PERFORMANCE-BASED WORK STATEMENT (PWS)
GENERAL DESCRIPTION
The US Geological Survey’s (USGS) Hammond Bay Biological Station (HBBS) requires clean and healthy working conditions at the facility located at 11188 Ray Road, Millersburg, MI 49759.
The HBBS is seeking a contract with a contractor to provide janitorial services per this Work Statement (WS) and Task Schedule.
SCOPE
The Contractor shall provide all labor, travel, administration, supervision, supplies, and equipment (except those items specifically provided by the Government as referenced in this document) to perform janitorial services at the HBBS. Performance shall be in strict accordance with all terms, conditions, general and special provisions, and attachments contained in this contract or incorporated by reference.
Standard janitorial practices and proper use of cleaning materials and equipment are required.
This work shall include the cleaning of three buildings. The wet lab, office building and sterilization building (See Attachment 1 Cleaning Contract Floor Plans). The work shall also include coordination and communication with the on-site Contracting Officer Representative (COR) to report emergency issues, organize schedules, order supplies and equipment, and arrange for cleaning of occupied spaces and on-the-spot needs. All work shall be accomplished in such a manner as to ensure clean, safe, and reliable product. All work shall conform to the requirements of the Occupational Safety and Health Administration (OSHA) and shall be in keeping with the standard practices of janitorial services. The quality of the workmanship, including all materials, shall comply with applicable Federal specifications.
GENERAL REQUIREMENTS
Hours of Operation
The Contractor shall arrange his/her work to avoid interference with the normal occurrence of Government business. All work schedules shall be submitted to the Contracting Officer (CO) or the Contracting Officer’s Representative (COR) for approval. Whenever services have been scheduled on the date a holiday occurs, such services shall be performed on the following working day. In no event shall the Contractor change approved work schedules without prior consent from the CO or COR.
In the wet lab building (Attachment A is 1,551 sq. ft.) janitorial services shall be provided 3 days each week, Monday, Wednesday, and Friday, during the months of April, May, June, July, August, and September, and twice a week on Wednesday and Friday during the months of January, February, March, October, November, December, (except Federal holidays and emergencies), after normal business hours (8:00 am and 4:30 pm), unless otherwise agreed by the CO or the COR. If requested, prior approval by the COR is required before any work may be performed on weekends or Government holidays.
In the office building (Attachment B is 1,851 sq. ft. and Attachment B-1 is 788 sq. ft.) janitorial services shall be provided once a week, on Wednesday, during all months of the year (except
Federal holidays and emergencies), after normal business hours (8:00 am and 4:30 pm), unless

otherwise agreed by the CO or the COR. If requested, prior approval by the COR is required before any work may be performed on weekends or Government holidays.
In the sterilization building (Attachment C is 768 sq. ft.) janitorial services shall be provide once a week, on Wednesday, during the months of April, May, June, July, August, and September, and no days January, February, March, October, November, and December (except Federal holidays and emergencies), after normal business hours (8:00 am and 4:30 pm), unless otherwise agreed by the CO or the COR. If requested, prior approval of the COR is required before any work may be performed on weekends or Government holidays.
Contractor’s Representative
The Contractor shall designate in writing the Contractor’s Representative, (foreman, superintendent, or manager) as the on-call point of contact who must establish and maintain a notification or communication system so that information can be conveyed to the Contractor from the Government on a 24-hour basis, seven (7) days a week.
Prior to beginning work, the Contractor’s Representative shall designate, in writing to the CO and to the COR, a representative who shall be in charge of the work and be available on-site at all times when work is being performed. When the Contractor's Representative is temporarily unavailable to manage the contract effort for a period longer than 72 hours, including absences due to vacation or illness, the Contractor shall provide to the COR a written designation of an alternate representative, itemizing any limitations in the alternate's authority. An equally qualified alternate representative shall be provided to act on behalf of the supervisor during his/her temporary absence.
The Contractor’s Representative shall direct the Contractor’s work force and operations in strict accordance with the PWS requirements. The Contractor’s Representative shall be empowered with sufficient authority to enable him/her to meet conditions that arise in the day-to-day operations without undue delay.
