About this file
This is a Statement of Work (Schedule A) issued by the State of Michigan for Request for Proposal No. 180000002885, addressing non-destructive testing services for the Michigan Public Safety Communications System (MPSCS). The project requires a contractor to perform Cylindrically Guided Wave Technique testing and evaluation on anchor rods at 120 guy towers located throughout Michigan. The scope encompasses testing and reporting on the condition of 100 percent of anchor rods at each site, with particular focus on identifying external or internal corrosion, axial cracking, and circumferential cracking. The contract explicitly excludes remediation work for identified issues. The State reserves the right to reduce the number of sites included in the scope. The MPSCS prefers completion of all work during the 2018 calendar year, pending weather restrictions. Contractor representatives must be available during State normal working hours of Monday through Friday, 7:00 a.m. to 6:00 p.m. EST, with possible evening and weekend hours negotiable based on project timeline requirements. A project kick-off meeting is required within nine calendar days of contract award, and a comprehensive project plan must be submitted within fifteen calendar days of the effective date.
Pricing terms are not explicitly detailed in this document; however, invoices must include itemized descriptions of contract activities with unit pricing. The contractor will be compensated via electronic funds transfer (EFT) only, and overtime, holiday pay, and travel expenses are excluded from reimbursement. The contractor must demonstrate GUL Wavemaker G3 Level 1, 2, or 3 certification for all guided wave inspectors and submit documentation of testing methodology, parameters, and techniques prior to commencing work. Liquidated damages of $5,000 plus $500 per day apply for late or improper completion of contract activities. Closeout packages must include photographs and documentation of testing work, with the contractor required to provide an example closeout package with its bid submission. Key personnel must be approved by the State and cannot be removed without prior written consent; unauthorized removal of key personnel may result in credits up to $50,000 per individual.
