GENERAL INFORMATION

CONTRACT AWARDS:

AWARD WILL BE MADE TO MORE THAN ONE OFFEROR. CONTRACTORS MAY BID ON ONE UNIT, ALL UNITS, OR ANY COMBINATION OF UNITS. CONTRACTOR MUST BID ON ALL ITEMS ASSOCIATED WITH EACH UNIT BID ON.

All BID ITEMS MAY OR MAY NOT BE AWARDED EACH YEAR.

Surface Blading on the Mark Twain National Forest.

Provide all motor patrols, operators, laborers, fuel, transportation and other supplies and equipment necessary to perform road grading services on the Salem, Houston-Rolla, Potosi-Fredericktown, Poplar Bluff, Eleven Point, and Ava-Cassville-Willow Springs Ranger Districts of the Mark Twain National Forest.

Culvert Installation / Replacement on the Mark Twain National Forest.

Provide all new pipes, aggregate base, equipment, operators, laborers, fuel, transportation and other supplies and equipment necessary to perform culvert installation and/or replacement of culverts at various locations on the Salem, Houston-Rolla, Potosi-Fredericktown, Poplar Bluff, Eleven Point, and Ava-Cassville-Willow Springs Ranger Districts of the Mark Twain National Forest.

Diameter and types of culverts are listed in the schedule of Items.

Crushed Base Aggregate and Crushed Clean Aggregate Placement on the Mark Twain National Forest.

Provide all operators, laborers, fuel, transportation and other supplies and equipment necessary to perform placement services of aggregate on the Salem, Houston-Rolla, Potosi-Fredericktown, Poplar Bluff, Eleven Point, and Ava-Cassville-Willow Springs Ranger Districts of the Mark Twain National Forest. The COR shall determine whether base aggregate or clean aggregate will be used on the roads and the locations of aggregate placement. Weight tickets shall be provided to the COR for each load of aggregate delivered to the roads.

Gradation and types of aggregate listed in the schedule of items.

This solicitation is for an indefinite-quantity contract. It is effective for the period stated in the Period of Performance paragraph below. The quantities of services specified in the Schedule are estimates only and are not purchased by this contract. See Far. 52.216-22 Indefinite Quantity. Actual work location of services will be issued on delivery orders.

For this solicitation, the Government used a unit of one as the estimated quantity for evaluation purposes when no work of this type is currently planned during the award period. Other estimated quantities shown were determined from current work plans. (See Clause 52.216-21 REQUIREMENTS) PRE--PROPOSAL MEETING: Contractors are strongly urged to attend this meeting. Attendance will be noted. The purpose of this meeting is: (1) to review the requirements of the Performance-Based Service Contract type of contracting, (2) to discuss the format for submitting proposals, and (3) to discuss the evaluation process for this contract.

PRE-OFFER TOURS: Offerors are urged to inspect the contract areas due to the variability of the roads involved. Pre-quote tours will not be given unless at least one interested quoter contacts Forest Service personal to arrange for the meeting(s).

BASIS OF AWARD: Award(s) will be made to the most highly ranked offeror(s) (1) whose total proposal is technically acceptable, has demonstrated past performance in meeting customer satisfaction, and (2) whose past performance and technical/cost relationship presents the BEST VALUE and is the most advantageous to the Government. Offerors that do not submit a technical proposal may not be considered for award.

AGAR 452.211-74 PERIOD OF PERFORMANCE (FEB 88):

This contract is a fixed price contract for a basic period of performance from date of award through September 30, 2016, plus four one-year option periods.

Submission of an offer for the basic contract period constitutes an offer to perform in the Option Period(s), if exercised.

QUALITY ASSURANCE SURVEILLANCE PLAN

Surface Blading on the Ava/Cassville/Willow Springs, Eleven Point, Houston/ Rolla, Poplar Bluff, Potosi/Fredericktown, and Salem Ranger Districts of the Mark Twain National Forest.

Culvert Installation / Replacement on the Salem, Houston-Rolla, Potosi-Fredericktown, Poplar Bluff, Eleven Point, and Ava-Cassville-Willow Springs Ranger Districts of the Mark Twain National Forest.

Aggregate Placement on the Salem, Houston-Rolla, Potosi-Fredericktown, Poplar Bluff, Eleven Point, and Ava-Cassville-Willow Springs Ranger Districts of the Mark Twain National Forest.

