About this file

This is a Schedule D Service Level Agreement document for a Michigan state contract opportunity (RFP No. 260000002293) related to Investment Consulting Services. The agreement establishes two primary service level agreements that the contractor must meet throughout the contract term. SLA #1 requires the contractor to attend all meetings as specified in the Statement of Work, including monthly meetings and up to four SMIB meetings per year, with 100% compliance as the acceptable standard. SLA #2 requires the contractor to provide all reports—including monthly, quarterly, and annual deliverables—on the established timelines, also with 100% compliance as the acceptable standard. Both agreements require contractors to communicate any extenuating circumstances to the Program Manager prior to scheduled meetings or report due dates for consideration before service credits are assessed.

The agreement specifies that failure to meet either service level agreement will result in a 1% service credit applied against the total value of the monthly invoice for the month in which the failure occurred. The State reserves the right to apply these credits toward any payable due to the contractor or to require direct payment to the State, which must be completed within ten days of notice of assessment. The State also reserves the right to reconsider or amend SLA amounts in the event of split awards. No specific pricing, contract term, or renewal information is detailed in this service level agreement document.