The Contractor Representative and alternate shall be designated as key personnel and be able to read, write, speak, and understand English, and have full authority to act for the Contractor. The Contractor shall provide the telephone number of the Contractor Representative and alternate to the CO within ten calendar days after award of the contract.
The Contractor shall immediately notify the COR and CO in writing when a decision to terminate the Contractor Representative, the alternate representative, or an employee has been made.
Quality Control Program (QCP)
The Contractor shall establish a quality control program to assure the requirements of the contract are provided as specified. Copies of the Contractor’s QAP shall be provided to the COR at a preperformance conference. This program shall include, but will not be limited to the following:
A. An inspection system covering the services stated in the Task Schedule section. It must specify the areas to be inspected on a scheduled or unscheduled basis and the title of the individuals who shall perform the inspections.
B. A method for identifying deficiencies in the quality of services performed before the level of performance is unacceptable and for proposing any corrective action to be taken.

C. A method shall be included to ensure that all keys issued to the Contractor by the Government are not lost, misplaced, and not used by unauthorized individuals (7. Key Control, below applies).
D. A file of all inspections conducted by the Contractor and any corrective actions taken. This documentation shall be made available to the Government at all times during the term of the contract. This file shall be the property of the government and shall be turned over to the COR within ten (10) days after the end of each calendar quarter or upon end and/or termination of this contract.
E. Include a customer complaint feedback system for correction of complaints. All customer complaints shall be answered, and all necessary corrective actions taken within one workday, unless otherwise approved by the CO or Contract Specialist by written notice.
The Government will monitor the Contractor’s performance under this contract. Each phase of the services provided under this contract is subject to Government inspection during both the Contractor’s operations and after completion of the tasks. The Government’s Quality Assurance Program (QAP) and the Government’s Quality Assurance Surveillance Plan (QASP) are not a substitute for a QCP by the Contractor.
Operation Management Plan
The Contractor shall establish and maintain an Operation Management Plan for each building and updated with each option. At a minimum the Operation Management Plan shall include:
A. A schedule shall indicate the time of day, the type of work to be performed and its frequency of performance (daily, weekly, monthly, quarterly, semi-annually, and annually). Also, the schedule shall indicate which day weekly activities are performed;
B. A list of chemicals and compounds along with the required compliance with manufacture use and compliance with local, state, and federal regulation. No chemical or cleaning compounds or combination of chemicals or compounds shall be used that will damage, weaken, or discolor floors, floor coverings, woodwork, or furniture. Any cleaning or waxing material or combination of materials that do not comply with fire regulations shall not be used nor shall they be permitted in any building or any area covered by this contract. All cleaning supplies requested for use are subject to COR approval;
C. An understanding of all the areas to be worked;
D. Supervisor/Manager and employee lists and responsibilities;
E. Employee conservation practices i.e. the mechanical equipment controls for heating, ventilation, and air conditioning systems shall not be adjusted by Contractor employees;
lights shall be used only in areas at the time when work is being performed; water faucets or valves shall be turned off after the required usage is complete; lights and fans turned off when not in use;
F. Maintenance and Out-of-Order control measures: i.e. the Contractor shall notify the COR whenever a building deficiency is identified (e.g., chemical spill, leaking pipe, stopped up sink, power outage, doors or other equipment appear to be working improperly) and post an Out of

Order sign;
G. Personnel owned property control measures: i.e. the protection measures pertaining to articles of personal or monetary value found by the Contractor’s employees are turned into the
COR;
H. Key Control Program - the Contractor shall establish and maintain effective control of keys (7.
Key control, applies); and
I. Hazard Communication Training - the Contractor shall establish and conduct an effective Hazard Communication Training for Contractor and Contractor employees.
Consequences of Non-performance and Unsatisfactory Work
The Government will give the Contractor written notice of deficiencies via a Contract Discrepancy Report (CDR) whenever required services are not performed or the quality of work does not conform to the specifications of this contract. Repeated instances of “non-performance” or “unsatisfactory” work may be grounds for termination or default of this contract.
Contractor Personnel
All work under this contract must be performed by personnel who are experienced and completely qualified to provide professional janitorial services as required by the specific tasks.
