About this file
This is a pricing schedule for a Request for Quote (RFQ) issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 15 for medical and surgical supplies to support the Chelmsford CMOP (Consolidated Mail Outpatient Pharmacy) facility. The solicitation number is 36C77027Q0001, with a closing date of October 12, 2026, at 5:00 PM Pacific Standard Time. Quotes must be submitted via email to Jennifer.Knight@va.gov using the provided Excel spreadsheet, along with required documentation including SF1449, OEM authorized dealer or distributor certification, and a Certificate of Compliance for Line Item 2.
The RFQ contains three line items with a partial set-aside structure: Line Item 1 (Heat and Moisture Exchanger Provox 8262, 720 units) and Line Item 3 (Catheter Set Intermittent 14FR, 400 units) are full and open competition, while Line Item 2 (OneTouch Verio Glucose Test Strips, 2,400 units) is reserved exclusively for Service-Disabled Veteran-Owned Small Businesses (SDVOSB). The pricing schedule requires vendors to provide unit pricing and total pricing for each line item. All quoted pharmaceuticals and medical surgical supplies must have expiration dates no less than one year from the delivery date. The solicitation enforces Buy American Act compliance and includes strict gray market prevention requirements, mandating that vendors be OEM-authorized dealers or distributors with proper authorization documentation, as any gray market items delivered constitute a breach of contract. Only one quote per vendor is permitted.

Gray Market Prevention: Deviation to policy allows non-manufacturing vendors selling medical surgical supplies to satisfy the Gray Market Prevention language by providing letters of authorization from a manufacturer's authorized wholesaler/reseller via an authorization letter directly from the manufacturer.
(a). Gray market items are Original Equipment Manufacturer's (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment, and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactured or gray market items will be accepted.
(b). Vendor shall be an OEM authorized dealer or authorized distributor for the proposed medical supplies, medical equipment, and/or services contracts for maintenance of medical equipment (i.e. replacement parts). Verified by an authorization letter or other documentation from the OEM, such that the OEM's warranty and services are provided and maintained by the OEM. All software licensing, warranty and services associated with the medical supplies, medical equipment, and/or services contract for maintenance of medical equipment shall be in accordance with the OEM terms and conditions.
(c). The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. this includes termination of the contract or, solely at the VA's election, allowing the vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
