Q.2 On page 112, the RFP mentions sections C (Monitoring, Evaluation and Learning Plan) and D (Contractor Past Performance Information). Do those sections need to be included as part of the technical approach in the proposal or do they refer to annexes 2, 5, and 6?

A.2 Both, C and D sections, refer to Annexes 2, 5, and 6.

Q.5 On page 123, under Section (O) Fixed Fee, there is a paragraph that states the cost proposal must also include the following: (a) Salary ranges for all professional staff and (b) Pay Scale for short term consultants. Please clarify whether this should be part of the fixed fee section, and please clarify whether Offerors are only required to provide salary ranges and pay scale for named staff and consultants.

However in section H. 39 page 55, it states that hard copies of the SF294 and SF295 are no longer accepted and contractors are required to submit these reports electronically. Could USAID confirm if you can access the SF294 report through eSRS and Offerors should only submit SF294s which cannot be accessed electronically?

A.7 SF 294 is a part of Technical Proposal. Annex 10: SF 294 “Subcontracting Report for Individual Contracts” (no page limit). In addition, contractors are required to submit these reports electronically to the electronic Subcontracting Reporting System (eSRS).

This information is not included in the page limitation.” Could USAID please confirm that this information is solely meant for each of the projects provided for Annexes 5 and 6? In addition, should this information be included as additional pages for Annexes 5 and 6’s combined total page limit of 6 pages?

A.11 USAID can confirm that this information is solely meant for each of the projects provided for Annexes 5 and 6 and should be included as additional pages and not subject to the total and 6 pages page limitation.

Q.12 On page 129 and in section D, there are 6 elements listed as being used to evaluate the contractor’s past performance information. Could USAID please clarify whether the offeror should include information on each of these elements if they are applicable for each past performance reference in annexes 5 and 6?

A.12 Yes, potential offeror should include all six elements listed on page 129 under Section D “Contractor Part Performance”, if applicable.

Q.13 Could USAID please confirm that the executive summary is excluded from the 25 page technical proposal page limitation and share what its page limit is?

A.13 No. The executive summary is included in the 25-page limitation. No separate page limit for executive summary is provided.

A.16 Yes. USAID confirms that L.6 General Instructions to Offers section should be followed.

Q.17 Could USAID please confirm that the “Management Structure chart” referenced on the bottom of page 111 is the same as Annex 7: Organizational Chart and the chart should “illustrate the roles of the organizations in activity implementation”. If they are not the same, would USAID please elaborate on what the difference between the two charts is and consider allowing for an additional annex so that the Management Structure chart may be included that way?

A.17 Technical proposal should include the Management Plan as described on page 111.

Organizational Chart is an additional document presenting organization’s personnel positions’ titles and responsibilities in a form of a chart.

Q.18 For the staffing plan referenced on page 112, would USAID consider allowing it to be included as an annex?

A.18 No, it should be a part of Technical Proposal.

Q.19 Will USAID please confirm that there can be as few as three key personnel proposed (if one candidate is proposed to cover two or three functions) per page 111?

A.19 Yes, there can be as few as three Key Personnel proposed by an Offeror (if one candidate to cover two or three functions).

Q.20 Can USAID please clarify what is meant by partnership on page 111 in the sentence, "...implementers shall not bind local staff to an exclusivity clause for key or non-key personnel or partnership"? Does partnership pertain to partners/subs?

A.20 Partnership means potential project partners.

On page 57, the RFP states that "resumes for key personnel may not exceed four pages in length and two pages for all other personnel. Each resume for Key Personnel shall be accompanied by a letter of commitment." Could USAID please clarify the total page limit for resumes, letters of commitment and references for all key personnel and the total page limit for resumes for all other personnel? In order to properly represent proposed staff, key and non-key personnel, would USAID also please consider excluding Annex 3 from the total page count of 60 pages for the technical proposal or increase the page count for Annex 3 and the overall page limit for the technical proposal.

A.22 USAID confirms that Annex 3: Resumes, and Letters of Commitment for Key Personnel. Please include at least 3 Reference Contacts for Key Personnel at the end of the résumés for key personnel (6-page limit). The paragraph on the page 114 should read as below:

“Annex 3: Resumes, and Letters of Commitment for Key Personnel (6-page limit);”

Q.23 Does USAID require a certificate of registration to operate in Ukraine?