The Contractor or designated representatives must be present on the job site at all times when work of this contract is being performed. The Contractor or designated representative shall receive and respond to instructions from the CO or his/her representative regarding work scheduling and contract compliance. Each employee shall be furnished with written instructions by the Contractor, which shall include operational procedures, frequency schedules, security procedures, and other information pertinent to the satisfactory performance of assigned duties.
On August 27, 2004, the President signed Homeland Security Presidential Directive – 12 (HSPD- 12, Reference DOI-AAAP-0081). The directive and subsequent implementation guidance mandates that all Federal employees and Contractors who are affiliated with an agency and who require access to federally controlled facilities undergo an identity proofing and registration process in addition to a background investigation in order to obtain a Personal Identity Verification (PIV) credential which is called the Smart Card. The Smart Card is the new Federal identification badge. To comply with this mandate, all Contractors working at the HBBS shall be required to complete this process.
Contractor substitute employees shall meet the necessary requirements as set forth in this WS at all times. Non-approved employees shall not enter the facility and shall not perform work for this contract. The CO or the COR shall be notified each time there is a change of personnel working in the Center. This notification shall be made before the employee starts work in the Center.
The CO or the COR may require removal of an employee(s) from the performance of this contract. Any of the Contractor’s employees deemed incompetent, careless, insubordinate or otherwise objectionable, or whose continued employment is contrary to the public interest may be removed. The Contractor shall assure that all employees are well trained in the peculiarities of Janitorial Services prior to job placement. An introduction of Contractor personnel to various facilities, systems, and requirements will be accomplished by Government personnel at the beginning of the contract. Thereafter, the Contractor shall be responsible for further training

necessitated by personnel turnover or contract modification.
The Contractor’s employees shall abide by HBBS and Federal policies.
Employees shall only park in main parking lot. Alcohol, drugs, and weapons are not allowed.
Contractor employees shall refrain from disturbing papers on desks, or opening drawers or cabinets. The Contractor or employees shall not bring children or pets to the premises while the work is being performed.
During completion of janitorial duties, the Contractor shall ensure all supplies are stored properly and all storage room doors are locked. All work, supplies, and equipment shall conform to the requirements set forth by the Department of Labor, Occupational Safety and Health Administration (OSHA). The Contractor shall take all necessary precautions for the protection of the work and safety of the public.
The Contractor shall comply with the regulations governing the operation of premises which are occupied and shall perform this contract in such a manner as not to interrupt or interfere with the conduct of Government business.
7. Key Control
The Contractor shall establish and implement methods to ensure that keys issued to the Contractor by the Government are not lost, misplaced, or used by unauthorized persons. The Contractor will be issued keys by the Government for access to necessary entry areas at the time of contract award.
The Contractor shall establish key assignment records and shall enforce quality controls for key control. No keys shall be duplicated. Upon completion, all keys shall be returned to HBBS. Any keys not returned shall be charged to the Contractor at a cost of $100.00 each by deductions from amounts due to the Contractor.
The Contractor shall report the occurrence of a lost key immediately to the COR. In the event keys are lost or duplicated, the Contractor shall, upon written direction of the CO, pay for rekeying or replacement of the affected lock(s) without cost to the Government. The Government may replace the affected lock(s) or perform-rekeying and deduct the cost of such from the monthly payment due the Contractor.
The Contractor shall prohibit the use of keys issued by the Government by any persons other than the Contractor’s employees. The Contractor shall prohibit the opening of locked areas by the Contractor’s employees to permit entrance of persons other than the Contractor’s employees engaged in the performance of assigned work in those areas.
8. Hazard Communication
The Contractor shall provide employees with Hazard Communication training. Employees shall not begin working under this contract until completion of Hazardous Communication Training.
Per OSHA 29 CFR 1910.1200 Hazard Communication Standard, this training also includes instruction on the appropriate handling of chemicals requiring a Material Safety Data Sheet.
The Contractor shall perform this contract in a safe manner, observing any necessary safety

precautions. The contractor shall promptly correct any condition that, in the opinion of the CO or COR, endangers the safety of personnel and/or property during the performance of this contract.
