STANDARD PROTECTIVE FOOTWEAR
1. Scope. This work statement is for protective footwear sold from contractor-owned, contractor-operated trailers at Puget Sound Naval Shipyard (PSNS) and Intermediate Maintenance Facility (IMF) Bremerton, and Naval Station Everett WA locations. The Bremerton location will be operated on a full time basis where the Everett location will be operated on a part time basis. Everett personnel will be served two (2) times per month using mobile trailers or trucks suitable for the dispensing of safety footwear. The Contractor will:
a. Issue contractor-owned Standard Protective Footwear, per section ten (10).
b. Provide employees with the option to purchase protective footwear of their choice, which are not the styles described in section ten (10), hereafter called “Employee Purchase Footwear.”
c. Conduct receipt-inspection of Employee Purchase Footwear that is purchased from an outside source per section seven (7) part c.
d. Collect and report data on Standard Protective Footwear and Employee Purchase Footwear sales per section six (6).
e. Process employee Safety Shoe Chit forms, Attachment one (1), and Claim for Reimbursement for Expenditures on Official Business forms, Attachment two (2), per section seven (7).
2. Definitions
a. Standard Protective Footwear includes those styles described in section ten (10). Only the Standard Protective Footwear per section ten (10) is reimbursable under this contract. The Contractor will stock and issue the Standard Protective Footwear to employees who present a proper Safety Shoe Chit form, Attachment One (1).
b. Employee Purchase Footwear includes two types:
1) Subject to the Contractor will stock and provide employees with the option to purchase protective footwear, which are not the styles described in section ten (10). The employees must pay for Employee Purchase Footwear with their own personal funds. Such sales represent a private sale between the individual employee and the Contractor. These footwear sales are not reimbursable by the Government under this contract.
2) Protective Footwear that the employees purchase from an outside source. It is estimated that there are approximately 200 pairs per month of outside sales for an annual estimate of 2,400 shoes.
c. Employee(s) means “Government employee(s).”
d. Issue means to provide free of charge to the employee(s).
3. General Requirements
a. Protective footwear is intended to provide protection for the toes against external forces through the use of a protective toe box, which is incorporated into the footwear. Footwear must be marked and meet or exceed American Society of Testing Materials (ASTM) International standards, F2412-11, Standard Test Methods for Foot Protection or F2412-05, Standard Test Methods for Foot Protection, and F2413-11, Standard Specification for Performance Requirements for Protective (Safety) Toe Cap Footwear or F2413-05, Standard Specification for Performance Requirements for Protective Footwear. Footwear that meets the American National Standard Institute (ANSI) Z41-1999 standard is compliant with the new ASTM standards. At least one shoe of each pair shall be stamped, stitched, or have a sewn-in label on the inside of the footwear that indicates that the footwear meets the requirements stated (ASTM F2412-11 or ASTM F2412-05 and F2413-11 or F2413-05 or ANSI Z41-1999).
b. The Contractor shall display, distribute, sell, or issue only footwear that meets the ASTM F2412-11 or ASTM F 2412-05; and F2413-11, or F2413-05, or ANSI Z41-1999 requirements described
c. The Contractor shall assist the employee in fitting shoes, and will recommend the proper size of footwear. The Contractor shall provide all measuring devices and fitting tools required to provide the employee with the best fit.
d. Costs for maintenance such as resoling, reconditioning, or “cobbler” services are not included in the contract; therefore, are not reimbursable under this contract.
4. Facilities
a. The Government will provide a footprint inside the CIA at PSNS & IMF, Bremerton Washington, and as directed at Everett. Site visit will be coordinated after award.
b. The Contractor shall provide a trailer that is no larger than eight (8) feet high by twelve (12) feet wide by fifty-six (56) feet long at the –Bremerton, WA site and mobile trailers or trucks for Everett, WA site.
c. The Government will provide 120-volt electrical service at PSNS and IMF, Bremerton, WA, at no charge to the contractor. The Government will provide access to a restroom at all three sites at no charge to the Contractor. The Government will assist with installation of a telephone service connection for the Contractor’s use at Bremerton, WA. The Contractor will be responsible for the costs incurred from equipment, maintenance, and monthly billings of telephone services.
d. The facility’s primary use in Bremerton WA will be for the issuance of Standard Protective Footwear. Sale of Employee Purchased Footwear shall not interfere with the issuance of Standard Protective Footwear.
