VHAPM Part 813.106 Page 1 of 2 Original Date: 08/30/17 Revision 02 Eff Date: 03/31/2020

DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C246-21-AP-2638

1. Contracting Activity: Department of Veterans Affairs, NCO 06, Hampton VA Medical Center, 100 Emancipation Dr, Hampton, VA 23667 Purchase Request No. 659-21-2-042-0090 for W.G Hefner Salisbury VA Medical Center.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The W.G. Hefner VAMC in Salisbury , NC has a requirement to procure an Intellivue Guardian Monitor System. This brand is required by the MedSurg Unit as an addition to add another floor for patient care.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: The Intellivue Guardian Monitor System is the only brand that will connect to the existing Philips Guardian System currently being used throughout the Salisbury VAMC. This system compiles patient data into a database and analyzes it to predict deterioration up to 24 hours in advance. The medical center must utilize compatible devices as well as to standardize for staff training, resident training, and provider training. Compatibility and standardization of the device is key to safe and efficient patient care. It is in the best interest of the medical center to have compatible equipment to reduce the potential for operational error and utilize current inventory of accessory supplies. Obtaining additional accessories and supplies would be cost prohibitive. Compatibility and standardization are an important aspect of a successful surgical practice and ultimately enhance patient safety.

4. Description of market research conducted and results or statement why it was not conducted:

Research was conducted using VIP, FPDS, NSSL, NAC, GSA, and Internet. A VIP using NAICS 339112 and keywords “patient monitor” revealed that there are five (5) SDVOSB and no VOSB vendors but only one (1) is capable of providing the products that can meet this requirement.

The OEM, Philips, is the sole proprietor of this Intellivue Guardian Monitor System and they have two (2) authorized distributors, Alliant Healthcare and Four Points Technology.