Failure to observe this requirement may result in contract termination, with the Contractor liable for any such damages that may occur. The Contractor is obligated to perform a thorough inspection each working day of all areas to assure that all necessary safeguards relative to safety and fire hazards are being taken.
The Contractor shall become familiar and require his/her employees to become familiar with the method of turning on a Fire Alarm. The Contractor and his/her employees shall be cognizant of and shall observe all requirements for handling and storage of combustible supplies and materials, and for daily disposal of combustible waste and trash. During a Fire Alarm or Fire, the Contractor’s employees shall be trained to properly respond in accordance with applicable Center instructions and procedures.
The Contractor shall maintain an accurate record of all accidents and shall report to the CO or COR all accidents resulting in death, trauma, or occupational disease. All accidents must be reported to the COR immediately. The Contractor shall provide a written accident report within
(2) work days of the accident.
9. Property and Personal Damage
The Contractor shall be responsible for all damage to property and to persons, including third parties that occur as a result of his or her agent’s or employee’s fault or negligence. The term “third parties” is constructed to include employees of the Government. In all instances where Government property or equipment is damaged, a full report of the fact and extent of such damage shall be submitted in writing to the CO or COR within twenty-four (24) hours.
10. Government Furnished Property (GFP)
The Government will provide the necessary cleaning supplies to be used under this contract.
Contractor will submit a written request to the COR when supplies are low and need to be reordered.
11. Janitorial Closets
Facilities and storage spaces (where they exist) will be made available to the Contractor.
Facilities and spaces assigned to the Contractor shall be maintained by the Contractor in a clean and sanitary manner. Light, heat, electricity, water, and local telephone service as required for the performance of this contract will be made available to the Contractor. The CO or the COR shall have access to the facilities, spaces, and equipment at any time for inspection or for other purposes as required. At the completion of this contract, the janitorial areas assigned shall be returned in the same condition or better than existed at the beginning of the contract.
TASKS AND DELIVERABLES
The Contractor shall provide the following tasks to the following standards resulting in the following deliverables:

Task 1: The Contractor shall clean general, office, stairways, and lobby spaces.
Standard: Office space, including conference rooms, lobby, corridors, and entranceways, shall be free of surface dust, dirt, debris, carpet stains, trash, streaks, and smudges, and present an overall appearance and smell of cleanliness. Horizontal surfaces less than 70 inches shall be free of dust, dirt, oil spots, or smudged surfaces. Surfaces in plain sight above 70 inches shall be free of dust and insect debris. Trash shall be collected from work areas and placed in the trash receptacles. Boxes or cartons whether empty or full of discarded materials that have been placed alongside the regular waste receptacles or marked as trash shall be removed with other wastes.
Mixed paper, corrugated cardboard, and glass are currently recycled; other items may be added due to further regulations. These separated materials are to be disposed of in their proper receptacles. Trash containers shall have plastic liners and be kept free of trash. Molding shall be free of dirt, debris, and all types of marks. Desks with papers, computers, and keyboards shall not be dusted. The Contractor shall vacuum all ventilation grilles and outlets, per the Task Schedule, using a vacuum and attachments. Unusually dirty grilles shall be damp-wiped clean.
Carpets, rugs, and hard floors shall be free of removable spots, stains, soiled traffic pattern(s), dirt, debris, gum, and crusted material. Equipment used shall remove dirt and soil without damaging or causing rapid deterioration of the carpets, rugs, and hard floors being serviced.
When damp- or wet-mopping, the Contractor shall use neutral soap solutions. Floors shall be dried after wet mopping to prevent any standing water from being absorbed by floor material or seeping into seems of the floor covering. Mop water splashed on baseboards, doors, furniture, and equipment shall be removed immediately; special care shall be used in mopping stairs to prevent water from dripping. The Contractor shall move any furniture necessary to complete the tasks and replace when work is completed.
Deliverable: The Contractor shall deliver clean offices as provided in the attached schedule.
Task 2: The Contractor shall clean restrooms.
