About this file

This is a Single Source Determination and Findings (D&F) document under simplified procedures for a noncommercial acquisition by the Department of Veterans Affairs Network Contracting Office (NCO) 10 at the Dayton VA Medical Center in Dayton, Ohio.

The acquisition concerns the replacement of two faulty tap changers at the Dayton VA Medical Center that have been causing voltage fluctuations and damage to electrical equipment. The tap changers are approximately 30 years old (from the mid-1990s) and cannot be repaired. The government cost estimate is redacted. The single source award is justified under FAR 13.101(b) based on urgency and criticality—the tap changers present an imminent risk of failure that could result in erratic voltage facility-wide, causing equipment damage and directly impacting patient care, particularly during high summer cooling demands. Utility Instrumentation Service (UIS) identified the problem and has offered a reasonable and timely remediation plan. The Acquisition Plan Action ID is 36C250-26-AP-4405, and the document was signed by Contracting Officer Brian Rosciszewski on 7/16/26. The document is Revision 02 with an effective date of 12/30/2025.

Single Source Determination and Findings under Simplified Procedures for Noncommercial Acquisitions

Department of Veterans Affairs

Network Contracting Office (NCO) 10

Determination and Finding for Single Source Awards IAW FAR 13.101(b)

Acquisition Plan Action ID: 36C250-26-AP-4405

Identification of the agency and the contracting activity:

Dayton VA Medical Center 4100 West Third Street Dayton, OH 45428-9000

Description of services needed:

The Government cost estimate for the above services is $XXXXXX.

The voltage at the subject facility has been experiencing fluctuations causing damage to electrical equipment. The cause of the voltage issue was identified as two tap changers being faulty and in need of replacement. The tap changers are old (presumably from the mid-1990s) and are unable to be repaired. The tap changers must be replaced as quickly as possible to prevent additional equipment damage or “brown outs” which would directly impact patient care.

Authority:

FAR 13.101(b).

Supporting rational for single source:

This acquisition is limited to a single source due to the urgency of fixing these critical components. The program office is concerned that these tap changers present an imminent risk of failure, particularly as the facility faces elevated electrical demand to maintain HVAC performance during the hot summer months. Should they fail, the voltage to the entire facility would be erratic which would cause damage to equipment and impact patient care. Utility Instrumentation Service (UIS) has identified the problem and offered a reasonable and timely plan to correct it. As a result, this acquisition is limited to a single source, UIS.
