About this file

This is a Justification for an Exception to Fair Opportunity document for the Department of Veterans Affairs (VA), Veterans Integrated Service Network 8 (VISN 8). The VA seeks to procure a 12-month firm-fixed price delivery order under the NASA Solution for Enterprise-Wide Procurement (SEWP) for HealthLevel Inc. Foundations Radiology Analytics software licenses, maintenance, and technical support. The software is critical for VISN 8's radiology operations, managing an annual case load of 1.6 million radiology studies across seven VA facilities, and integrating with the VistA imaging system to provide comprehensive analytics, order management, and performance tracking.

The document justifies a sole-source procurement from HealthLevel Inc. due to the proprietary nature of its Foundations analytics platform, which is deeply integrated into VISN 8's existing infrastructure. Market research conducted in May 2025 confirmed no alternative vendors can meet the VA's technical requirements, particularly the system's ability to interface with VistA, process SIU appointment messages, and support the complex radiology workflow. The proposed action includes software subscription, maintenance, and technical support, with options for two additional 12-month renewal periods. Failure to renew would significantly disrupt patient care, medical services, and critical medical information systems.

VISN 8 Foundations Radiology Analytics

JUSTIFICATION FOR AN EXCEPTION TO FAIR OPPORTUNITY

1. Contracting Activity: Department of Veterans Affairs (VA) Office of Procurement, Acquisition, and Logistics Technology Acquisition Center 23 Christopher Way Eatontown, NJ 07724

2. Description of Action: The proposed action is for a firm-fixed price delivery order issued under the National Aeronautics and Space Administration (NASA) Solution for Enterprise-Wide Procurement (SEWP) Government Wide Acquisition Contract (GWAC) for brand name HealthLevel Inc. Foundations Radiology Analytics term software licenses, maintenance, and technical support to ensure continuous service for Veterans Integrated Service Network (VISN) 8 Picture Archive and Communication System (PACS) system and all associated modules..

3. Description of the Supplies or Services: The Department of Veterans Affairs, Veterans Integrated Service Network 8 (VISN 8), requires a renewal requirement for brand name HealthLevel Foundations Radiology Analytics (Foundations) term software subscription, maintenance, and technical support to maintain its Foundations system. VISN 8 has an annual case load of 1.6 million radiology studies, which each consist of one or more exams, per year and has experienced a drastic increase in required Radiology studies. Volumes, which are grouped studies per year, are based on digital imaging and communications in medicine(DICOM) Study Instance Unique Identifier (SIUID) reported from the VISN 8 PACS system.

VISN 8 uses HealthLevel’s Foundations software to gather Veterans Health Information Systems and Technology Architecture (VistA) data from VISN 8 Facilities. The Foundations system gathers the aforementioned studies and volumes so that VistA can integrate it into the patient record. The VistA imaging system integrates clinical images, scanned documents and other non-textual data into the patient's electronic medical record. VistA data includes imaging order data, scheduled appointments, lab results, and imaging reports, which were combined in the Foundations database to perform extensive analytics and customized real-time reports for Radiology leadership. The use of the Foundations system with Order Management, Workload and Appointment Analysis, Case Data Analytics, Radiology Order Protocoling, Ongoing Professional Practice Evaluation (OPPE)/Peer Review, and Patient Flow modules has improved core analytical functionality while providing a streamlined capability of transcribing information through multiple Radiology platforms, inclusive of PACS and VistA. Foundations serves as the foundational infrastructure for the entire VISN 8 imaging ecosystem, supporting hundreds of providers, thousands of Veteran patients, and managing millions of diagnostic images. The current VISN 8 infrastructure is dependent on the Foundations platform, which integrates and consolidates medical imaging performance insights, data governance, and security into a source-of-truth with analytics to align administrative and clinical staff. This integrated Foundations infrastructure empowers clinical and administrative teams with actionable insights, improves data governance, and ensures consistent, high-quality care delivery across all VISN 8 medical centers and imaging sites. The VA requires software that provides core analytics functionality for