Standard: Restrooms shall be cleaned with a disinfectant cleaner. Fixtures, metal chrome surfaces, washbasins, shower stalls, mirrors, counter tops, deep sinks, and dispensers shall maintain a high level of luster, if applicable, and be free of dust, mold, mildew, streaks, rust, and encrustation. Toilet bowls shall be cleaned as far into the trap as possible and under the inside rim with disinfectant cleaners. The commode seat, exterior of commodes, and all adjoining fixtures shall be rinsed and wiped dry upon completion of cleaning. Shower stalls shall be washed and cleaned with a disinfectant cleaner. A neutral soap solution or commercial product specifically made for the cleaning purpose shall be used; stubborn stains shall be removed with a mild nonabrasive cleaning product. After washing, fixtures shall be rinsed with clear water and dried thoroughly. Partitions, doors, vents, sills, and walls shall be free of dust, and dirt. The full surface area of all stall partitions, doors, and waste receptacles shall be damp wiped with a disinfectant cleaner.
NOTE: A special set of sponges, cloths, brushes, mops, etc. shall be used for cleaning toilets and urinals and SHALL NOT be used for cleaning any other portion of restroom facility. Toilet cleaning materials shall be clearly marked for those purposes and shall not be used for other purposes.
The Contractor shall re-supply restrooms so that supplies do not run out. This includes but is not

limited to: toilet paper, paper towels, toilet/urinal deodorant cakes, and hand soap. Adequate quantities of dispenser supplies shall be available at all times, including weekends and holidays.
Sanitary napkin disposal containers shall be emptied, cleaned, disinfected, and lined daily. Paper towels shall be placed in the dispenser with the folded side facing downward, so the towels can be removed easier. Restrooms shall be free of discarded material and waste receptacles shall be emptied daily. Trash shall be collected and put in the trash receptacles. Waste receptacles shall be lined and kept relatively free of trash, dirt, stains, and debris. Flooring and base molding shall be free of dirt, debris, all types of marks, and foreign matter; flooring shall be damp mopped using disinfectant cleaner.
Fixtures shall be free of smudges, marks, spots, or unsightly discoloration. Toilet bowls and urinals shall be de-scaled and the entire surface shall be free from streaks, stains, scale, scum, urine deposits, and rust stains. Floors shall be free of visible loose dirt or streaks, scuff marks, heel marks, other stains and discoloration, and mop strings. Corner, crevices, moldings, and ledges shall be free of all dust and dirt. Flooding of floors shall be avoided at all times. Surfaces shall be left dry. Sufficient supplies shall be provided to meet usage.
The Contractor shall identify any malfunctioning, broken, missing, or unusable toilet equipment such as soap dispensers, towel dispensers, or other maintenance requirement(s) to the COR for maintenance services or replacement and installation.
Deliverable: The Contractor shall deliver clean restrooms as provided in the attached schedule.
Task 3: The Contractor shall clean all common areas to include drinking fountains, kitchen, and entrances.
Standard: Kitchen and break room shall be free of obvious dirt debris and extraneous matter.
Trash shall be collected daily and placed in the trash receptacle, dumpster, outside. Trash containers shall be lined and kept free of trash, dirt, stains, and debris. Refrigerators, counters, furniture, stove, refrigerator, and sink surfaces shall be cleaned daily with a disinfectant cleaner.
Surfaces shall be free of dust, dirt, mold, mildew, streaks, rust, and encrustation. Molding shall be free of dirt, debris, and all types of marks. Flooring shall be free of dirt, debris, foreign matter, and damp mopped using a disinfectant cleaner.
Floors shall be clean and free of trash and foreign matter with no dirt or dust left in corners, crevices, molding, and be free of streaks, and mop strand marks.
The Contractor shall empty all wastebaskets, cigarette butt receptacles, and other trash containers within the area and return them to their initial locations. Trash containers shall be odor free and contain clean plastic trashcan liners. Boxes, cans, and papers that are placed near a trash receptacle and marked “TRASH” shall be removed by the Contractor. The Contractor shall pick up any trash that may fall in or around the facility or grounds during the removal of collected trash. The Contractor shall deposit the trash in a trash receptacle, or dumpster, outside in such a manner as to not have trash spilling or blowing out. The Contractor shall remove all trash, including the boxes, resulting from replacement of various special occasions, such as office staff moving from one building to another, delivery of a large amount of supplies, furniture, or equipment. Trash receptacles shall be emptied when the trash level is half-full and new plastic bag liners installed when emptied. Mixed paper, corrugated cardboard, and glass are currently recycled; other items may be added due to further regulations. Containers will be supplied by the Government.

