The proposed work consists of the contractor shall provide all labor, materials, transportation, supervision, and the proposed work consists of the contractor providing all labor, materials, transportation, supervision, and incidentals necessary to remove and install floor carpet and tiles at Southwest Indian Polytechnic Institute (SIPI) in Albuquerque, New Mexico, in the accordance with the Statement of Work.
Period of Performance: (120) calendar days from the issuance of the Notice to Proceed.

Replacing Carpet and Ceramic Tiles with
Luxury Vinyl Floor Tile at the SIPI Residential Halls Southwestern Indian Polytechnic Institute
Albuquerque, NM 87120
1.0 General Requirements
1. Introduction
Southwestern Indian Polytechnic Institute has identified remodeling needs in residential halls (Golden Eagle Lodge and Four Winds Lodge). This service will require carpet removal/ceramic tile removal and install commercial grade vinyl luxury tile and cove base in buildings #113 and #114.
2. Location
Southwestern Indian Polytechnic Institute is located at 9169 Coors Road, Albuquerque, NM 87120. Golden Eagle Lodge identified as building #114 and Four Winds Lodge identified as building #113.
Direction: Contracting Officer Representative (COR) will oversee project and all correspondence will go directly through Contracting Officer for all approval. Changes to ‘scope of work’ will be directly brought to the attention of Contracting Officer. Contractor shall accept directions only from the Contracting Officer (CO) or the designated Contracting Officer Representative (COR). CO must be informed in writing of any changes to the scope of project other provisions within Scope of Work (SOW).
3.0 Performance Time
Performance period associated with this project will be for 120 calendar days after final approval/issuance of purchase order. COR has the authority to revise purchase order as deemed necessary. Work to be completed from Monday – Friday. Government personnel will be present if contractor work to be completed during Government business time.
4.0 Progress Chart
Work progress will be reported and submitted to COR for approval and updates of percentage of work completed will also be submitted with total cost. Dates of work started and completed will be given to COR for review and approval to ensure scheduled completion. Contractor shall commence work under this contract promptly, execute the work diligently, and achieve final completion and acceptance including final cleanup of the premises within the contract period specified and updated progress will be submitted with each payment estimate.
Protection of Existing Facilities and Work: Facilities will be kept clean and free of damage from contractor.
COR will be notified of any property damage from contractor. The Contractor shall be liable for the damages caused by the Contractor's negligent performance of any of the services furnished under this contract.

2.0 Scope of Work
Contractor will provide/furnish adequate technical and coordinators to ensure scheduled completion of project as proposed. Background checks will be coordinated by COR before Contractor is allowed on campus.
Appropriate documents will be provided by COR for immediate completion. A pre-planning meeting will occur before the Notice to Proceed is signed by contracting office which will consist of all parties involved in the contract.
Assessment will be conducted on-site to determine conditions. All discrepancies/changes will be reported by Contractor to COR. Weekly completion schedule will be submitted to COR and percentage of project completion will be delivered to COR. Appropriate adjustments will be made based on submitted completion weekly schedule.
Floors will be prepared by Contractor by removing existing carpet, cove base and ceramic tiles to include adhesive in both residential halls.

