Second Set of Questions, SFJ600-13-R-0001

1. Claims increased significantly during 2012. Please advise if there were any claims which exceeded $25,000.

Information on claims amount will not be released, however, the medical claim amounts for all employees have been provided in answer to Question 1, in the Q&A posted on the Fedbiz Opps website on August 7th 2013. However, for ease of reference we have included the maximum annual reimbursement referenced in

Section C.1.2., “Maximum Annual Reimbursement with the exception of expenses incurred under paragraph 4.e above, the insurance company will reimburse domestic covered expenses up to a limit of 15,000 Fiji dollars per person contract year.

The insurance company will reimburse covered expenses described under paragraph 4.e of the plan up to a separate limit of 10,000 U.S. Dollars per employee per contract year.”

2. If so, please advise if the employees generating claims over $25,000 are still employed by the Embassy.

This type of individual employee medical & employment information will not be released.

3. If so, have they recovered and returned to work?

Please see answer to Question 2.

4. If these are dependent claims, are the dependents recovered or still on claim?

Please see answer to Question 2.