The Contractor shall wash all drinking fountains with a neutral soap solution, rinse with clear water, and wipe dry with a clean cloth.
Deliverable: The Contractor shall deliver clean commons areas in accordance with the timeliness described in the Cleaning Schedule.
Task 4: The Contractor shall maintain floors by sweeping, mopping and buffing.
Standard: The Contractor shall maintain the floors by sweeping, mopping and buffing all floors, stairways, except carpeted areas, shall be swept, dust mopped, damp mopped, wet mopped, dry buffed, and spray buffed, as needed, to ensure they have a uniform, glossy appearance and freedom from dirt, debris, dust, scuff marks, heel marks, other stains and discoloration, and other foreign matter. Baseboards, corners, and wall/floor edges shall also be clean. All floor maintenance solutions shall be removed from baseboards, furniture, trash receptacles, etc. Chairs, trash receptacles, and other moveable items shall be moved to maintain floors underneath these items. All moved items shall be returned to their original and proper position.
Deliverable: The contractor shall deliver clean and highly buffed floor surfaces as directed in the Task Schedule.
Task 5: The contractor shall clean glass surfaces.
Standard: Clean interior glass; after glass cleaning, there shall be no traces of film, dirt, smudges, water, or other foreign matter. All stains will be removed including metal frames and doors.
Deliverable: The contractor shall deliver clean glass surfaces as directed in the Task Schedule.
Task 6: The contractor shall clean carpets and upholstered chairs.
Standard: Provide carpet cleaner machine and cleaning solution to scrub and clean carpeted areas and chairs in office building once during the month of December.
Deliverable: The contractor shall deliver clean carpets and upholstered chairs in the office building.
SPECIFICATIONS AND CONDITIONS
1. The Government shall furnish the necessary equipment and supplies, including but not limited to the following: vacuum cleaner(s) and attachments; floor buffer(s); wax stripper(s); floor wax, toilet bowl cleaners; disinfectant; window and glass cleaner; cleaning brushes, mops; cleaning pads; other cleansers as required; toilet paper; paper towels; hand and liquid soap; napkins; toilet cakes; and trash can liners.
2. Floors in offices and shared areas are carpeted and vinyl floor covering; floors in restrooms;
hallways, entry area and stairwells are tile.
3. Under this contract all cleaning shall be done after working hours (8:30 am to 4:30 pm), Monday through Friday (except Federal holidays), unless agreed otherwise, and in accordance

with the attached work schedule.
4. ALL LABORATORIES AND FISH HOLDING AREAS ARE OFF-LIMITS TO
CONTRACTOR PERSONNEL. Please see attachments of floor plans to designate off limit areas.
5. At least ten (10) days prior to commencing work, the Contractor shall meet in conference with the CO or COR at a time mutually agreeable to discuss and develop mutual understanding relative to scheduling and administration of the work required by this contract.
6. The Contractor’s representative shall meet with Government representatives weekly during the first month of the contract. Meetings shall be held as often as necessary thereafter as determined by the CO or COR. The purpose of these meetings shall be to resolve identified problems, to schedule required work, to establish or adjust priorities, and to evaluate Contractor performance for payment purposes.
7. Upon commencement of work under this contract, the Contractor shall implement and maintain a safety program for employees performing work under this contract. This detailed program shall comply with all Federal and State Occupational and Safety and Health Acts and all safety provisions within this contract. The program shall identify the Contractor Safety Representative and Environmental Manager. The Contractor shall place and remove appropriate warning signs for wet or slippery floor areas caused by cleaning or waxing applications. The Government provided warning signs will be in accordance with OSHA Safety Standards. Cloths, mops, or brushes containing a residue of wax or other combustible material subject to spontaneous ignition shall not be stored inside the building. The Contractor shall provide the COR with copies of Material Safety Data Sheets (MSDS’s) for all cleaning chemicals used. The Contractor shall maintain and distribute the MSDS to employees.
