PROJECT 556-23-100 CAMPUS-WIDE LAND AND UTILITY SURVEYING

STATEMENT OF WORK

NAICS Code: 541370 Surveying and Mapping (except Geophysical) Services

Small Business Size standard: $16.5 Million applies.

Period of Performance: 240 days from Notice to Proceed

I. BACKGROUND AND GENERAL INFORMATION

1. The Captain James A. Lovell Federal Health Care Center (FHCC), located at 3001 Green Bay Road North Chicago, IL 60064, has a requirement to obtain campus-wide professional land surveying services to locate and record boundaries, structure data, and utilities above and below ground, across approximately 108 acres, and approximately 64 individual structures totaling 1.8 million square feet, some dating back to 1925, but with significant clinical facilities of modern construction.

2. The survey shall include property lines, roads, lots, parcels and easements, surface topography, and the square footage and elevation for each building. The survey shall include all utility systems, identifying all parts of each – whether underground or pole-mounted. Utility systems include campus domestic water, fire hydrants, sanitary sewer, storm sewer, steam transmission and condensate return piping, inter-building fiberoptic and other telecommunications cable including that serving security systems, electrical power distribution, and campus lighting.

3. Services shall include, but not be limited to, the furnishing of all materials, labor, equipment, tools, supervision, and miscellaneous items necessary to complete this requirement.

4. The work shall be completed during normal business hours. Any work needing to occur outside normal business hours shall be included in the Surveyor’s proposal, and will require advance approval by the COR.

5. The Contractor shall not be admitted to areas of the property other than those designated for this project.

6. The Contractor shall remain solely responsible for providing, installing, and maintaining all safety barricades and signage needed to keep its work area safe and separate from pedestrian and vehicular traffic. Well in advance of need, the Contractor is required to coordinate and collaborate through the VA COR with the VA Police Department at this facility to develop and gain approval of a multi-phased traffic safety plan.

II. DELIVERABLE SCHEDULE

1. Following Notice to Proceed, the Contractor shall commence work under this contract and continuously progress until achievement of VA final acceptance of the project deliverables.

2. Contractor shall provide a detailed Critical Path Method time schedule for the project, including the start and finish dates for the project and each phase of it, and all activities within each to complete the work. The goal is not only to establish a management plan for the Contractor to effectively work from, but a tool which enables VA to plan

Scope of Work: 556-23-100 Page 2 of 13 supporting activities and measure progress.

3. Provide a fully loaded cost schedule. All sub-consultants must be assigned and accountable for activities. Break out all costs by phase. Do not front-load the submission (e.g., shift design effort to Schematic Design). Use of the three required VA pay application forms, submitting them as Pay App 00 as a Schedule of Values, which when approved also ensures that the pay applications are also formatted correctly.

4. Prepare and submit for review and acceptance a proposed baseline schedule reflecting each phase of the work, each critical activity, and each essential task required to optimally meet the objectives of the project. It is expected that this timeline will be shorter than that presented in the Period of Performance, which is part of why the Design Development phases are conglomerated here (to enable optimization). Upon acceptance, this will become the Baseline Schedule from which subsequent progress updates are developed and reported, and how progress payment applications are objectively reviewed. Baseline schedules lacking the detail required will be rejected.

Listing activities required of VA along the critical path must be included.

5. List of all architect/engineer sub-consultants, including: The discipline they were retained to support or fulfill, company name, office address, office telephone, and primary point of contact (name with telephone number and email address) for each discipline.

3. The Surveyor shall research all public and VA facility survey record documents related to this project to compare to survey findings to produce a set of drawings and documentation as described within this Scope of Services, including its attached specification section 02 21 13 Site Survey (attached).

4. The Project Site Manager shall attend all project meetings, prepare Status Reports on the project, and submit them to the COR. Status Reports shall contain meeting minutes, accomplishments, arising concerns, and proposed solutions, any proposed changed orders, and any other pertinent information required to report the progress of performance.

5. The Surveyor shall provide drawings and reports, based on survey findings, noting all discrepancies, and submit them to the VA Contracting Officer and COR.

6. Pay Application Process:

Exclusive use of the following (three) VA forms is required. Fillable PDF versions are available from the COR. The forms at C and D are also available pre-populated with GLAC or PCAC information. Do not submit any other documents

1) Contractors Request for Progress Payment/Invoice summarizes the VA Form 10- 6001a information, and includes a Principal certification/attestation signature block.

The owner/Veteran must transmit a written delegation allowing others to sign on behalf of the firm to the Contracting Officer. This typically works with the same delegation enabling senior employees to make contract decisions on behalf of the firm.

a) Project Number and Title must match the VA number and naming.

b) Verify the correct Contract, Task Order, and PO numbers, as well as the COR and CO Names (which may change along the way).

c) Begin with Invoice # 0 for the initial draft and SOV submission at Kick-Off.

d) Prior to submission, verify that the Invoice Date matches the date of transmission to the VA COR for review. Back-dated submissions will be rejected.

e) Statement of Costs Under this Contract Through date is always the end of the billing period month – even when submitting “Pencil Copy” for early review mid-month – except for the final payment.

f) Actual Percentage Complete is intended to report design progress, not contract cost spending progress.

g) Verify that the entries here match those of the other forms.

h) If your PDF software is set to lock the document after you sign it, uncheck those settings. Minor corrections may be made by the COR to enable approval without returning for revision and delaying approval.

Scope of Work: 556-23-100 Page 7 of 13

2) VA Form 10-6001a Contract Progress Report shall be used to prepare a Schedule of Values (SOV), which when approved will become the baseline/template from which to develop subsequent progress report portions of invoices.

https://www.va.gov/vaforms/medical/pdf/vha-10-6001a-fill.pdf

a) Report for Period Ending is always the end of the billing month.

b) Project Number and Title must match the VA number and naming.

c) Verify the correct Contract number, and Award and NTP dates. Date Const.

Started is not applicable, or may report when design actually started.

d) Schedule performance data at boxes 9 and 10 are established by the approved Baseline Schedule. The Baseline Schedule cannot be changed without CO approval.

e) Contract Completion Date at box 11 is the base contract completion date for design. If the period of performance is extended by the CO, such as awarding CPS, the Extended Completion Date at box 12 will reflect that.

f) Lines 15 through 37 list the several parts of the work in phases, major parts of phases, major activities, or other objective measurable parts of the work never lasting more than a billing period.

g) Lines 38 and 39 are specific to contract modifications of cost, whether added or deducted, correlating to Lines 55A through 60.

h) Disregard boxes 43 and 44, and 51 through 54.

3) Subcontractor Payment Summary, like the 10-6001a, is prepared and submitted at Kick-Off with the other required documents.

a) Project Number and Contract number must match the VA number.

b) Verify the correct Award date.

c) Report for Period Ending is always the end of a billing month.

d) The values here must match those of the 10-6001a.

e) All consulting firms must be listed, along with their SDVOSB or VOSB status (pre-verified in SAM) if their participation is expected to be considered as part of self-performance compliance.