Q.24 Can USAID provide us a copy of the Local Compensation Plan (LCP) for Ukraine?

A.24 Please be advised that the maximum daily rate for senior locally hired specialists is $251.23 under USAID contracts. However, Offerors are expected to conduct their own market research for determining the salary scale for locally hired positions in Ukraine or rely on their previous work experience and other implementing partners’ experience in the region.

The ranges for the grades of the Local Compensation Plan for Ukraine are provided in the tables below (in U.S. Dollars).

Q.25 Can USAID provide clarification regarding the expected level of coordination with other Ministries/sectors besides the MOH in Ukraine?

A.25 It is an Offeror responsibility to determine the level of coordination with other Ministries/sectors besides the Ministry of Health of Ukraine in order to achieve activity goal and objectives listed in the Scope of Work.

A.26 SIAPS is adapted and implemented eTB Manager, a web-based tool for managing information needed by national TB programs

Q.27 As Annexes 8, 9 and 10 have no page limits, does the overall Technical Proposal plus annexes still need to be under 60 pages as stated on page 108 of the RFP?

A.27 The Technical Proposal is limited to sixty (60) pages including Annexes.

Q.28 Page 57, Section H.47 states: The Contractor shall provide résumés for all professional personnel (new or replacement) accompanied by Position Descriptions for long-term employees and Terms of Reference/SOW for short-term consultants. Résumés for Key Personnel may not exceed four pages in length and two pages for all other personnel. Q.29 Page 108 of the RFP states a 5-page limit for Annex 3 while page 114 lists a 6-page limit for Annex 3. Please confirm if Annex 3 should be 5 or 6 pages. Given that there are 5 key personnel positions requiring a resume and letter of commitment for each, we request a minimum of a 10-page limit in order to include at least a one-page resume and letter of commitment for each position.

A.29 See the answer to Q22 above.

Q.30 Page 13, Section C. V; Transition: Who is the incumbent of …”the existing SIAPS contract…” so that the new vendors can ensure a continuous provision of technical assistance and guarantee a comprehensive knowledge transfer between the incumbent and the new implementing partner(s)”

A.30 Management Sciences for Health (MSH) is the incumbent of the existing SIAPS project.

Q.31 Page 118, L.8 (c): The offeror must use the illustrative budget format contained in Attachment J.4 “SUMMARY BUDGET FORMAT.” Can the offeror use their own budget templates/formats as long as it includes all the details that are present in the J.4 Summary budget format?

A.31 The Offeror may provide its own budget templates/formats but only in addition to the J.4

Summary budget format required by USAID.

Q. 32. Page 120: Proposed Costs/Price and Cost Proposal Narrative states: “Salaries and wages must be based on the requirements of the positions proposed and NOT on the proposed individual's’ salary histories. Salaries for long-term and short term CCN staff must not exceed a threshold as specified in Compensation Plan (LCP) in the amount of $65,320/year ($251.23/day).”Would USAID please provide a copy of the Local Compensation Plan so that we can ensure that salaries are commensurate with position titles and work experience?”

A.32 See the answer to Q.24 above.

Q.33 Page 128, B. Management Plan, Staffing Plan, Institutional Capacity, and Key Personnel states:

USAID's FY 2016 worldwide small business subcontracting goal is 18%. However Page 150 states that USAID’s Small business subcontracting goals are 19%.

Question: Could USAID please clarify and provide the correct FY2017 Small business goals?

A.33 USAID confirms that USAID's FY 2016 worldwide small business subcontracting goal is 18% as stated on page 128.

Q.34 Page 4: para B.4 (4) states: “The contractor will not bill any amounts against this contract in excess of the amounts specified for each line item.” Would USAID consider adjusting this statement to:

“The contractor will not bill any amounts to the staff and consultant line items in excess of the amounts specified in the contract budget for those line items. Amounts for other line items may be adjusted up or down, but adjustments may not exceed 10% of the amount in the contract budget for any affected line item.”?

A.34 No.

Q.35 Page 9: para above “objective one…” states: “The AIP must… … which are identified in the AWP,…” Does AWP mean annual work plan and is it the same document as the AIP?