This Quality Assurance Surveillance Plan (QASP) has been developed pursuant to the requirements of the Performance Work Statement (PWS) in Contract___________. This plan sets forth procedures and guidelines that will be used in evaluating the technical performance of the contractor.

A. PURPOSE OF THE QASP

The QASP is intended to accomplish the following:

Define the roles and responsibilities of participating government officials and the contractor;

Identify the types of work to be performed and the inspections to be performed to validate adequacy;

Describe the evaluation methods that will be employed by the government in assessing the contractor’s inspection program and performance to the quality/performance standards required for each task;

Provide copies of the contractor’s inspection records and Government quality assurance monitoring forms that will be used by the government in documenting and evaluating the contractor’s performance;

Describe the process of performance documentation.

The contractor is responsible for performing inspection services pursuant to the inspection clause and for providing and maintaining the inspection forms, approved by the government, to perform the contractor’s inspection services. The contractor has represented themselves, to be responsible for meeting the prescribed quality/performance standards in the statement of work.

B. ROLES AND RESPONSIBILITIES OF GOVERNMENT OFFICIALS

The following government officials will participate in assessing the quality of the contractor’s performance. Their roles and responsibilities are described as follows:

The Contracting Officer’s Representatives’ (COR’s) responsibilities will be listed on their designations and may include the following: monitoring, assessing, recording, and reporting on the technical performance of the contractor on the schedule of inspections submitted by the contractor and approved by the government, witnessing the contractor’s performance of the inspections performed by the contractor or will evaluating the contractor’s inspection results as to documented findings relative to meeting the standards for each task/subtask within the statement of work, and primary responsibility for completing quality assurance monitoring forms which will be used to document the inspection and evaluation of the contractor’s work performance.

The Contracting Officer (CO) will have overall responsibility for overseeing the contractor’s performance and for the day-to-day monitoring of the contractor’s performance in the areas of contract compliance, contract administration, cost control, and property control; reviewing the COR’s assessment of the contractor’s performance; and resolving all differences between the COR’s and the contractor’s version.

C. TYPES OF WORK TO BE PERFORMED/SCHEDULE OF INSPECTIONS

This contract calls for the contractor to perform the following tasks and subtasks and to perform inspections at the completion of the tasks/subtasks as a minimum and or for intermediate inspections as determined at contract award by the COR/CO. The contractor’s quality control plan/schedule will be modified accordingly and will be one of the bases for the award of the contract.

D. METHODS OF SURVEILLANCE

The government, through the COR’s, will monitor the contractor’s performance on a continuous basis. The method the government will use is identified in C above. The COR/Inspector will first verify the results of the contractor’s inspection, completeness and adequacy of the documentation of the contractor’s inspection results. The government COR will perform periodic independent inspections after the contractor has performed their inspection if questions develop regarding the results of the contractor’s inspections. Results of the independent inspections that differ from the results noted by the contractor on their inspections the costs thereof for the re-inspection costs will be assessed against the contractor. The contractor shall be required to rework/repair or replace the effort until the quality/ performance standard is achieved.

Inspections will be performed to the schedule submitted by the contractor and approved by the government. Variances from the schedule must be pre-approved by the CO.

E. QUALITY ASSURANCE (QA) FORMS AND REPORT

1.

The COR will use a quality assurance monitoring form t o document and evaluate the contractor’s performance under the contract. This can consist of a Form, a Memo for record or an entry in a daily or weekly diary. The form when completed will document the COR’s understanding of what the contractor was supposed to do, what was actually done, and the impact or consequences of what was done. The documentation will also identify the additional steps required of the contractor if there is a nonconformance.

2. The COR will judge each event in accordance with the performance standards and performance requirements stated in the Quality/Performance Requirements stated in the contract either in section 5 of the Statement of Work (SOW) or in section 7 of the SOW.

3. The COR must substantiate all tasks which he judges to be indicative of “unacceptable” performance. Performance at the “acceptable” level is expected and need not be substantiated.

4. In accordance with Section C above, the contractor shall furnish (weekly, at a minimum) to the COR copies of all inspection forms generated by the contractor witnessing inspections performed. The COR will forward copies of all completed QA monitoring forms to the CO and contractor inspection records prior to processing payments. The contractor is required to respond in writing to any negative QA monitoring form(s) within 5 working days after receipt of the form(s).

