JUSTIFICATION FOR SOLE SOURCE

Only One Source

Purchase Request No: ________________________________________ Directorate: Directorate of Surgical Services Department: Main Operating Room

POC:

Tel. No.:

Suggested Source: Covidien

1. Description of supplies/services.

The Main Operating Room (MOR) is requesting to purchase directly from Covidien as required items for basic cases utilizing electrosurgical cautery and tissue dissection that can only be procured through direct purchase currently. The requested items include the Ligasure device for both laparoscopic and open General Surgery, Urology and OB/GYN cases.

2. Demonstration of contractor's unique qualifications.

Covidien is the only manufacturer of Endoscopic and open Ligasure devices which are a surgical staple in the MOR. They are not available through our Prime Vendor or

3. Description of market research.

Covidien/Medtronic is the source that our PVM, has directed us to purchase directly from for the immediate future. Covidien is also the only manufacturer of the Ligasure® cautery device, which has been adopted as a staple across many surgical services and current surgical technique relies upon.

Covidien/Medtronic will continue to be carried by PVM as some catalog items are still available through PVM. Only Covidien/Medtronic was contacted for price quotation.

Purchasing other electrocautery/electrosurgery consumables would require the purchase or rental of the manufacturer’s proprietary equipment to utilize them; which would prove to be neither expedient nor costeffective. Delaying the MORs ability to support the performance of these types of cases will result in delayed surgical intervention, worsening of patient condition, Rev 13 OCT 2015 disengagement to network and/or lowered patient satisfaction.

4. Quantity and dollar value

Estimated value of the requirement $250,000. We plan to submit a purchase request each month and/or as needed dependent on the surgical case load schedule of approximately $25,000.00- 30,000.00 Per month, until the $250,000.00 funds has been exhausted for the agreement.

5. Delivery requirements.

Delivery is required as soon as shipping allows after purchasing.