8. Inspections shall be conducted frequently and unannounced by the Contracting Officer's Representative (COR). Work not conforming to specifications will be brought to the attention of the Contractor for correction. A daily diary of problems and vendor contact information will be maintained by the COR and a copy submitted to the Contracting Officer for administrative action.
Acceptance of the work shall be based on compliance with WS.
9. All the Contractor’s employees are required to attend an orientation conducted by the COR.
The orientation will include an explanation of the occupant agency’s function and a tour of the facility. Information will also be presented to familiarize contract employees with key client agency personnel and areas of the building/facility requiring special attention and restrictions.
10. The Contractor shall make sure employees practice utilities conservation. Contractor shall operate under conditions that prevent the waste of utilities including turning off lights when not in use, turning off water faucets or valves when not in use, and ensuring building is secured at work’s end.
ACCEPTANCE LEVELS
The Contractor service requirements are summarized as performance objectives that relate directly to contract service requirements. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to performance success.

BASIC CLEANING SERVICES
Main Floors: All floors, except carpeted areas, shall be swept, dust mopped, damp mopped, wet mopped, dry buffed, and spray buffed, as needed, to ensure they have a uniform, glossy appearance and are free of dirt, debris, dust, scuff marks, heel marks, other stains and discoloration, and other foreign matter. Baseboards, corners, and wall/floor edges shall also be clean. All floor maintenance solutions shall be removed from baseboards, furniture, trash receptacles, etc. Chairs, trash receptacles, and other moveable items shall be moved to maintain floors underneath these items. All moved items shall be returned to their original and proper position.
Trash Removal: All trash containers shall be emptied and returned to their initial location. Boxes, cans, and papers placed near a trash receptacle and marked “TRASH” shall be removed. Any obviously soiled or torn plastic trash receptacle liners shall be replaced. The trash shall be deposited in the nearest outside trash collection container. Trash receptacles shall be left clean, free of foreign matter, and free of odors. Mixed paper, corrugated cardboard, and glass are currently recycled; other items may be added due to further regulations. Containers will be supplied by the Government.
Clean Glass: Clean interior and exterior glass after glass cleaning; there shall be no traces of film, dirt, smudges, water, or other foreign matter. All stains will be removed including metal frames and doors.
Clean Drinking Fountains: Clean and disinfect all porcelain and polished metal surfaces, including the orifices and drain, as well as exterior surfaces of fountain. Drinking fountains shall be free of streaks, stains, spots, smudges, scale, and other obvious soil. Drinking fountains shall be cleaned with a neutral soap solution, rinsed with clear water, and wiped dry with a clean cloth.
Vacuum Carpets: Vacuum carpeted areas. The carpeted area shall be free of all visible dirt, debris, litter, and other foreign matter. Any spots shall be removed by carpet manufacturer’s approved methods as soon as noticed. All tears, burns, and raveling shall be brought to the attention of the Government representative. Area and throw rugs, and mats are included to receive this service. Equipment used shall remove dirt and soil without damaging or causing rapid deterioration of the carpets and rugs being serviced. The Contractor shall remove and replace all necessary furniture and equipment before and after vacuuming.
General Spot Cleaning: Perform spot cleaning on a continual basis. Spot cleaning includes, but is not limited to removing, or cleaning smudges, fingerprints, marks, streaks, spills, etc., from washable surfaces of all walls, partitions, vents, grillwork, doors, door guards, door handles, push bars, kick plates, light switches, temperature controls, and fixtures. The surface shall have a clean, uniform appearance, free of streaks, spots, and other evidence of soil.
General Dusting: All horizontal surfaces shall be dusted or cleaned to eliminate dust collection.
All countertops and tabletops shall be free of all visible dust, debris, litter, and other foreign matter. It is NOT the Contractor’s responsibility to dust or care for the tops of desks or office machines.