F. ANALYSIS OF SURVEILLANCE RESULTS

1. The CO will review each QA monitoring form prepared by the COR. When appropriate, the CO may investigate the event further to determine if all the facts and circumstances surrounding the event were considered in the COR’s opinion outlined on the forms. The CO will immediately discuss every event receiving a substandard rating on the forms. The CO will immediately discuss every event receiving a substandard rating with the contractor to assure that corrective action is promptly initiated.

2. Upon completion of the contract/task order the COR will perform a performance evaluation of the contractor using the results documented as above and provide to the CO for further action.

SECTION I - GENERAL

SCOPE OF WORK: The contractor shall provide any and all labor, equipment, supplies, services, permits, and licenses necessary to perform road maintenance which include surface blading, culvert installation and aggregate placement on the Ava/Cassville/Willow Springs, Eleven Point, Houston/Rolla, Poplar Bluff, Potosi/Fredericktown, and Salem Ranger Districts of the Mark Twain National Forest.

The maintenance work to be performed shall include surface blading of the road traveled way and shoulders to reestablish a crown on the road surface as well as ditch cleaning and reconstructed were needed.

The maintenance work shall also include the installation or replacement of culverts at locations identified by the COR(s).

Delivery and placement of aggregate shall be tailgate spread as specified at locations identified by the COR(s). COR(s) shall specify type of aggregate prior to delivery.

The normal maintenance period will be April 8 to November 15. The Contractor shall follow all State and Federal regulations, including OSHA guidelines.

This statement of work consists of completing the tasks/subtasks in Section 5 to the performance requirements of the technical specifications. Mileages listed have been determined by electronic distance meter.

LOCATION:

The various roads are located on the Salem, Rolla-Houston, Potosi-Fredericktown, Poplar Bluff, Eleven Point, Ava, Cassville and Willow Springs Districts of the Mark Twain National Forest as delineated on the attached maps and listed in Section 330 of these Specifications. Specific locations, mileage data, and maps may also be issued with each task order.

130 - Operators: Experience in the operation of motor graders, backhoe loader and drivers used in this work are considered essential. All operators shall have experience in this type of work or demonstrate their ability to operate a motor grader, backhoe loader and dump trucks. Operators not meeting these requirements will be subject to disqualification.

140 - Equipment: All equipment used in the work shall be of the proper type, of a sufficient size, in good operating condition, and otherwise meet the equipment specifications for each type of maintenance work. The Contracting Officer may order removal and replacement of any unsatisfactory equipment at the Contractor's expense. All equipment must meet all Federal and State licensing and safety requirements. Exhaust stacks of all engines, except those with exhaust-driven turbocharges, shall be equipped with spark arresters that will prevent the expulsion of sparks. The moving of equipment shall be considered incidental to the contract.

Backhoe loader will be required for removal of old culverts and installation of new culverts.

150 – Safety

151: The Contractor shall exercise due caution and care when working to prevent undue conflict with public users of roads. Roads shall remain passable to traffic during the contractor's operations. The roads should remain open to traffic during blading operations. Contractor shall be allowed to close the road during culvert installation or replacement. All culvert work shall be completed by the end of the day or else the trench is covered prior to their leaving the project site.

152: Flashing signals and a reflectorized slow-moving vehicle emblem shall be installed on all slow-moving equipment (i.e., road graders, tractors, trucks, etc.) working on the roadway so that they are visible for oncoming traffic.

153: The Contractor shall post warning signs on both ends of the section of the roadway being worked to warn road users of the work in progress. Signs shall be of the size, number, and colors as required in Section 6B-25 of the current edition of the "Manual on uniform Traffic Control Devices. Signs will be moved as needed so that a sign does not precede or lag machinery by more than one mile. Signs shall be posted only when machinery is actually working.

154: All work shall be scheduled so that at the end of a day's work, the road is passable for the type of traffic normally using the road. Road Maintenance segments will generally not be permitted to exceed one (1) mile intervals on through roads and two (2) mile intervals on dead-end roads.

155: No berms or windrows of material shall be left on a road overnight. If it is necessary to leave a berm on the roadway overnight it shall be properly signed as a hazard and visibly illuminated at night (See Manual of Uniform Traffic Control Devices, Section 6E-5, Lighting Devices).

