About this file

This is a Subcontractor Annual Representations & Certifications (SARC) form used by Fermi Research Alliance, LLC (FRA) to collect required information from potential subcontractors and suppliers. The form requires companies to provide detailed business information including ownership structure, size classification, accounting system details, quality certifications, and various federal compliance certifications.

The form contains five main sections: Business Information, Offeror Responsibility, Export/Import Control Certifications, Federal Acquisition Regulation (FAR) Certifications, and a final Certification section. Key requirements include disclosing parent company information, DUNS number, SAM.gov registration status, business size/type classifications, accounting system approvals, executive compensation data, Equal Opportunity compliance, and affirmative action program status. The certifications are effective for 12 months from signature date, with companies required to notify FRA within 15 days of any status changes. The form supersedes the previous FL-6 form and includes recent updates related to E-Verify certification and quality standards documentation requirements.
