TO REFLECT THE FOLLOWING: Provide answers to questions submitted by interested vendors (see page 2 of this amendment).

1. It appears the VA is going to provide the Inspector for all testing and inspections, is that correct?
VA: The VA will provide third party elevator inspector(s) and inspection service to perform Annual and Semi-Annual elevator Inspections per ASME Codes.
As well as third party elevator inspectors to oversee and certify the 5 (five) year load tests as specified in the bid schedule.
See Paragraph 1. General Requirements for additional information and requirements.
2. The keyed switches on the specified elevators (Medical emergency/Independent service) Are they provided through the VA as it is there vendor or are we responsible for providing that?
If we are to provide do we know which of the 2 vendors you are currently using?
VA: The requested by contract keys & key cores are to be provide and installed by the contractor. Qualified vendors are:
• Block Iron and Supply, 3121 Syene Rd, Madison or Block Iron and Supply, 1016

3. Are we responsible for contracting the lift-net vendors of is that going through the VA contract for their portion of the repairs and upgrades?
VA: This contract/contractor is responsible for the Lift Net repair and upgrades. See
Paragraph 9. D. vi.
4. The dumbwaiters 9 & 10 to be removed. Can we leave the rail system in place they take up very little space and would provide a significant savings in price?
VA: It is acceptable to leave in place the rear (of the shaft) U channel rail support system and the front of the shaft angle iron support rail. All other items are to be removed Per Paragraph 5. K.; Base Year Only Items.
5. Is our monthly price to include the potential major repairs like the following:
Hoist machine, hoist motor and breaks; Motor drives; Control boards; Hoist cables; Idler sheaves and deflector sheaves; Buffers; Governors; Safety’s; Rope grippers and the like?
VA: Yes, It is the intent of this monthly maintenance contract price to include all of the potential major items in question. Paragraph 10. F. i through xxiii lists these items. All
‘Safety’s & Rope grippers’ are included.