Fill of suitable road material over the top of installed culverts shall be no less than 1 foot after compaction and will be uniform with road surface on either side of culvert. No dip in the road across the culvert will be allowed. No open trench will be left unattended at any time.

160 - Damage to Property: The Contractor shall be responsible for any damage caused by his/her personnel or equipment to any existing structures such as culverts, signs, cattle guards, and riprap protecting culvert inlets and/or outlets. The Contractor is responsible for verifying existence and location of utilities and is liable for any damage by construction activity. The Contractor is responsible for coordinating with the Missouri One Call System at least three (3) working days prior to excavation work. Damaged items shall be repaired or replaced by the Contractor at no additional cost to the Government.

170 - Pollution and Erosion Control: During all operations under this contract, the Contractor shall take reasonable measures to prevent or minimize soil erosion and pollution of water and other resources. All refuse, including garbage, rubbish, and solid or liquid wastes, from the Contractor's operations, including camps, parking areas, and equipment maintenance areas, shall be stored, collected, and disposed of in a sanitary and nuisance-free manner. Refuse shall be disposed of in State-approved landfills or as approved by the Contracting Officer's Representative (COR).

BOUNDARIES: The road right-of-way will be considered as the boundaries.

DISPOSAL OF CULVERTS: Old replaced pipes will be removed from the site and disposed of by the contractor.

RE-MEASUREMENT: The Contractor may at any time after issuance of a task order request a re-measurement of road miles. The request must be in writing. Payment will be based on the re-measurement of the mileage, except no adjustment shall be made for miles less than 0.1 of a mile. The Contractor shall pay for the cost of re-measurement if the re-measurement results in an increase of less than 10 percent. Method of re-measurement to be used will be determined by the Government.

PAYMENT: Payment for performed work will be based on actual miles and work completed by the Contractor and accepted by the Government. Payment will be processed for completed miles and completed culvert installation/replacement and for a minimum amount of $2,500.00.

CONTRACTOR REPRESENTATIVE: The contractor shall provide a contract representative for the performance of the work. The name of this person and an alternate(s) who shall act for the contractor when the contractor is absent shall be designated in writing to the contracting officer. The contractor representative or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The contractor representative and alternate(s) must be able to read, write, speak, understand English, and communicate with his crews.

PROTECTION OF IMPROVEMENTS: Any improvements damaged by the contractor’s work shall be repaired to the condition that existed prior to the start of work at the contractor’s expense.

TECHNICAL SPECIFICATIONS

SURFACE BLADING:

EQUIPMENT SPECIFICATIONS: The equipment to be used for surface blading and/or ditch pulling shall be a motor grader, self-propelled, tandem drive, with a moldboard not less than twelve (12) feet in width.

This work shall consist of surface blading of the road traveled way and shoulders with a motor grader to re-establish a crown on the road. Incidental cleanup and drainage work will be required by cleaning and reshaping ditch lines were needed.

Surface blading shall include a minimum of five (5) passes on all roads.

The roadbed, including turnouts, shall be shaped to reasonably conform to the typical sections existing on the ground. When necessary, scarification will be used to eliminate ruts and potholes or to otherwise loosen the surface. On rutted or corrugated roads, additional passes may be required to remove ruts and corrugations. The contractor shall maintain the traveled way width as existing.

Contractor will be responsible for removing obstructions in the roadway and ditch lines that the motor grader does not remove. This will consist of removing loose rocks over three (3) inches in diameter from the roadway after blading operations and cleaning culvert inlets, outlets and catch basins to previously constructed lines so that the flow of water to and from the culvert is not impeded. All fallen trees, limbs, or other obstructions within the roadway clearing width (from tree line to tree line) and roadway ditch line, at the inlet and outlet of culverts or in the lateral ditches shall be removed and disposed of outside of the tree line. This is considered incidental to surface blading.

Work shall be done by pulling the road surface material from both shoulders of the road to the center. The shoulders of the road will be bladed from the top of the Inslope on each side of the road to allow road to drain into the ditches. The material shall then be graded across the entire surface of the road, leaving a crown in the road. Roads graded by this method shall be free of loose stones over three (3) inches in diameter and have no encroachment from the shoulders. No accumulation of over-size rock, sod or debris will be permitted on the roadway or shoulders. All usable material shall be incorporated on road. Only with COR approval will it be permissible to grade unusable material from roadway. In no case will unusable material be disposed of on fenced or open private land.

