SDP Plan Form Summary

This is a Supplier Diversity Program (SDP) Plan Form required by the Commonwealth of Massachusetts for the RFR 2026 Laundry Services opportunity at the Veterans Home at Chelsea. The form is mandatory for all bidders responding to this contract opportunity, regardless of whether the bidder holds existing diverse business certification. All parts marked "Required" must be completed for bid eligibility, while sections marked "Optional" may earn bidders additional evaluation points. The form consists of six parts: Bidder Contact Information, SDP Spending Commitment, Proposed SDP Partners, Business Practices, Additional Supplier Diversity Initiatives, and Certification. Bidders must complete Part II (SDP Spending Commitment) and meet the minimum 1% total SDP commitment requirement unless the RFR specifies otherwise. No contract award shall be granted without an SDP commitment that satisfies RFR requirements.

The SDP spending commitment must be allocated between two categories: subcontracting (goods or services directly related to the contract) and ancillary products and services (goods or services supporting general business operations). All proposed SDP partners must be certified by an eligible organization such as the Supplier Diversity Office (SDO), SBA, state or city certification offices, NMSDC, or WBENC in one of nine recognized certification categories: MBE, WBE, SDVOBE, VBE, M/NPO, W/NPO, V/NPO, DOBE, or LGBTBE. Bidders remain responsible for ensuring their proposed partners obtain required SDO certification and for meeting their SDP commitment regardless of partner certification outcomes. The completed SDP Plan Form must be submitted as an Excel file (not converted to PDF) with the bid response, and an authorized individual must complete the certification statement under pains and penalties of perjury.