5. Standard Protective Footwear
a. The Contractor shall stock, inventory, fit, and issue the Standard Protective Footwear.
b. The Contractor shall prominently display a sign that distinguishes the “Free” Standard Protective Footwear from the “for sale” Employee Purchase Footwear. It must be clear that the employee has a choice between Free Standard Protective Footwear, or Employee Purchased Footwear paid for with their own personal funds.
c. If the issued footwear does not fit properly, the Contractor shall replace “unworn” footwear within thirty (30) calendar days after the employee has signed for receipt of footwear. Unworn is defined as showing no evidence of wear or soiling, such as when worn on carpeting only. If the Contractor determines that they are unable to properly fit the employee, the Contractor will direct the employee to their parent shop/code with the Safety Shoe Chit form marked Uncommon Size in area C of Attachment One (1).
d. If a manufacturer’s defect or material failure is discovered within 90 days of receipt (i.e. heels come loose, soles delaminate, or stitching separates) the Contractor shall replace the footwear at no expense to the Government or employee. The Contractor shall track all product defects and recommend removal of items from inventory upon identifying a trend. The Contractor shall provide an alternate style that meets the specifications to replace the -defective footwear. ”
e. The Contractor is not required to issue replacement footwear at the Contractor’s expense if excessive wear or abuse, such as cuts or burns from heat or chemicals, is apparent. If disagreements arise, the Contractor will direct the employee to their parent shop or code. Contractor is required to notify PSNS & IMF Production Planning Dept. S/06 P with details of the disagreement.
6. Employee Purchase Footwear
a. The Contractor shall prominently display in the trailer their standard commercial catalog that shows the style numbers and prices available for Employee Purchase Footwear. The Contractor shall ensure that the Employee Purchase Footwear meets the requirements outlined in Section 3, General Requirements.
b. The Contractor shall not charge more than their current published catalog price for the Employee Purchase Footwear.
c. The Contractor’s standard commercial return/refund policy will apply to all Employee Purchase Footwear sales. The policy must be clearly displayed.
7. Forms Processing
a. Safety Shoe Chit form, Attachment One (1), number PSNS 5100/67. The Contractor shall provide the original Safety Shoe Chit forms to the Production Planning Dept. S/06P daily. Blocks A and B must be completed prior to issuing or selling any protective footwear. The Contractor will complete Block C and utilize the Comments block for documentation of any problems encountered.
1) If the employee obtains footwear from the Contractor, Block E will be signed and dated. The Safety Shoe Chit form is kept by the Contractor until forwarded to PSNS & IMF Bremerton Production Planning Dept. S/06P daily.
2) If the employee is unable to be fitted, the Contractor shall check the Uncommon Size block, return the Safety Shoe Chit form to the employee, and direct the employee to their parent shop or code for further direction.
b. Claim for Reimbursement for Expenditures on Official Business form, Attachment two (2), Standard Form 1164. The Contractor shall provide the original Claim for Reimbursement for Expenditures on Official Business form to the Production Planning Dept S/06P daily. The Naval Station Everett location will provide this information weekly. The employee will enter their Name, Shop/Code, Badge number, and Mailing Address on the Form. The employee will sign and date Block 10. The completed reimbursement forms, Attachment Two (2), will be collected by the Contractor and forwarded along with the Safety Shoe Chit form, Attachment One (1); a completed Electronic Funds Transfer, PSNS & IMF 7200/18, (if needed and original purchase receipt to Production Planning Dept. S/06P daily. All supporting documents must state any of the following: “Safety footwear”, ASTM F2412-05 and F2413-05 or ANSI Z41-1999. It is the employee’s responsibility to have an Electronic Funds Transfer on file or make available to Code 600. The Contractor shall assure all required precautions and laws are followed when in the possession of and/or dealing with information protected by the Privacy Act 5 USC Section 552a.
