About this file

This is a pricing schedule request for Request for Proposal No. 260000002052 issued by the State of Michigan for Captive Actuarial Examinations of Insurance Companies. Contractors must submit pricing proposals using Microsoft Word format, with the option to provide supplementary pricing schedules in non-modifiable formats such as PDF. The proposal must include all costs associated with contract activities, encompassing one-time charges, setup fees, and potential expenses such as shipping, handling, and per-piece pricing. Bidders should identify all staff and roles who will be included in service delivery and provide corresponding hourly rates for each position.

Contractors are encouraged to offer expedited payment terms by specifying a percentage discount off invoices if payment is received within a stated number of days following invoice receipt, with the timeframe excluding processing time for funds to reach the contractor's financial institution. By submitting a proposal, contractors certify that all pricing was developed independently and without consultation, communication, or agreement with competing contractors. The pricing schedule must be completed as requested to avoid disqualification of the proposal.
