About this file

This Schedule I document outlines Service Level Agreements (SLAs) for food suppliers under Michigan Department of Corrections (MDOC) Request for Proposal No. Dept 171, ID 190000000142 for Food and Food Service Products. The document specifically details two key metrics for supplier performance: Metric 1 focuses on timely and documented delivery, while Metric 2 addresses accurate deliveries.

The agreement establishes strict compliance requirements with financial penalties for non-compliance. For both metrics, suppliers face penalties of $500 for each of the first five occurrences of non-compliance per calendar month, escalating to $1,000 for each subsequent occurrence. Additionally, there is a specific penalty of $5,000 for any delivery containing pork products, which are strictly prohibited. All deliveries must match order confirmations exactly, include proper documentation through packing slips, and adhere to approved delivery schedules. Split deliveries must be completed within three days of the original order. The State maintains discretion to apply penalties either as credits against payables or as direct payments due within 10 days of demand.
