About this file

This document is Schedule A to a contract for radiologic equipment service and repairs. The contract is between the Awarding Authority and a Contractor. The Contractor must provide repair services for a list of covered radiologic equipment, including items such as computed radiography systems, medical radiographic systems, radiographic tables and tube-stands. The Contractor must seek written approval from the Awarding Authority prior to procuring any necessary services or replacement parts, and will be reimbursed for the Contractor's actual costs. The Contractor must use original equipment manufacturer parts where possible, and may substitute "equal" parts with the Awarding Authority's prior written consent. The Contractor must minimize any interruption or downtime to the Awarding Authority's facilities. The contract also includes reporting requirements, where the Contractor must submit copies of service approvals and proof of payment for any repair parts with their monthly invoices.

The contract does not specify pricing terms, set asides, or funding details. The list of covered radiologic equipment indicates this is likely a state or local government contract opportunity, but the specific agencies, consortiums or jurisdictions involved are not named.
