(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

(ii) This solicitation is issued as a Request for Quotation (RFQ).

(iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-04.

(iv) This procurement is set aside for Small Business. The NAICS code is 562111 and the Small Business Size Standard is $41.5 Million.

Chippewa National Forest has a need for Garbage pickup at their campgrounds throughout the Forest for the 2022 camping season.

Quotes will be accepted until May 16, 2022, 12 Noon

Prior to submitting a price quote the Contractor should become familiar with all existing conditions, locations, and limitations pertaining to the work specified.

Addenda to paragraph (b) Submission of Offers:

2) Offers submitted in response to this solicitation shall include a technical proposal, a price proposal, and contractor representations and certifications.

a) Technical Proposal – The technical proposal shall address the evaluation factors in 52.212-2. At a minimum technical proposal shall include:

i) Past Performance – provide a list of similar projects completed by your firm over the past three years. Include a brief description of the project, dollar amount, year completed, and project owner contact information (name, phone, and email). The government may use past performance information from any available source. If a company does not have past performance information available, information may be provided for predecessor companies, key personnel, or subcontractors. In the event that there is no past performance information available, the offeror will receive a neutral rating in this factor.

a) Price Proposal – Include the following: 1) completed, signed SF-1449 from Section A of this solicitation, 2) Acknowledge any amendments issued on sam.gov for this solicitation by completing and signing the posted SF-30, and 3) Include the completed price schedule listed in Attachment B as a separate document in this solicitation.

b) Representations and Certifications – Fill in the check boxes for provisions 52.204-24, and 52.212-3, and found in Section F of this solicitation and include a copy with your offer.

2) Submit offer by email to joyce.rairdon@usda.gov so that it is delivered into this inbox by the due date and time. Emails should contain 3 separate attachments (Technical Proposal, Price Proposal, and Representations and Certifications) in Microsoft Word or Adobe PDF format. (ix) 52.212-2 Evaluation – Commercial Products and Commercial Services (NOV 2021) (Provision)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

(d) Comptroller General Examination of Record. 452.204-70 Modification for Contract Closeout (OCT 2021) Upon contract closeout for contracts utilizing Simplified Acquisition Procedures (SAP) according to FAR 13:

(a) If unobligated funds in the amount of $1000 or less remain on the contract, the Contracting Officer (CO) shall issue a unilateral modification for deobligation. The contractor will receive a copy of the modification but will not be required to provide a signature. The CO shall immediately proceed with contract closeout upon completion of the period of performance, receipt and acceptance of supplies or services, and final payment.

(b) If unobligated funds of more than $1000 remain on the contract, the CO shall issue a bilateral modification for deobligation. The contractor will receive a copy of the modification and will be required to provide a signature. (The CO may also request a Release of Claims be completed by the contractor, although not required for contract and orders using SAP procedures.) If the bilateral modification and Release of Claims are not returned to the CO within 60 days, the CO shall release the modification as unilateral and proceed with contract closeout upon completion of the period of performance, receipt and acceptance of supplies or services, and final payment.

Period of Performance The period of performance of this contract is from May 18, 2022 through April 30, 2023.

Post Award Conference A post award conference with the successful offeror is optional. It will be scheduled within __5__ days after the date of contract award. The conference will be held at the discretion of the COR.

Insurance Coverage Pursuant to FAR clause 52.228-5, Insurance-Work on a Government Installation, the Contractor will be required to present evidence to show, as a minimum, the amounts of insurance coverage indicated below:

(a) Workers Compensation and Employer's Liability. The Contractor is required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 shall be required, except in States with exclusive or monopolistic funds that do not permit worker's compensation to be written by private carriers.

The Offeror shall not complete the representation at paragraph (d)(1) of this provision if the Offeror has represented that it "does not provide covered telecommunications equipment or services as a part of its offered products or services to the Government in the performance of any contract, subcontract, or other contractual instrument" in paragraph (c)(1) in the provision at 52.204-26, Covered Telecommunications Equipment or Services—Representation, or in paragraph (v)(2)(i) of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Products or Commercial Services. 2022 Toilet Pumping Contract Statement of Work

Chippewa National Forest Beltrami, Cass, and Itasca Counties

Scope of Work

The contractor will:

1. Provide a truck or trucks in good operating and mechanical condition capable of unloading four and six cubic yard containers at campgrounds on the Chippewa National Forest as well as 30 cubic yard roll offs at two administrative sites.

a. The truck(s) furnished shall meet State of Minnesota regulations for class of vehicle

b. Four and six yard dupsters shall be equipped with bear-proof lids and not have any holes.

2. Be responsible for protecting the public and Forest Service personnel, Forest Service facilities, and resources in the course of the work.

3. Place the empty containers on the special gravel or blacktop pad. The dumpsters shall not be left in a hazardous or unsafe position.

4. Haul in such a manner as to prevent the contents from leaking, spilling or blowing out and should be done in compliance with all Federal, State and local regulations. Where spillage does occur, the contractor shall pick the material up immediately.

5. Collect the regular weekly pickups on Monday or Tuesday of the week for the dates listed on Attachment A FY22 Garbage Pickup Schedule. Roll-Offs shall be collected during the weeks listed on Attachment A FY22 Garbage Pickup Schedule.

6. Service the dumpsters between the hours of 9:00 a.m. and 6:00 p.m.

7. Dispose of waste in a State of Minnesota licensed sanitary landfill.

8. Obtain State and county or other licenses or permits necessary and pay disposal fees.
