About this file
This document is a Request for Quotation (RFQ) issued by the Ohio Department of Natural Resources (ODNR) for epoxy flooring resurfacing at Salt Fork State Park. The project involves installing decorative epoxy flake coating on floors in two shower houses (36 total shower stalls, approximately 144 sq ft per shower house) and one restroom building (106 sq ft per side), with work scheduled to commence no earlier than December 8th, 2025, and be completed by February 28th, 2026. Contractors are encouraged to conduct a site visit prior to bidding, and the contract will be awarded to the lowest responsive and responsible bidder based on a low lot total calculation. The project requires complete floor preparation, including tile removal, surface grinding, moisture barrier installation, drain adjustment, and cove base sealing.
The contract will be administered through the Ohio Shared Services (OSS) with specific invoicing requirements, including submission of a detailed invoice with a unique invoice number, complete itemization of services, and adherence to state procurement guidelines. The contract is valid through June 30th, 2026, with a fixed destination/pre-paid/allowed delivery condition. No state sales tax will be applied, and the total project will be funded through the state's procurement budget. The park manager, Adam Sikora, is the point of contact, and contractors must provide their own heating equipment during the installation process, as the restroom building lacks internal heating.
