STATEMENT OF WORK

North Half

C.1.0 GENERAL:

Contractor shall furnish vault pumping services on an as needed basis which includes all equipment, supervision, transportation, labor, operating supplies, and incidentals necessary to pump, wash down, cleanup, disinfect, and dispose of all waste products removed from the vault toilets and fish cleaning vault, in accordance with the following contract specifications and in accordance with State and Federal regulations.

C.2.0 DESCRIPTION:

C.2.1 See Sections B and J for pumping sites. All vaults located in the Vale District are listed in Section B. Some vaults may be pumped several times a year and others not at all. The purpose of this RFQ is to establish a price in the event the vault needs to be pumped.

C.2.2 Scheduling of Work: The BLM will notify the contractor five (5) days prior to pumping each designated site (not including Saturday and Sunday). Generally, the contractor may complete the required servicing between the hours of 8:00 a.m. and 5:00 p.m.

C.3.0 DEFINITIONS:

Sewage - liquid, semi-liquid, and solid waste matter.

Debris - all other refuse found in vault toilets and fish cleaning station.

C.4.0 TECHNICAL SPECIFICATIONS:

1. The Contractor shall furnish all toilet pumping equipment, tank truck, tools, approved disinfectant, and other equipment and supplies necessary to perform work in accordance with these specifications.

2. Water drafting equipment (if necessary, to get water from a lake, pond, or stream) shall consist of pumps, tanks, hoses, etc., and shall be completely separate from toilet waste pumping and hauling equipment.

3. All disinfecting solutions shall be approved by the Contracting Officers Representative (COR).

4. All equipment shall be in full accordance with any local, State, and county regulations.

5. The Contractor is responsible for all work completed including sub-contractors; all sub-contractors must meet the same requirements outlined in the contract as the primary Contractor.

a. The Contractor shall be properly licensed and is responsible for obtaining all necessary permits and licenses for pumping, hauling and disposing of the effluent.

b. The Contractor shall be responsible for making disposal arrangements, including disposal of all sewage, debris, and trash, and payments of all fees. The contractor shall follow state and federal laws in regards to disposal of the waste.

BLM VALE DISTRICT Page 12 of 70

C.4.1 Pumping Procedures: If there is an outside manhole available, sewage and debris should be removed through the manhole. If there is no manhole available, sewage and debris will have to be removed through the toilet riser.

1. All sewage and debris shall be removed from vault. Vaults may contain trash such as beverage containers, plastic bags, etc. Removal of these items is a requirement of the contract. Contractor shall be responsible for the removal, transport and proper disposal of trash and refuse found within the vault.

2. ALL DEBRIS REMOVED FROM THE VAULT SHALL BE PLACED INTO A LEAK PROOF

CONTAINER SO THAT NO CONTAMINATION, TO THE SURROUNDING GROUND,

WILL RESULT.

3. Pre-charge the vault with clean fresh water so that there is eight (8) inches of water covering the bottom under the toilet riser. Contractor shall pump all wet effluent from toilet vaults. Vaults must be pumped dry, with less than one inch of effluent remaining in the vault for each pumping.

Immediately upon completion of pumping the vault, the Contractor shall charge the vault with approximately 80 gallons of clean water except where specified in Section 4.0. Contractor can use delineated sites on project maps for water supply. Contractor shall ensure that a method is used that does not contaminate the water supply source (i.e., ensuring a different hose is used for clean water application). Although the Government is providing a water supply source for the contractor to use at some sites, it is the sole responsibility of the Contractor to ensure that proper hauling and servicing applications are followed. For areas annotated in Section 4.0 that do not have a water supply present, the contractor shall be required to provided adequate clean water to fulfill the aforementioned requirement.

4. Thoroughly clean and disinfect all contaminated areas resulting from the pumping process. Either sweep or wash all waste that is deposited on the concrete surrounding the manhole cover back into the vault, if possible. The waste that runs off the edge of the concrete, onto the dirt or into the gravel edge, will be an attractant for flies, a potential health risk, and an odor problem. This problem should be avoided.

