated for contract award. Incomplete quotes will be considered non-conforming and may not be considered, pursuant to FAR 13.106-2(b)(3).

(1) Price: Complete the highlighted blanks in SECTION B.2 PRICE SCHEDULE.

(2) Technical Capability: The quotation shall detail how the contractor will provide the services detailed in Section B.3 Performance Work Statement during the Period(s) of Performance set forth in Section B.2

Price Schedule. The quote shall also include the following information:

mailto:Robert.Knight1@va.gov https://sam.gov/content/opportunities mailto:Robert.Knight1@va.gov

b. Pictures of the delivery vehicle(s) that will transport the floor mats and floor pads;

(3) Prior Experience: Submit information, if any, of contracts or subcontracts providing services similar to the requirements of this Contract. The quote shall specify if any of the prior contracts or subcontracts involved providing services to Department of Veterans Affairs healthcare facilities. Information should include contract number, dates of service, and a point-of-contact with a telephone number or e-mail address. For evaluation purposes, a lack of prior experience will not disqualify a company.

(4) Administrative Information:

a. Complete SF1449 blocks 17a, 30a, 30b, and 30c on Page 1;

b. Complete Acknowledgment of Amendments (if any), on Page 2; and

(5) Representations & Certifications (Note: The Government will not consider quotes that do not provide the following certifications, and any incomplete responses shall be deemed ineligible for evaluation and award.):

a. Provide a copy of the quoter’s Small Business Program Representations under FAR 52.219-1(c), available at SAM.gov | Entity Information; this representation must include the NAICS code for this acquisition, set forth in Block 10 of Page 1; or in the alternative, you must provide a statement representing in good faith that your company meets the small business size standard corresponding to the

NAICS code identified in this Solicitation, pursuant to FAR 19.301-1.

b. Complete the representation found under provision FAR 52.204-24(d), Representation Regarding

Certain Telecommunications and Video Surveillance Services or Equipment, found in Section E.

c. Complete the representations and certifications as directed under provision FAR 52.212-3, Offeror

Representations and Certifications – Commercial Items, found in Section E.

Guidance on Section (5) Representations & Certifications

This section of Amendment 1 provides detailed guidance regarding submission of the

Representations & Certifications required in paragraph (5), above.

a. Your company’s Small Business Program Representations under FAR 52.219-1(c) is available at

SAM.gov | Entity Information. Enter your company’s information on this screen to find the SAM profile:

https://sam.gov/content/entity-information https://www.acquisition.gov/far/part-19#FAR_19_301 https://sam.gov/content/entity-information

Select your company, then select Reps and Certs (FAR/DFARS) on the left side of the page:

Scroll down the page until you reach FAR 52.219-1:

Click on FAR 52.219-1 and scroll down until you reach 52.219-1(c):

Provide a copy of this table with the quote. This representation must include the NAICS code for this acquisition, set forth in Block 10 of Page 1.

If this representation does not include the NAICS code for this acquisition, you must provide a statement representing in good faith that your company meets the small business size standard corresponding to the

NAICS code identified in this Solicitation, pursuant to FAR 19.301-1.

b. The next requirement is a representation found in Section E, FAR 52.204-24(d):

52.204-24 REPRESENTATION REGARDING CERTAIN

TELECOMMUNICATIONS AND VIDEO SURVEILLANCE SERVICES OR

EQUIPMENT

The first paragraph of FAR 52.204-24 states:

The Offeror shall not complete the representation at paragraph (d)(1) of this provision if the Offeror has represented that it ‘‘does not provide covered telecommunications equipment or services as a part of its offered products or services to the Government in the performance of any contract, subcontract, or other contractual instrument’’ in paragraph

(c)(1) in the provision at 52.204–26, Covered Telecommunications Equipment or

Services—Representation, or in paragraph (v)(2)(i) of the provision at 52.212–3, Offeror

Representations and Certifications–Commercial Items. The Offeror shall not complete the representation in paragraph (d)(2) of this provision if the Offeror has represented that it

‘‘does not use covered telecommunications equipment or services, or any equipment, system, or service that uses covered telecommunications equipment or services’’ in paragraph (c)(2) of the provision at 52.204–26, or in paragraph (v)(2)(ii) of the provision at 52.212–3.

What this paragraph means is that you do not complete the representations set forth in subparagraph

52.204-24(d) if your company has already made those representations with its SAM registration. You can check your company’s SAM registration via https://sam.gov under Entity Information on the homepage of that website. Under your company’s entity information page, there will be a menu on the left side where you can check the current Representations and Certifications made with your SAM registration:

Under that list of Reps and Certs, you should find the representation made for FAR 52.204–26, as referenced in the first paragraph of FAR 52.204-24, above. Click on the hyperlink for 52.204-26, and it will provide the representation made for your company. Here is an example:

https://sam.gov/

In this case, the quote does not require the FAR 52.204-24(d) representations be completed. Instead, the quote should reference the company’s SAM registration and the representations already made under FAR

52.204-26.

c. The final requirement is also found under Section E, FAR 52.212-3:

52.212-3 OFFEROR REPRESENTATIONS AND CERTIFICATIONS—

COMMERCIAL ITEMS

The first paragraph on FAR 52.212-3 states:

The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for

Award Management (SAM) accessed through https://www.sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (v) of this provision.

Current SAM registration is a requirement for contract award; this requirement cannot be waived.

Accordingly, assuming that your company’s SAM registration is current, you will reference FAR 52.212-

3(b), which states:

(b)(1) Annual Representations and Certifications. Any changes provided by the Offeror in paragraph (b)(2) of this provision do not automatically change the representations and certifications in SAM.

(2) The offeror has completed the annual representations and certifications electronically in SAM accessed through http://www.sam.gov. After reviewing SAM information, the

Offeror verifies by submission of this offer that the representations and certifications currently posted electronically at FAR 52.212–3, Offeror Representations and

Certifications—Commercial Items, have been entered or updated in the last 12 months, are current, accurate, complete, and applicable to this solicitation (including the business size standard(s) applicable to the NAICS code(s) referenced for this solicitation), at the time this offer is submitted and are incorporated in this offer by reference (see FAR 4.1201), except for paragraphs _______________.

In other words, if no representations and certifications made for your company’s annual SAM registration have changed, you do not need to submit anything more for this paragraph. If, however, a representation or certification needs to be updated, you need to update it here, as well as in SAM.

In summary, section (5) Representations & Certifications requires the following:

(1) a copy of your company’s small business representations under FAR 52.219-1 that includes the

NAICS code for this acquisition, or in the alternative, you must provide a statement representing in good faith that your company meets the small business size standard corresponding to the

NAICS code identified in this Solicitation, pursuant to FAR 19.301-1;

https://www.sam.gov/ http://www.sam.gov/

(2) a statement that the representation made for FAR 52.204-26 of your company’s SAM registration is current, and therefore no FAR 52.204-24 representation is required; and

(3) a statement that the representations and certifications made for your company’s SAM registration are current, unless there is a representation or certification that needs to be updated
