About this file

This is a pricing schedule template for a Drawtex dressing procurement under VA contract solicitation 36C77026Q0134. The schedule is designed to capture unit pricing for Drawtex dressings being procured through the Department of Veterans Affairs Veterans Health Administration, Veterans Integrated Service Network 15 for the Charleston CMOP (Consolidated Mail Outpatient Pharmacy) facility. The pricing template establishes the framework for vendors to submit their costs; however, the specific line items, quantities, delivery schedules, and detailed pricing terms are not populated in this version, indicating this is a blank template awaiting contractor completion during the proposal submission phase.

As a pricing schedule template, this document serves as the standardized format through which competing vendors must structure their financial proposals for the Drawtex dressing supply contract. The template ensures consistent pricing submission across all proposals, enabling the VA to conduct fair and comparative evaluations. Contractors responding to solicitation 36C77026Q0134 are required to complete this schedule with their proposed unit prices and total costs according to the specified delivery locations and periods outlined in the broader solicitation documents.

Gray Market Prevention: Deviation to policy allows non-manufacturing vendors selling medical surgical supplies to satisfy the Gray Market Prevention language by providing letters of authorization from a manufacturer's authorized wholesaler/reseller via an authorization letter directly from the manufacturer.

(a). Gray market items are Original Equipment Manufacturer's (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment, and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactured or gray market items will be accepted.

(b). Vendor shall be an OEM authorized dealer or authorized distributor for the proposed medical supplies, medical equipment, and/or services contracts for maintenance of medical equipment (i.e. replacement parts). Verified by an authorization letter or other documentation from the OEM, such that the OEM's warranty and services are provided and maintained by the OEM. All software licensing, warranty and services associated with the medical supplies, medical equipment, and/or services contract for maintenance of medical equipment shall be in accordance with the OEM terms and conditions.

(c). The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right to enforce any of its contractual remedies. this includes termination of the contract or, solely at the VA's election, allowing the vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items.