5. Clean thoroughly around the manhole cover gasket area and replace the cover. The manhole cover MUST fit with an airtight seal so that the venting process of the vault toilet is not hampered.

C.4.2 Pumping Through the Toilet Riser When There is No Outside Manhole: Remove all debris and pump. Extra care shall be taken to prevent waste spillage onto the interior floor and wall surface while removing debris and while pumping. The toilet paper dispenser and toilet riser shall be protected from damage and the toilet paper shall be covered with a waterproof material prior to removing debris or pumping, in order to prevent the paper from getting contaminated.

1. During the final cleanup, the interior floor surface and any wall surface that was contaminated by waste shall be thoroughly disinfected. Again, every effort shall be made to sweep or wash the spilled waste back into the vault. The Contractor shall not allow the spilled waste to be washed or swept out through the front door because the waste will collect at the edge of the concrete and the dirt (or gravel) and will become an attractant for flies, a possible health risk, and an odor problem.

2. Thoroughly wash and disinfect both the inside and outside of the toilet riser and the seat and cover. The toilet riser and seat shall be dry before the Contractor leaves the work site. Remove the

BLM VALE DISTRICT Page 13 of 70 waterproof covering that was protecting the toilet paper and make sure the paper did not get contaminated. If the paper did get contaminated, the Contractor shall properly dispose of the paper and furnish a new roll. The door handles or knobs shall also be disinfected and dry before the Contractor leaves the job site.

3. The Contractor may pump additional toilets that are part of this contract in order to “top off” or fill the pumper truck to capacity.

4. Contractor shall provide documentation of total gallons pumped for each trip.

5. Each unit shall be available for immediate use after the pumping and final cleanup. The Contractor shall not leave any toilet location until final cleanup is accomplished.

C.4.3 Drafting of Water: Contractor may draft fresh water (to be used for washing down the vault walls, emulsification of the waste, and for final cleanup) from lakes, ponds and streams under the following conditions:

a. Drafting shall be accomplished where it least interferes with the public use of the facility and at areas least viewed by the public.

b. No waste shall be introduced into any body of water.

c. All drafting hoses and apparatuses shall be clean and shall not be used for vault pumping procedures.

d. The Contracting Officer shall approve all sites used for drafting water.

C.4.4 Fish Cleaning Station: The fish cleaning vault system consists of one vault with two compartments. One being approximately 5200 gallons and the other approximately 2700 gallons.

Contractor shall pump the smaller compartment completely dry and leave approximately 150 gallons in the larger compartment. The contractor shall follow state and federal laws in regards to disposal of this waste.

C.4.5 Off-site Disposal of Sewage, Debris and Fish Materials: The Contractor shall be properly licensed and shall be responsible for making disposal arrangements, including disposal of all effluent, debris, and trash, and payments of all fees.

1. Disposal shall conform to all Federal, State, and local laws and ordinances.

2. The Contractor shall be responsible to prevent spillage and to haul all waste material (sewage and debris) in watertight containers. Handling of all waste material shall be in accordance with all applicable health regulations.

b. The Contractor shall be responsible for making disposal arrangements, including disposal of all sewage, debris, and trash, and payments of all fees. The contractor shall follow state and federal laws in regards to disposal of the waste.

C.4.6 Conduct of Personnel: The Contractor and his/her employees shall make every reasonable effort to prevent interference with the normal use and occupancy of recreational areas while engaged in the performance of the contract.

BLM VALE DISTRICT Page 14 of 70

1. Due to the nature of the duties under the contract, the Contractor and his/her employees shall keep themselves and equipment presentable at all times. C.1.0 GENERAL:

Contractor shall furnish vault pumping services on an as needed basis which includes all equipment, supervision, transportation, labor, operating supplies, and incidentals necessary to pump, wash down, cleanup, disinfect, and dispose of all waste products removed from the vault toilets and washrack, in accordance with the following contract specifications and in accordance with State and Federal regulations.

Debris - all other refuse found in vault toilets and washrack.

