DELIVERY TIME FRAME: Staggered monthly deliveries; with first delivery made 10 days (ARO) after receipt of order, over a 4-month period.

All responsible sources may submit a quotation, which if received timely, shall be considered by this agency.

Vendor must be registered and current in SAM.gov.

Responses must be concise and be specifically directed to the requirement referenced above. It is the offeror’s responsibility to monitor SAM.GOV for changes or amendments.

Offeror shall supply their state wholesale distributor licensure with offer, verifying compliance with the Drug Supply Chain Security Act (DSCSA). Vendors that fail to submit a copy of their state license shall be deemed non-compliant.

All solicitation packages will be submitted via email.

Solicitation Packages must include the following:

1. SF1449 - Solicitation cover page (Signed)

2. Quote - Pricing Schedule (Excel format) including Place of Manufacture.

3. State Wholesale Distributor License must be unexpired and valid at the time quotes are due

4. Buy American Act (BAA) Certificate: if quoting non-domestic product.

Solicitation Packages failing to include required documents/ information shall be deemed non-compliant.
