To: Interested HubZone Certified Contractors Attn: Government Contracts Points of Contact Subject: Request for Quote SB1342-15-RQ-SEVSUP

The National Technical Information Service (NTIS) has a requirement for contractor support as part of its continuing deployment of its Enterprise IT Service Management (ITSM) system, which utilizes ServiceNow. The contractor shall provide implementation services to support NTIS efforts to setup, test, and move the ITSM solution into full production use, in accordance with the attached Statement of Work (SoW).

This is a Combined Synopsis Solicitation prepared in accordance with the format in Subpart 12.6, as supplemented by Commercial Items, Simplified Acquisition Procedures. This action is set aside for HubZone certified Contractors in accordance with 52.219-3. This is a Brand Name Justified action under 13.106-1(b). A copy of the Justification of Other than Full and Open Competition is as attached. This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-79, effective December 15, 2014. Terms and Conditions are as attached.

The Government Estimates, but does not guarantee, the following level of effort:

Contract Type

The resultant Task Order will be Firm Fixed Price.

Basis for Award

This order will be evaluated on a low price, technically acceptable basis (LPTA). A quote will be found technically acceptable when it clearly and convincingly demonstrates that the contractor understands the requirements and has quoted the necessary technical capabilities to meet it, and

The Government reserves the right to review the contractors past performance record contained in the Past Performance Information Retrieval System (PPIRS) and/or other databases.

Quotation Response Instructions

Quotations submitted shall be either font size 11 or 12. Margins shall not be smaller than .75 inches on all sides. Cover Sheets should be provided, as should a separate page for each volume including any Exceptions and rationale for said exceptions. Quotes shall clearly set forth any exceptions or assumptions taken with regard to any part of this RFQ. Contractors are cautioned that any exceptions or assumptions to the terms and conditions of the solicitation will be at their own risk.

Coversheets, resumes, and exceptions do not count towards page limit totals.

Volume I

Technical Approach:

Volume I shall contain a complete description of the contractor’s technical approach that will be utilized to accomplish all tasks in the attached SOW. The contractor’s response to Section 5.0 of the SOW must be sequentially ordered and identify the SOW paragraphs by number in order to be considered a conforming quote. Failure of the contractor to comply with sequentially addressing all the SOW requirements or simply restating the SOW requirements may result in the quote being determined non-responsive. Any significant deviations from the estimated level of effort shall be explained in this section.

Staffing:

Volume I shall contain the contractor’s planned staffing approach that will be utilized to support the Government’s requirements. It shall specifically address all aspects of organization and governance necessary to accomplish the work contained in the SoW, and shall provide a complete description of the company’s approach to hiring and retaining highly qualified employees and maintaining adequate staffing levels.

Contractors shall identify and define and/or concur with the specific labor categories proposed to fulfill the requirements outlined in the SoW to include resumes of any ‘Key’ Personnel as well as proof of any requisite certifications or training. Resumes shall be provided for proposed Key Personnel.

Past Performance:

Volume I shall also contain two(2) past performance references for the contractor’s demonstrated Past Performance (Federal, States, Local Government and/or private) within the past three (3) years. Past Performance shall be reviewed in order to ensure minimal risk(s) and establish high confidence in the contractor’s potential capabilities. Past Performance references should reflect projects similar to the requirement in size, scope, and complexity, and should also demonstrate the quality of the services provided. Furthermore, for the referenced contracts, the contractor shall provide the following information:

a. Name and address of the company or agency

b. Points of Contact

c. Telephone numbers and email addresses

d. Contract type and number

e. Identify if you were Prime or Subcontractor

f. Contract Value (Original and Current Value, with an Explanation for Cost Growth)

g. Period of Performance (including start date and original end date, with an explanation of any delays)

h. Type of work performed

i. Problems encountered and their resolutions

j. Any subcontractors or partnerships

In addition, a Past Performance Questionnaire (PPQ) is provided as Enclosure #2 of the CSS.

The contractor shall forward this questionnaire to each of the references provided and request the response be submitted by the solicitation due date to the address specified in the PPQ.

If a contractor does not have a record of Past Performance for this type of work, they may substitute Past Performance history of individuals to be assigned, provided said individuals are ‘key’. A lack of any past performance will be considered neither a positive nor negative with regards to the Contractor’s response.

Volume II

Volume II shall document all prices for the subject requirement. As part of submitting a complete pricing volume, the contractor shall complete and return the attached Excel Pricing Spreadsheet, unlocked with formulas visible if used. Rounding shall be to the nearest cent or to the nearest whole hour, as applicable. Failure to complete the Pricing Sheet may result in a determination of non-responsiveness. All proposed labor categories shall be fully burdened. All pricing shall be quoted as Government on-site.

Quote Submission Instructions

Quotes shall be submitted as quickly as possible, but no later than as listed in the FBO posting.

Quotes shall be submitted by electronic mail to Mr. Robert Coyle at Robert.Coyle@nist.gov.

Questions may be directed to the same.

This RFQ shall not be considered direction to proceed with the subject effort, nor a promise of future direction to proceed. Contractors are solely responsible for submission costs.