C.4.0 TECHNICAL SPECIFICATIONS:

6. The Contractor shall furnish all toilet pumping equipment, tank truck, tools, approved disinfectant, and other equipment and supplies necessary to perform work in accordance with these specifications.

7. Water drafting equipment (if necessary, to get water from a lake, pond, or stream) shall consist of pumps, tanks, hoses, etc., and shall be completely separate from toilet waste pumping and hauling equipment.

8. All disinfecting solutions shall be approved by the Contracting Officers Representative (COR).

9. All equipment shall be in full accordance with any local, State, and county regulations.

BLM VALE DISTRICT Page 16 of 70

10. The Contractor is responsible for all work completed including sub-contractors; all sub-contractors must meet the same requirements outlined in the contract as the primary Contractor.

a. The Contractor shall be properly licensed and is responsible for obtaining all necessary permits and licenses for pumping, hauling and disposing of the effluent.

b. b. The Contractor shall be responsible for making disposal arrangements, including disposal of all sewage, debris, and trash, and payments of all fees. The contractor shall follow state and federal laws in regards to disposal of the waste.

C.4.1 Pumping Procedures: If there is an outside manhole available, sewage and debris should be removed through the manhole. If there is no manhole available, sewage and debris will have to be removed through the toilet riser.

6. All sewage and debris shall be removed from vault. Vaults may contain trash such as beverage containers, plastic bags, etc. Removal of these items is a requirement of the contract. Contractor shall be responsible for the removal, transport and proper disposal of trash and refuse found within the vault.

7. ALL DEBRIS REMOVED FROM THE VAULT SHALL BE PLACED INTO A LEAK PROOF

CONTAINER SO THAT NO CONTAMINATION, TO THE SURROUNDING GROUND,

WILL RESULT.

The toilet paper dispenser and toilet riser shall be

BLM VALE DISTRICT Page 17 of 70 protected from damage and the toilet paper shall be covered with a waterproof material prior to removing debris or pumping, in order to prevent the paper from getting contaminated.

7. Thoroughly wash and disinfect both the inside and outside of the toilet riser and the seat and cover. The toilet riser and seat shall be dry before the Contractor leaves the work site. Remove the waterproof covering that was protecting the toilet paper and make sure the paper did not get contaminated. If the paper did get contaminated, the Contractor shall properly dispose of the paper and furnish a new roll. The door handles or knobs shall also be disinfected and dry before the Contractor leaves the job site.

8. The Contractor may pump additional toilets that are part of this contract in order to “top off” or fill the pumper truck to capacity.

9. Contractor shall provide documentation of total gallons pumped for each trip.

C.4.4 Washrack: Waste is generated from a concrete slab-on-grade vehicle washrack. Washrack contains undercarriage spray bar as well as having a hand wand. Sediment basin to be pumped is 1500 gallons. Oil Separator is 550 gallons and sump pump is 130 gallons. The sediment basin and oil separator are tested annually for Total Petroleum Hydrocarbons (TPH) and Toxicity Characteristic. With these results a Liquid Waste Transport Manifest will be produced and sent to Clay Peak Landfill and a copy also provided to the contractor upon pumping of the washrack.

1. BLM shall remove cattle guard grate for pumping the sediment basin.

BLM VALE DISTRICT Page 18 of 70

C.4.5 Offsite Disposal of Sewage and Debris: The Contractor shall be properly licensed and shall be responsible for making disposal arrangements, including disposal of all effluent, debris, and trash, and payments of all fees.

3. Disposal shall conform to all Federal, State, and local laws and ordinances.

4. The Contractor shall be responsible to prevent spillage and to haul all waste material (sewage and debris) in watertight containers. Handling of all waste material shall be in accordance with all applicable health regulations.

2. Due to the nature of the duties under the contract, the Contractor and his/her employees shall keep themselves and equipment presentable at all times. The Contracting Officer may, in writing, require the Contractor to remove from the work site any employee the Contracting Officer deems incompetent, careless, discourteous to the public, or is otherwise objectionable, or for theft, possession, and/or removal of materials, supplies, equipment or any government-owned property.