c. Before signing the Safety Shoe Chit form, Attachment One (1), for employee purchase footwear that was purchased from an outside source, the Contractor will check the footwear for an ASTM/ANSI tag to ensure that it meets the ASTM/ANSI specifications. The Contractor shall not sign the Safety Shoe Chit and will direct the employee to their parent shop or code for further direction if any of the following are true:
1) Footwear does is not marked to meet ANSI or ASTM standards;
2) An incomplete or reproduced receipt or invoice is received;
3) An unauthorized or missing signature appears in Section B of the Safety Shoe Chit form, Attachment One (1), (a list of authorized signatory s will be provided by the Shipyard and will be updated periodically).
d. The Contractor’s monthly invoice to PSNS & IMF Bremerton will be entered into the Wide Area Workflow (WAWF) System and back-up documentation will be forwarded to Production Planning Dept. (S/06P) for verification of charges. Once accepted in WAWF, the invoice will be certified and sent electronically to DFAS Cleveland for payment.
8. Delivery. The Contractor shall maintain adequate stock of the Standard Protective Footwear to ensure that ninety percent (90%) of the requested footwear is available the same day. When footwear is not available for issue on the same day, the Contractor shall deliver the Standard Protective Footwear to the individual within seven (7) calendar days.
9. Hours of Operation:
a. The Contractor will maintain the following hours of operation for the following locations:
PSNS Bremerton:

b. The Contractor’s operation will be as stated in section nine (9) part a excluding the following Federal holidays:

Additionally, PSNS & IMF normally closes operations between Christmas and New Year’s Day. The decision to close is made yearly and is not predictable. If PSNS & IMF closes operations for a predefined period of time, the Contractor will not be required to operate the footwear trailer during that time. PSNS & IMF Code Production Planning Dept. S/06P or will notify the contractor by phone or in writing of such closures. When the Shipyard does not close, the Contractor shall keep the trailer open during the hours set forth in section nine (9) a. If by chance the Contractor cannot open the trailer on time, the COR shall be notified.

**Boots are for Military use and must meet the following description: Plain toed, laced safety boots, made of water resistant leather, black, and outsoles constructed from Nitrile rubber that is inherently heat and flame resistant.
11.IMF Bangor.
IMF base at Banger WA, is initiating their own safety shoe program effective the beginning of this contracts Period of Performance. In the event that their command runs into problems that they cannot recover from, this contract will provide a contingency base year, and four option years specifically for servicing Bangor base. Services provided will be identical to the services given at PSNS and Naval Base Everett with the exception of the dates described in section 9.
The table below shows the boot styles and the updated quantities for Bangor ONLY.

12. REPORTS.
The Contractor will maintain a database of sales information regarding Standard Protective Footwear, Employee Purchase Footwear sold by the Contractor, and Employee Purchase Footwear purchased from an outside source. The Contractor will provide an electronic copy of this database information by the 1st of each month to Production Planning Dept S/06P and or The report will contain the following information:
1) Name of Government employee (First and Last Name)
2) Parent Shop/Code
3) Badge Number
4) Name corresponding to Authorizing Signature
5) Identify if the shoes issued/sold were Standard Protective Footwear or Employee Purchase Footwear (Note: It is not necessary to distinguish between whether the Employee Purchase Footwear was purchased inside or outside of the Shipyard.)
6) Price of shoe issued / sold and any credit for returns
7) Date of issue / sale
8) Location of Sale
9) Size of footwear issued / sold
10) Style Number of footwear issued / sold (not applicable to footwear purchased from a source outside the Shipyard)
The electronic report must be submitted in a Microsoft-based product (i.e. Excel or Access) so that it may be accessed using existing Shipyard software. The media used for data transfer must be large enough to hold the volume of data collected during the month (e.g. Zip file, CD-Rom, etc.).