Golden Eagle Lodge: C-wing Corridor: 1382 sq. ft.; lounge: 677 sq. ft.; recreation: 1,470 sq. ft.; lab corridor:
94 sq. ft.; and D-wing corridor: 1366 sq. ft. Four Winds Lodge: A-wing corridor: 1382 sq. ft.; lounge: 677 sq.
ft.; recreation: 1,470 sq. ft.; lab corridor: 94 sq. ft.; and D-wing corridor: 1366 sq. ft. Cove base for Four Winds Lodge and Golden Eagle Lodge: 2,000 linear feet per residential hall.
Contractor will take necessary precautions to remove carpet, ceramic floor tiles and adhesive. Disposal of old carpet and ceramic floor tiles is the responsibility of contractor. During the removal of existing carpet and ceramic floor tiles and installation luxury vinyl tiles, Contractor will ensure personnel is wearing appropriate Personal Protection Equipment (PPE). Contractor will provide necessary PPE to personnel.
Drawings/Plans: Contractor shall submit to COR, prior to ordering any material, for review and approval of all proposed materials, styles and quantity.
Preparation: Luxury vinyl tiles to be used shall meet or exceed all applicable National Fire Protection Association and Fire Code & Life Safety Code requirements. Adhesives and sealers shall be of low or no
P a g e 23 | 96 volatile organic compound. Any portion of the luxury vinyl tiles labeled or unlabeled as seconds, second quality, flawed, irregular, factor irregular, and/or returns are not acceptable.
In an attempt to mitigate returns visits to the work area, every effort should be made by the Contractor to remove all hardware that is not to be replaced over with transition reducer or edge trim and moldings applied before commercial grade LVT is installed so that commercial grade LVT is fitted securely. Such items include but are not limited to plates, baseboards, moldings, rubber and metallic reducer/edge/trim/transition kits and similar items already installed that are to be replaced. Removal of these items are required to completely apply adhesive and commercial grade LVT under the items and adjacent surfaces receiving commercial grade LVT and reduce the amount of cuts required.
The Contractor shall schedule and perform cleaning of substances (including oil, existing adhesives, greases, and other foreign matters) from all substrates that could impair the bond of the various commercial grade LVT systems or cause transmitting before actually applying new adhesive so that no cleaning agents or treatments, contaminants, old carpet/tile backing, adhesive, dirt, dust, or debris developed from the cleaning and surface preparation process will fall onto or remain on the subsurface or adjacent area causing a degradation in overall appearance, visible damage, soiling, adhesive failure, delaminating and dimensional stability.
Inspect surfaces and fill all cracks, gouges, holes, and abrasions with compounds suitable for each substrate (e.g. spackle, plaster, or cement patching compound). No adhesives shall be applied on areas until the substrates are cured and dry.
From the point of manufacture, sale, pre or post installation, if the Contractor or Contracting Officer discovers or determines any portion of the supplied luxury vinyl tiles to contain nicks, cuts, blemishes, discoloration, crooked sides, veins, spots, or any condition or flaw which is not consistent with First Quality, the Contractor shall remove and return the defective portion of the tiles to their supplier and supply/re-install any portion of the luxury vinyl tiles at no cost to the Government.
Adhesives and sealers shall not be thinned or altered in any way, shape, fashion or form. Containers not displaying manufacturer’s product identification will not be acceptable.
3.0 General Requirements
3.1 Contractor’s Qualification
Contractor shall have at least Five (5) years of experience in the business of installing and repairing commercial grade vinyl of all types. If the Contractor subcontracts any of the work, the Subcontractor shall have at least (5) years of experience in the business of installing and repairing commercial vinyl tiles.
3.2 Communication and Coordination
Contractor shall maintain an effective Communication and Coordination Policy with COR and SIPI utilizing email, telephones, mobile phones, faxes, cameras etc. to ensure the COR is kept abreast of current condition of the jobsite for the term of the Contract. The Contractor’s key employees shall carry a mobile phone while performing all services at SIPI residential halls.
3.3 Contract Manager

1) Contractor shall provide a Contract Manager. The Contract Manager shall have full authority to act for the Contractor and serve at all times to carry out all provisions of the Contract.
2) Contract Manager shall have a minimum of Three (3) years of experience in successfully managing Contracts of the size and scope described in this Statement of Work.
3) The name of the Contract Manager and a qualified alternate who shall assume the Contract Manager’s duties when the primary Contract Manager is absent shall be designated in writing to the COR Fifteen (15) business days prior to the Contract start date.
4) The Contract Manager shall be available at all times while the Contractor’s personnel are on the job-site.
5) The Contract Manger shall meet with the COR at the COR’S request in accordance with this Statement of Work.
3.4 Contractor’s Supervision
1. Contractor shall provide on-site for all work performed at the residential halls, a Supervisor with background clearance.
2. Supervisor shall also be able to communicate clearly and understandably (both verbally and in writing) with all of its employees and Subcontractors to provide instruction and direction.
3. Contractor’s employees and Subcontractors shall understand verbal and signage warnings pertaining to safety and security.
3.5 Contractor Personnel
1) Contractor’s employees shall at all times while on the jobsite, whether on or off duty, shall conduct themselves in a professional, orderly and safe manner. Rudeness, fighting, being under the influence of alcohol or drugs, gambling, soliciting, stealing, taking pictures or bringing cameras devices anywhere on Government property (unless fulfilling the requirements of this Contract), and any immoral or otherwise undesirable conduct shall not be permitted.
2) Contractor agrees to transfer promptly from the SIPI property any employee or employees that SIPI and/or COR advises are not satisfactory and to replace such personnel with employees satisfactory to the Government; but in no event shall SIPI be responsible for monitoring or assessing the suitability of any employee of the Contractor.
4.0 Planning and Estimating
All facets of the planning, estimating, removal, installation, warranty, and repair are coordinated with the COR. Upon arrival, the Contractor should be fully capable of identifying existing carpet, carpet removal methods, luxury vinyl tiles, quantities, adhesives, sealers, rubber and metal reducer/edge trim, transition moldings. Baseboards, seam locations, wall base types and other accessories.

The Contractor shall discuss manufacture’s literature and provide samples to the COR for approval prior to commencing luxury vinyl tile installation. Other materials incorporated into the work shall also be approved by the COR prior to installation.
The Contractor shall notify the COR during planning and estimating about anticipated problems using the materials specified over existing substrates.
Contractor shall notify the COR immediately (verbally and in writing) of all damage and/or discrepancies identified at the job-site prior to the start of work as well as any property damage that occurs while performing this SOW.
The Contractor shall adhere strictly to all manufacturers’ recommendations with respect to temperature and surface conditions set forth for the particular product literature unless otherwise approved in advance by the COR. Nothing in the SOW shall be deemed to reduce the manufacturers’ recommendations.
The Contractor shall examine all substrate conditions under which commercial grade LVT will be installed to ensure compliance with the commercial grade LVT manufacturer’s requirements and compatibility for all applications of adhesives and floor preparation guides for various and applicable substrates.