C.4.7 Resource Damage: Any damage or defacement that occurs to government property or land resources during the Contractors operations shall be repaired to its original condition at the Contractor’s expense. Onsite inspection will be made by the COR prior to the acceptance of work.

C.4.8 The below sites will be pumped utilizing the schedule services that are annotated. An Asterisk (*) denotes sites that do not have a water supply present, the contractor shall be required to provided adequate clean water to fulfill the aforementioned requirement. Some sites have running water nearby, if Contractor does choose to use that water source refer to Section C.4.3.

CONTRACT ADMINISTRATION DATA

The following local clauses are provided in full text:

CONTRACTOR PERFORMANCE ASSESSMENT REPORTING SYSTEM (JULY 2010)

(a) FAR 42.1502 directs all Federal agencies to collect past performance information on contracts. The Department of the Interior (DOI) has implemented the Contractor Performance Assessment Reporting System (CPARS) to comply with this regulation. One or more past performance evaluations will be conducted in order to record your contract performance as required by FAR42.15.

(b) The past performance evaluation process is a totally paperless process using CPARS. CPARS is a web-based system that allows for electronic processing of the performance evaluation report. Once the report is processed, it is available for Government use in evaluating past performance as part of a source selection action.

(c) We request that you furnish the Contracting Officer with the name, position title, phone number, and email address for each person designated to have access to your firm's past performance evaluation(s) for the contract no later than 30 days after award. Each person granted access will have the ability to provide comments in the Contractor portion of the report and state whether or not the Contractor agrees with the evaluation, before returning the report to the Assessing Official. The report information must be protected as source selection sensitive information not releasable to the public.

(d) When your Contractor Representative(s) (Past Performance Points of Contact) are registered in CPARS, they will receive an automatically-generated email with detailed login instructions. Further details, systems requirements, and training information for CPARS is available at https://www.cpars.gov/. The CPARS User Manual, registration for On Line Training for Contractor Representatives, and a practice application may be found at this site.

(e) Within 60 days after the end of a performance period, the Contracting Officer will complete an interim or final past performance evaluation, and the report will be accessible at https://www.cpars.gov/. Contractor Representatives may then provide comments in response to the evaluation, or return the evaluation without comment. Comments are limited to the space provided in Block 22. Your comments should focus on objective facts in the Assessing Official's narrative and should provide your views on the causes and ramifications of the assessed performance. In addition to the ratings and supporting narratives, blocks 1 - 17 should be reviewed for accuracy, as these include key fields that will be used by the Government to identify your firm in future source selection actions. If you elect not to provide comments, please acknowledge receipt of the evaluation by indicating ''No comment" in Block 22, and then signing and dating Block 23 of the form. Without a statement in Block 22, you will be unable to sign and submit the evaluation back to the Government. If you do not sign and submit the CPAR within 30 days, it will automatically be returned to the Government and will be annotated: "The report was delivered/received by the contractor on (date). The contractor neither signed nor offered comment in response to this assessment." Your response is due within 30 calendar days after receipt of the CPAR.

(f) The following guidelines apply concerning your use of the past performance evaluation:

(1) Protect the evaluation as "source selection information." After review, transmit the evaluation by completing and submitting the form through CPARS. If for some reason you are unable to view and/or submit the form through CPARS, contact the Contracting Officer for instructions.

(2) Strictly control access to the evaluation within your organization. Ensure the evaluation is never released to persons or entities outside of your control.

https://www.cpars.gov/ http://www.cpars.csd.disa.milj/ https://www.cpars.gov/ http://www.cpars.csd.disa.mil/

BLM VALE DISTRICT Page 22 of 70

(3) Prohibit the use of or reference to evaluation data for advertising, promotional material, preaward surveys, responsibility determinations, production readiness reviews, or other similar purposes.

(g) If you wish to discuss a past performance evaluation, you should request a meeting in writing to the Contracting Officer no later than seven days following your receipt of the evaluation. The meeting will be held in person or via telephone or other means during your 30- day review period.

(h) A copy of the completed past performance evaluation will be available in CPARS for your viewing and for Government use supporting source selection actions after it has been finalized.
