SECURITY CLEARANCE
A security pre-clearance must be performed for employees referred to IHS through this contract.
Fingerprints must be completed and adjudicated before services are performed under this contract.
The performance of this contract will require routine access by the contractor's employees or subcontractors to facilities or systems controlled by the Indian Health Service (IHS). Before starting work requiring routine access to IHS facilities or systems, each person must complete an FBI National Criminal History Check (Fingerprint Check) adjudicated by an IHS employee using the Office of Personnel Management Personnel Investigations Processing System. Contractors shall allow five business days for IHS processing of fingerprints taken electronically at an IHS site and thirty business days for non-electronic processing of fingerprints using FBI Form FD-258. A list of IHS sites with electronic fingerprint capability is available from the Contracting Officer.
In addition, no Contractor or subcontractor employee shall be permitted to perform work under this contract if listed on the HHS Office of Inspector General List of Excluded Individuals/Entities (LEIE), http://exclusions.oig.hhs.gov. As soon as practicable before the start of work, the contractor shall provide the contractor's performance of the work to the Contracting Officer with the names of all individuals to be used in the performance of work for screening against the LEIE. During the performance, the contractor shall provide the Contracting Officer with the names of any additional or substitute employees for screening before they begin work.

Each security preclearance shall be conducted sufficiently before the start of performance to avoid delays caused by denial of access. If this is a Time and Materials, Labor-Hour or Cost Reimbursement contract, the contractor shall not charge for or be reimbursed for labor hours or other costs incurred for employees who are unable to perform due to denial or access or the excess time required to resolve and clear unfavorable security clearance findings. If this is a Fixed Price contract, denial of access due to security clearance findings shall not be a basis for excusable delay or an increase to the contract amount.
The government will pay for the cost of processing the contractor's suitability clearances. However, multiple investigations for the same position may, at the Contracting officer's discretion, lead to a reduction(s) in the contract price of no more than the cost of the extra investigation(s).

STATEMENT OF WORK
For
SERVICE CONTRACT FOR THE HOSPITAL BOILERS
April 2024
Introduction
The Pine Ridge Comprehensive Healthcare Facility currently has three heating fuel-powered boilers that provide heating water used for heating the facility. The Aldrich manufactured boilers were installed in the summer of 2023. The boilers require four visits for inspections/servicing per year plus an agreement for emergency service calls as needed. All visits shall be performed by a qualified technician that can access proprietary software and provide the quality of servicing required to keep the units operating properly.
Purpose
The purpose of this contract is to provide for the preventive maintenance inspections for the boilers in the Pine Ridge Hospital. The period of performance for this contract is a Base + Four (4) Option Years.
Scope of Work
The contractor shall furnish all labor, equipment, materials, pay applicable taxes and have appropriate licenses to accomplish the tasks outlined in attached procedures and the following:
1. Documentation of qualifications for each staff member that will be working on site
2. Furnish all labor, parts, oil, and materials needed to perform preventative maintenance to the listed equipment during normal work hours.
3. Quarterly measure and adjust fuel to air ratio to prevent carbon buildup and maintain efficiency.
4. Quarterly inspect all pumps, motors, bearings, couplings, and all other related parts to ensure boilers are running at maximum efficiency.
5. Annually clean boiler heating surfaces thoroughly. Access to boiler firetubes may be gained by removal of the front and rear smokebox covers. Remove the turbulators located in the top pass of firetubes. Clean firetubes with flue brush and reinstall turbulators. Carefully check condition of turbulators. If turbulators are corroded or deteriorated replace them to ensure optimum boiler efficiency.
6. Annually check water level controls: Probe Type Low Water Cut Off: The probe is exposed to possible contamination in the boiler water. It is important to physically remove the probe from the boiler tapping annually and inspect it for accumulation of scale or sediment. Float Type Low Water Cut Off: the blow off valve should be opened at least once a quarter month (use greater frequency where conditions warrant), to flush out the sediment chamber so the device will function properly. Low water cut offs and water feeders should be dismantled annually by qualified personnel and inspected, to ensure proper function.
7. Provide a written report after each visit to the Facility Manager regarding the condition of the equipment and any recommendations for enhancement to maintain capacity, reliability and efficiency.
8. Provide travel and labor for trouble shooting emergency service calls. Repair parts must be quoted separately based on emergency service call trouble shooting.
9. Repair parts must be quoted and approved before repair work can begin.
Government Furnished Material
The Government shall provide the following:
• An employee to receive instructions and act on the recommendations provided by the Technician.
• Permit the Technician to start and stop the equipment in order to perform required servicing and testing.
• Proper operation of the equipment by Engineering Staff in accordance with operating procedures.
• Reasonable means of access to the equipment, including any required removal, replacement and refinishing of the building structure.
