AMENDMENT # A001
ATTACHMENT 6
1- Requested Documentation:
Government-issued photographic national identification card Completed SF-85 / Pages 1,2 and 6 (see attachment) Passport photograph Copy of the biographical page of a passport (if available) Police Record Residential certificate from a Mayor or municipality (Moukhtar) Lebanese Armed Forces Card (Draftee ID) if compulsory service was served Marriage or Divorce Decrees (if applicable) Government of Lebanon work permit
2- Biographical Information:
Full Name Parents’ Names Date and Place of Birth Civil Registry Location and Number Contact Information (e.g. cellular and home phone numbers)
The contractor shall comply and abide by embassy security requirements and procedures:
1- The contractor shall provide the above documentation and biographical information for all technicians, drivers, laborers, site engineers, managers etc. working under this contract to the Contracting Officer Representative, after being awarded the contract for access authorization. When security clearances are completed (usually 4-6 weeks after the submission of information), the Contracting Officer will notify the contractor the project start date by sending him a Notice to Proceed (NTP) letter. Any addition to the initial list must allow 4-6 weeks for necessary security for access authorization. Only personnel with legal Lebanese documentation or work permits can be employed for this project.
2- A list of the details of vehicles, cranes, trucks, etc.., needed for the operation of the work, as well as the driver/s names of these vehicles, must be submitted at least two days before the arrival of the vehicle to the compound for security check-up. The number of contractor vehicles that will be allowed access to the compound will be kept to an absolute minimum.

3- The materials, equipments and tools shall be inspected by the Embassy Security Force (ESF) guards, prior to the admittance to the compound. Any heavy equipment required for the project should be kept on the compound until it is no longer needed so as to preclude having to search it repeatedly.
4- Trucks filled with bulky materials (sand, gravel, cement bags, etc...) will not be allowed access to the compound except when escorted by an ESF guard before and after their loading. Otherwise, the contractor shall be required to move the cargo onto the embassy stake truck and to site using his resources.
5- All contractor personnel and their possessions will be pre-screened at the Access Control
Facility (ACF), before being allowed to enter the embassy. All contractor personnel will be required to submit a photo ID before receiving an embassy ID badge. These badges must be worn at all times while on the embassy compound.
6- No weapons, radios, mobile phones, cameras, or tape recorders can be brought into the embassy.
7- The contractor will not have access to the embassy parking lot and will be required to transport his personnel to and from the embassy daily.
8- Any losses due to delays that may arise from the procedures mentioned above, shall not in any case, be compensated by the embassy.