Basic Restroom Cleaning Services; Clean & Disinfect: Completely clean and disinfect all surfaces of sinks, toilet bowls, base of toilet, urinals, lavatories, dispensers, plumbing fixtures, PAGE OF PAGES partitions, dispensers, doors, door handles, walls, showers, and other such surfaces, using a germicidal detergent. Receptacles shall be free of deposits, dirt, streaks, and odors. Disinfect all surfaces of partitions, stalls, stall doors, entry doors, (including handles, kick plates, ventilation grates, metal guards, etc.), and wall areas adjacent to wall mounted lavatories, urinals, and toilets.
All floor and sink drain traps shall be free of debris, residue, and odors. Clean and disinfect all porcelain and polished metal surfaces, including the orifices and drains.
Descale Toilet Bowls and Urinals and Showers: Descaling shall be performed to keep areas free of Buildup i.e. soap films, hard water deposits and other build-up. Surfaces shall be free from streaks, stains, scale, scum, urine deposits, marks made from equipment use, and rust stains.
Sweep and Mop Floor: The entire floor surface, including grout, shall be free from litter, dirt, dust, and debris. Grout on wall and floor tiles shall be free of dirt, scum, mildew, residue, etc.
Neither dirt nor dust will be left in corners, under air/heat floor vents, under furniture, behind doors, or on stairs. Floors shall have a uniform appearance without streaks, swirl marks, detergent residue, or any evidence of soil, stain, film, or standing water. Moveable items shall be tilted or moved to sweep and damp mop underneath. When mopping, the Contractor shall use a neutral soap solution as necessary. Floors shall be dried after wet mopping to prevent any standing water from being absorbed by floor material or seeping into seems of floor covering. Floors shall be stripped, scrubbed, waxed, etc., as necessary, to maintain sanitary conditions and a clean, uniform appearance.
Stock Supplies: Contractor shall ensure restrooms and kitchen are stocked so that supplies including soap for the soap dispensers, toilet tissues, and paper towels, do not run out. Supplies shall be stored in designated areas. No overstocking shall be allowed. If supplies run out prior to the next service date, Contractor shall refill on the day of notification.
Contractor Quality Control Plan: The Contractor shall establish and maintain a complete Quality Control Program (QCP) that is acceptable to the CO to assure the requirements of the contract are provided as specified. This QCP shall be submitted at the time of proposal submission and updated as required.
QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)
The Government’s Quality Assurance Surveillance Program is not a substitute for Quality Control by the Contractor.
QUALITY ASSURANCE
The Government will monitor the Contractor’s performance in each functional area under this contract and reserves the right to use additional surveillance procedures as is deemed appropriate.
If the Contractor fails to complete a portion of work, or the work performed is unsatisfactory for any given service or portion thereof, the Contractor shall correct the deficient work at no additional cost.
The Government shall monitor the Contractor’s performance of services using random sampling, verified customer complaints, review of Contractor’s Quality Control documentation, or 100% inspection method. Customer complaints shall be substantiated by the COR for validity. Should the COR determine that the complaint is valid, the COR shall inform the Contractor orally and in

writing and give the Contractor additional time to correct the defect, if additional time is available. The COR shall determine how much time is available based on the complaint. Should the Contractor disagree with the complaint after the investigation of the site, the Contractor shall notify the COR; the COR shall review the matter to re-examine the validity of the complaint.
Unscheduled inspections shall be performed at any time. The Government reserves the right to alter or change the type of inspection at its discretion at any time.
The Government will periodically assess the Contractor’s overall performance on this contract.
The Contractor shall be afforded an opportunity to comment on each evaluation performed. The Contractor has seven (7) calendar days to submit written comments on unsatisfactory ratings.
PERFORMANCE EVALUATION MEETINGS
The contracting officer may require the contract manager to attend a random inspection conducted by the COR and to meet with the contracting officer, COR, and other Government personnel as deemed necessary. The Contractor may request a meeting with the contracting officer when the Contractor believes such a meeting is necessary. Written minutes of such meetings shall be recorded in the contract and signed by the contract manager and the contracting officer. If the Contractor does not concur with any portion of the minutes, such nonoccurrence shall be provided in writing to the contracting officer within five (5) calendar days following receipt of the minutes.
