About this file

This is a seller checklist document from the City of Philadelphia Procurement Department for submission of quotes through the PHLContracts software program platform. The checklist provides step-by-step guidance for vendors submitting bids for FormStack—an enterprise government plan offering 60 users, 1,200 forms, 100,000 submissions per form, 60GB storage, and single sign-on (SSO) functionality. The document outlines the submission process across multiple tabs within the PHLContracts system, requiring sellers to complete sections for general information, items pricing, mandatory questions, terms and conditions acceptance, and attachments. No specific response dates, due dates, site visits, bidder meetings, award dates, or contract terms are detailed in this checklist document.

The checklist emphasizes that sellers must maintain the "Seller" role rather than "Seller Admin" role during submission and must answer all mandatory questions to proceed. While the Local Business Entity (LBE) certification question must be answered, the LBE Program does not apply to the Software Program category. The document notes that software purchases are typically awarded to the City's current software vendors unless otherwise specified, and vendors may be required to submit product literature upon request. No pricing terms, set-asides, incumbent information, or funding amounts are specified in this procedural checklist.