A.35 This is a typo. The term Annual Work Plan (AWP) has been changed to Annual Work Plan, AIP means Annual Implementation Plan. The paragraph on the page 9 should read as below:

“The AIP must outline the implementation process and detail activity schedules, resource requirements, roles and responsibilities, monitoring, reporting and review procedures. Quarterly monitoring of milestones, which are identified in the AIP, must be conducted by the contractor with progress being reviewed monthly and reported quarterly to the COR.”

Q.36 Pages 13 & 14, under VII. Program Management; Management Plan: There are small but significant differences between the first and second paragraphs; e.g. the first paragraph retains mention of “grantees”. Would USAID consider deleting whichever paragraph is least reflective of what it wants?

A.36 Please consider “Grants under Contract are not authorized under this Contract” statements made under “Grants under Contract” on page 14 as well as Section K) “Grants under the Contract” on page 121.

Q.37 Page 14, last line under “Rapid Response Capacity”: USAID mentions “sub-grants”. Is there a possibility that the awardee may be authorized to issue sub-grants, and on page 111 in the first paragraph under “Management Plan” USAID mentions “grantee”? Would USAID consider clarifying whether the awardee is expected, or may, with approval, award grants under SAFEMed?

A.37 Please refer to “Grants under Contract” on page 14 and Section K) “Grants under the Contract” on page 121 that states: “Grants under Contract are not authorized under this Contract”.

Q.38 Page 16, third bullet near the top of the page reads: “suggest to the GOU concrete examples of how requirements on public procurement of medicines and health commodities. Would USAID consider clarifying this requirement? It appears to be incomplete.

A.38 Please read the third bullet on page 16 as: “Suggest to the GOU concrete examples of how gender equality requirement can be applied on public procurement of medicines and health commodities”.

Q.39 Page 31 contains a reference to “cooperative agreement provisions.” Would USAID consider clarifying the sentence containing this reference?

A.39 This is a typo. The paragraph on the page 31 should read as below:

“The MEL Plan is subject to final approval by USAID and is separate from the regular financial and other reports required by the Contract provisions.”

Q.40 Page 111, the third paragraph under a) Key Personnel and Other Professional Personnel states:

“…implementers shall not bind local staff to an exclusivity clause for key or non-key personnel or partnership.” Would USAID consider explaining how it will enforce this request, since some qualified key or non-key local candidates are likely to be currently employed by an organization likely to submit a proposal? Would USAID consider deleting the afore-mentioned request, and instead require proposals to identify the local key positions that will be filled locally and the qualifications and process to be used to fill those positions, and, thereby, really provide equal access to the best local key and non-key personnel?

A.40 There is no issue if a person is currently employed by an organization.

Q.41 Page 89 makes refers to: “the NAICS code referenced for this solicitation”

Would USAID consider providing the NAICS code for the SOL-121-17-000005?

A.41 The NAICS code for the SOL-121-17-000005 is 541990 – All Other Professional, Scientific, and Technical Services.

Q.42 Pages 107 and 108 contain a series of page limitations, some of which appear quite constraining, specifically:

Would USAID consider clarifying whether the page limit for Annex 3, is 5 pages (as indicated on page 8) or 6 pages (as indicated in the last line of page 114) for the CVs and letters of commitment for each proposed key staff person, or if it is a total of 5 pages for the 5 CVs and 5 letters of commitment?

Would USAID consider clarifying if the letters of commitment from proposed key staff persons should be in the Technical Proposal or in the Cost Proposal?

Currently the proposed page limits appear as show below:

# Item Description No. of pages 1 Technical Proposal 25 2 Annex 1: Year 1 Initial Implementation Plan 8 3 Annex 2: Draft Monitoring, Evaluation & Learning (MEL) 4

Plan and Draft Logical Framework 8 4 Annex 3: Resumes, Commitment Letters for Key Personnel inc. 3 References

7 Annex 6: Offerer's Major Sub-Contractors' PPR (from past 3 years)

Total 59

A.42 See the answer to Q22 above. In addition, Currently the proposed page limits appear as show below:

# Item Description No. of pages 1 Technical Proposal 25 2 Annex 1: Year 1 Initial Implementation Plan 8 3 Annex 2: Draft Monitoring, Evaluation & Learning (MEL)

Plan and Draft Logical Framework 8 4 Annex 3: Resumes, Commitment Letters for Key Personnel inc. 3 References

6 Annex 5: Offeror’s relevant past performance (from past 3 years)

7 Annex 6: Offeror’s Major Sub-Contractors' PPR (from past 3 years)

Total 60

Q.43 The page, font and margin limitations for the Annex 1 (Year 1 Initial Implementation Plan) and Annex 3 (Draft Logical Framework) permit 472 lines for each requirement. Would USAID consider providing flexibility on how proposals use the 35 page limit for Annexes?

A.43 Our recommendation is not to use a type smaller than Times New Roman 12 point for regular text and 8-point type for graphics and tables, provided it is legible. For Annexes 1 and 2 you may use 8-point type, if needed.

Q.44 Page 111: second paragraph under “Management Plan” mentions letters of commitment from “proposed partners.” Would USAID consider clarifying whether such letters of commitment should be included in the page limit of the Technical Proposal or in the Cost Proposal?

A.44 USAID confirms that Letters of Commitment for Subcontractors and proposed partners should be included into Cost Proposal.

Q.45 Page 112 states that “CVs for expatriate, short-term technical assistance shall be included…”, while on Page 108 USAID requests a “1-paragraph summary for each named…Expatriate and Ukrainian Short-Term Technical Expert (3 page limit)’. Would USAID consider clarifying whether full CVs are required for Short-Term Technical Experts and, if so, where they should be included and the associated page limit?

A.45 Full CVs are not required for expatriate, short-term technical assistance.

Q.46 Page 112, under Contractor Past Performance Information suggests that “Attachment J.10 – Contractor Performance Report – Short form may be used by offerors to present the information requested….” and also requests the offeror and each major sub-contractor to “List in an annex up to five of the most recent and relevant contracts”. Would USAID consider clarifying the apparent inconsistency between the length (one page) of the Contractor Performance Report – Short Form, the maximum number of relevant contracts for which performance is requested (“up to five”) from the offeror and also from each proposed major sub-contractor, and the page limit (3 pages each for the offeror and for “each major sub-contractor(s)” as stated for Annexes 5 and 6 on Page 108 and in the table for item #9 above?

A.46 Full CVs are not required for expatriate, short-term technical assistance.

Q.47 Page 115; Annex 5 states: “Offeror’s past performance information relevant to anti-corruption sector/activities from the past 3 years. On Page 108, the required content of Annex 5 makes no mention of “anti-corruption sector activities.” Would USAID consider clarifying the nature of the past performance information it considers to be relevant to respond to SOL-121-17-000005?

A.47 Annex 5 should include information relevant to anti-corruption sector/activities from the past 3 years, as described on page 115

Q.48 Page 118: Third paragraph under L.8 states: “and each part must be separated by a tab or colored divider page.” Would USAID consider clarifying how it wishes each part to be separated in an electronic submission; e.g. starting each part on a new page.

A.48 This is an oversight. Hard copies and their dividers are not required.

Q.49 Page 127: Third line under Contextually Appropriate contains the phrase “results which are not limited in the SOW” and the last phrase in the under Local Engagement contains the phrase “which are not limited to in the SOW”. Both quoted phrases appear to be missing words or are otherwise unclear.

Would USAID consider clarifying those phrases?

A.49 “which are not limited to in the SOW” is a citation from the Scope of Work stated in Section C.

Each of three activity objective includes, out of other, “Support under this component includes, but is not limited to, the following…”

Q.50 Page 129: The second paragraph under Overview of Technical Approach reads “Demonstrate a comprehensive understanding of activity demonstrate Technical Approach on implementation of the three objectives of SAFEMed.” Would USAID consider clarifying the quoted paragraph?

A.50 The text on page 129, 1.Overview of Technical Approach: will be read as follows:

“Demonstrate a comprehensive understanding of on implementation of the three objective of SAFEMed.”

Q51. We respectfully request that USAID provide a copy of the “Ukraine National Supply Chain Assessment Results" first referenced on page 8 of the RFP.
