The Government will assess the past performance on projects submitted under the Specialized Experience Factor and reserves the right to conduct telephone interviews with points of contact identified in the proposal and/or to review other recent projects rated in CPARS or other agency performance databases, review and/or contact Offeror-furnished references, or to review personal knowledge. The Government will first assess the relevancy of recent projects accomplished by the offeror to the scope of this contract and consider how well the Offeror performed on the contracts. The Government will consider the currency and relevance of the information, source of the information, context of the data, and general trends in contractor performance. The more recent and relevant experience, the more credit will be given. PHASE TWO EVALUATION FACTORS

**DO NOT SUBMIT PHASE TWO PROPOSALS UNLESS SPECIFICALLY INSTRUCTED BY THE CO.**

PHASE TWO EVALUATION FACTORS: The evaluation factors for PHASE TWO consist of Factor 1 – Design Concepts and Technical Solution, and Factors 2 – Schedule. Each factor shall have equal importance, and when combined, are significantly more important than price.

PHASE TWO - FACTOR 1: DESIGN CONCEPTS AND TECHNICAL SOLUTIONS

(20 pages max. excluding schematics)

Offeror shall submit a narrative description of how they intend to approach the scope of work to successfully complete design-build services for this project in accordance with solicitation requirements.

Provide at least one (1) 24 x 36 schematic illustrating your proposed layout but not a detailed design.

• The description shall include what manufacturer the offeror is proposing as their solution and a written narrative of the planned phasing so that the facility remains operational while the window and its components are being installed. The narrative will also detail how the offeror intends to prepare the site and install windows with all required connections to the facility within any specified time limits.

• Explain how project will be accomplished to ensure minimal interruptions with daily hospital operations and patient care.

• The description shall include in detail approach to construction safety and infection control.

• Demonstrate techniques for management of work coordination with subcontractors and measures/corrective action that will be taken for substandard performance.

EVALUATION CRITERIA PHASE TWO, FACTOR 1 – DESIGN CONCEPTS AND TECHNICAL SOLUTIONS:

Based on the narrative provided, the Government will evaluate the Offeror’s design concepts and technical solution to this design build project. The Government will evaluate the strengths, weaknesses, and deficiencies, if any, and assign one combined technical and risk rating for this factor. All requirements in this factor shall be met by the closing date of Phase Two submission.

PHASE TWO - FACTOR 2: SCHEDULE

(No page limit)

• Submit a preliminary (CPM) Critical Path Method schedule for the design and construction satisfying the requirements of this project. A practicable schedule showing the order in which the Contractor proposes to perform the work, and the dates on which the Contractor contemplates starting and completing the several salient features of the work (acquiring materials and equipment, mobilization, demolition, installation, commissioning, etc.). The schedule shall be in the form of a progress chart (Gantt chart) of suitable scale to indicate appropriately the percentage of work scheduled for completion by any given point during the period. Most importantly, indicate length of downtime required for any utilities affecting the operation of other parts of the facility on the schedule. The schedule shall show key milestone dates and switchover and start-up of equipment. The schedule is not included in the page limit.

To be considered technically acceptable, the proposed “draft” project schedule shall not exceed the project duration stated on the solicitation and include divisions of work required to successfully complete the project:

o Identify the tasks required to execute the project, being as detailed as possible using a work breakdown structure to break down the tasks into manageable elements in the logical order of execution.

o Identify the time for each task.

o Set start and finish times.

o Calculate the slack.

o Determine intermediate phases and total project duration.

o Clearly identify the critical path

Narrative - 5 page maximum

• The offeror shall submit a detailed project schedule (including all project stages) in a time scale bar graph format to demonstrate an efficient project planning for the works being procured.

The horizontal axis will be scaled for time beginning with the Notice to Proceed and concluding with contract completion. Assume the Notice to Proceed will be issued fifteen (15) calendar days after the award date. (Note that there is no guarantee the Notice to Proceed will be issued within this timeframe.)

• The vertical axis will show the milestones and major portions of the contract work. All schedule items will show a start date and a completion date. The detailed schedule will indicate specific tasks with dates for each step of the process including:

o Design Period: The design period sub periods (i.e., first and second reviews, other meetings, internal QUALITY ASSURANCE /QUALITY CONTROL plan reviews, etc.)

o Construction Period: Mobilization; Demolition method, equipment shutdowns, testing and certifications; Demobilization.

• The Offeror shall specify days of the week and the hours of construction operations during each phase of the work, and the percentage of contract completion that will be achieved at the end of each month of the contract.

• The Offeror will provide a written commitment as to the time frame (number of days after receipt of the notice to proceed) within which the Offeror will guarantee completion. The maximum anticipated completion of this project (including design and construction of the entire project) is indicated in the solicitation. Discuss a proposed approach for ensuring the project stays on schedule.

EVALUATION CRITERIA PHASE TWO, FACTOR 2 – SCHEDULE: The Government will evaluate and use as an aid to help determine that the Offeror understands and can perform all of the required elements to complete the project timely with limited impact, to the greatest extent possible, to the Medical Center.

The strengths, weaknesses, and any deficiencies in the schedule to successfully execute the project will be evaluated, and a one combined technical and risk rating for this factor will be assigned. All requirements for this factor shall be met by the closing date of Phase Two submission.

PHASE TWO EVALUATION AND SELECTION CRITERIA: All evaluation factors, other than price, when combined, are significantly more important than price. Price may be the determining factor if offerors are essentially equal in technical adjectival rankings.
