About this file

This is a seller checklist document issued by the City of Philadelphia Procurement Department for small order purchases submitted through the PHLContracts platform. The checklist applies to a micro purchase for knit caps (beanies) in navy blue to be awarded to the lowest responsive and responsible bidder in Pennsylvania. The award will be made as a whole or by line item, whichever serves the City's best interest. Sellers must quote all items to be eligible for award, and no delivery or services should commence until official notification from the department is received.

The checklist outlines a 13-step submission process requiring sellers to maintain the Seller role, answer all mandatory questions, enter unit costs for each item, accept terms and conditions, and submit required documentation including an executed Consent & Authorization form. Optional submissions include the LGBTQ Applicant Opportunity Data Form. The Local Business Entity (LBE) Program does not apply to small order purchases, though sellers must answer the LBE certification question to proceed. All validation warnings must be resolved before quote submission through the PHLContracts portal at www.PHLContracts.phila.gov.
