About this file

This document is an Addendum Memo issued by the City of Atlanta Department of Procurement for RFP/PS/EXE/2403-1240271, a contract opportunity for Mobile Community Referrals and Diversion services. The addendum incorporates responses to 12 questions submitted by potential proponents/bidders. It confirms that the proposal due date of 5/16/2024 has not been modified, and all other pertinent information remains unchanged. The contract opportunity seeks qualified vendors to provide citywide mobile referral and diversion services, with a desired launch date of 7/1/2024. The scope of work includes objectives related to mobile outreach, short-term and long-term support services, and data reporting requirements.

The budget for this project is not specified, as it is a Request for Proposal. There is an incumbent provider for these services. The addendum provides clarification on submittal requirements for the Financial Responsibility Review, as well as details on the target areas, service types, and program referrals. Responses indicate that services may include both behavioral health and physical health/wellness supports, with a person-centered, holistic approach based on the individual's needs.

Project Number & Title: RFP/PS/EXE/2403-1240271,1/ Mobile Community Referrals and Diversion.

Date: 4/30/2024

Questions and Responses The following questions and /or clarifications were requested by various proponents:

1. Question Under the Financial Responsibility Review section (Form 3) Attachment A, does our financial institution need to copy the content on their letterhead and sign it, and then does our organization also copy the content and put it on our letterhead, and the same for our two references or does our organization put the content on our letterhead and then we get everyone’s signatures (our financial institution, two references, and our organization)? Basically, does everyone sign one document or am I submitting multiple documents?

Response The content in Attachment A is for the certification that is required to be signed by the Chief Executive Officer, Chief Operating Officer AND Chief Financial Officer that your company is financial able to perform the services required for this project. Your financial institution is only required to confirm if your company is in good standing. Also, the 2 references from previous projects only need to speak to your company’s work or performance o said projects. Your company can just have your Chief Executive Officer, Chief Operating Officer AND Chief Financial Officer sign the attach A and return that to us, along with the 2 reference letters and the letter from the financial institution.

2. Question On the Contractor Disclosure and Declaration Form (Page 2 of 12), can we put our organization name under Offeror Name and Entity Submitting this Form? We were unsure of what “entity submitting this form” meant.

Response Yes.

3. Question On the Contractor Disclosure and Declaration Form, the question that states “Is the Offeror authorized to transact business in the state of Georgia?” We are a nonprofit that is incorporated in the state of Georgia, will our Articles of Incorporation from

Questions and Responses

Response Based on the Required Submittal Form 2 Offeror must submit accompanying documentation regardless of business formation type.

4. Question On the Required Submittal (FORM 3) Financial Responsibility Review section, is the reference letter 1 and reference letter 2 separate documents from the reference list? Do we need two reference letters plus 3 references listed?

Response The 2 reference letters can come from the 3 refences that you list.

5. Question Is there an incumbent?

Response Yes.

6. Question What is the budget for this project?

Response This is a Request for Proposal; therefore, the proponent will provide the budget required to implement the project.

7. Question When is the desired launch date?

Response July 1, 2024.

8. Question What additional data needs is the city looking for?

Questions and Responses

10. Question Can you clarify the proposed target areas?

Response Vendor will provide mobile diversion services across all of the City of Atlanta.

Exact response locations will be provided throughout each day by ATL311 for community referrals and the Atlanta Police Department for pre-arrest diversion.

Vendor will also provide ongoing community outreach and awareness-building across the City of Atlanta; if targeted/prioritized areas are identified at any point during vendor’s contract, City of Atlanta and vendor will select those areas collaboratively.

11. Question I am reaching out to clarify if these services would be behavioral support or physical health and wellness?

Response Generally, services will depend on/respond to both immediate/short-term and long-term needs of the individual(s) with whom vendor is engaging. Thus, services will likely include both behavioral health and/or physical health and wellness supports. Vendor must take a person-centered, holistic approach when engaging with individuals to follow their lead on the range of services they would most benefit from. For example, vendor may engage with an individual who may have immediate physical health needs benefiting from medical care, food, and shelter. In another situation, vendor may engage with an individual who may benefit from long-term case management, which may include job and/or legal navigation resources. Please reference Objectives 1-4 in the Scope of Work for additional detail.

12. Question The RFP seems a little vague on details of what kind of program referrals and services would one provide?

Thus, services will likely include both behavioral health and/or physical health and wellness supports.