Out sloped road segments shall be graded from the high side to the low side and back to the high side leaving no crown in the road. No accumulation of unusable material will be permitted on the roadway or shoulders.

All roads that have ditch lines shall be pulled and outlet ditches cleaned out, so water drainage works properly where the road has failed. Roads that have suffered damage due to outlet ditch failure shall have the ditch line leading into the outlet pulled and the outlet ditch reopened to allow water to drain. Blading shall be completed to allow surface water to drain directly into the roadside ditches without interruption by berms or windrows of excess surfacing material. This is considered incidental to surface blading.

Drainage dips and "V" dips shall be maintained with sufficient depth and width to adequately drain the roadway, and in no case will they be removed or filled in. The lower ends shall be shaped to adequately drain away from the roadway. Swales and humps of drainage dips and "V" dips shall be maintained so that traffic flows smoothly with no abrupt up or down motion. This is considered incidental to surface blading.

TECHNICAL SPECIFICATIONS

CULVERT INSTALATION/REPLACEMENT:

EQUIPMENT SPECIFICATIONS: The equipment to be used for culvert installation and or replacement shall be a backhoe loader.

Excavate trenches in natural ground to permit satisfactory joining of the culvert sections and thorough tamping of the bedding material under and around the culvert. Excavate trenches to a minimum width equal to the culvert diameter plus 2 feet. The bedding for the pipes shall be 1” crushed base aggregate and shall be no less than 0.5 feet in depth and equal to the width and length of the excavated trench.

Native material, free from rocks greater than 3” in diameter, will be placed around the pipe and compacted. The top of the culvert will have no less than 1 foot of compacted fill placed over the culvert and will be uniformed with the road surface. No dips in the fill material will be allowed.

Pipes will be skewed 30%, where obtainable to allow for proper drainage. Deviation of skew must be approved by the COR prior to placing the culvert. Ensure that the final installed alignment of all pipe allow no reverse grades and does not permit horizontal and vertical alignment to vary from a straight line from center of inlet to center of outlet by more than 2 percent (%) of pipe center length or 1.0 feet, whichever is less.

Retrenching of outlet ditch to allow drainage away from the outlet of the pipe may be necessary.

This is considered incidental to culvert installation or replacement.

Corrugated metal pipe shall be new in condition, 0.064” in thickness and approved by the COR prior to placing them in the roads. Culverts will be American made. Culverts that do not comply will be removed and replaced at the expense of the contractor.

High-density polyethylene (HDPE) Culverts shall be new in condition and installed according to pipe manufacturer’s specifications. Culverts will be American made. Culverts that do not comply will be removed and replaced at the expense of the contractor.

The normal maintenance period will be April 8 to November 15. The Contractor shall follow all State and Federal regulations, including OSHA guidelines.

This statement of work consists of completing the tasks/subtasks in Section 5 to the performance requirements of the technical specifications. Mileages listed have been determined by electronic distance meter.

AGGREGATE PLACEMENT:

EQUIPMENT SPECIFICATIONS: The equipment to be used for aggregate placement shall be dump trucks. Pup trailers are permissible.

This work shall consist of delivery and tailgate or placing of specified aggregate to the locations determined by the COR. The depth, width and length of the aggregate spread shall be determined by the COR prior to aggregate being delivered to the roads. Aggregate shall be spread in uniform layers. Excess material shall be spread to conform to the beginning and ending of previous loads that have been placed on the road. Crushed aggregate shall comply with Missouri Standard Specifications for Highway Construction, Section 310.2 and specifically to Section 1006, Grade B, INSPECTION AND ACCEPTANCE

INSPECTION:

Equipment Inspection:

The Government will inspect the contractor's equipment prior to the beginning of work to determine if the equipment complies with the terms, specifications, and provisions of the contract.

Equipment that does not comply must be replaced or deficiencies corrected prior to the beginning of work. This will be at the expense of the contractor.

Inspection of work: Inspection of three (3) or more completed roads will be performed by the COR or his inspector within three (3) workdays following notification of completion of the work. A road is identified in the Project List by number and mileage to be maintained. Timely inspection and acceptance of completed roads will require close coordination between the Contractor and the COR.

A procedure and schedule for final inspection will be agreed upon by the Contractor and the Government at the pre-work conference.

ACCEPTION:

Acceptance will be determined by inspection of work performed and will be based on adherence to the specifications within the contract.
