About this file
This is a Sensitive Security Information Acknowledgement Form issued by the City of Phoenix Aviation Department for use in connection with a sodium bisulfite procurement contract. The form serves as a compliance document requiring authorized representatives of bidding companies to acknowledge receipt of sensitive security information governed by 49 C.F.R. Parts 15 and 1520, which restricts disclosure to individuals with a documented need-to-know. The sodium bisulfite contract is a five-year agreement with an effective date on or about April 1, 2026, contingent upon City Council award and execution by the City Clerk's department. The contract will terminate upon the earliest occurrence of reaching the contract end date, including any exercised extensions, or upon termination pursuant to agreement provisions.
Signatories to the acknowledgement form must certify their understanding that unauthorized release of the restricted documents may result in civil penalties or other legal action, and they agree to destroy the sensitive materials upon the earlier of completing their review or use of the documents or the contract end date. The form requires completion of specific information including the document title(s), project name, contract end date, company name, authorized representative's name and signature, title, phone number, and address. This form is a mandatory compliance requirement for all parties receiving sensitive security information related to the City of Phoenix's sodium bisulfite procurement.
