About this file

This is a Product Sample Evaluation Form used by the State for assessing leather samples submitted in response to a footwear procurement request. The evaluation will be conducted at the MDOC (Maryland Department of Corrections) MSI (Marty Shavers Industries) facility using a sequential pass/fail methodology, whereby samples that fail any step are immediately disqualified from further evaluation. The leather samples must meet ten specific technical requirements outlined in RFP Schedule A, Section 1, including specifications for double butt leather suitable for footwear, uniform black coloring, freedom from defects, skin size between 10 to 14 square feet, weight between 3.5 to 4 ounces per square foot, grain surface quality suitable for buffing, pliability for toe pulling without tearing, acceptance of primer and glue application, color fastness after 24-hour water soaking, and finish integrity following the water soak test.

The evaluation process involves sequential testing steps performed by qualified evaluators at the designated facility, with provisions for a verifier to confirm findings. The form includes spaces for documenting observed conditions, actual sample performance, pass/fail determinations, and evaluator/verifier signatures with corresponding dates. Specific testing methodologies are defined, including use of a calibrated scale for weight measurement, buffing machines for grain side evaluation, toe pulling machines for leather pliability assessment, and 24-hour water immersion tests for dye bleed and finish separation evaluation. No pricing information, contract term details, set-asides, or award timelines are specified in this evaluation form document.
