JUSTIFICATION FOR OTHER THAN FULL AND OPEN COMPETITION
1. Contracting Activity: Department of Veterans Affairs (VA) Office of Procurement, Acquisition, and Logistics
Technology Acquisition Center 23 Christopher Way
Eatontown, NJ 07724
2. Description of Action: This acquisition is for an indefinite-delivery/indefinite-quantity (ID/IQ) requirements contract for brand name Remote Patient Monitoring (RPM) solution inclusive of brand name Global Telehealth Services – VirtualHealth (GTS-VP) RPM Tool Software as a Service (SaaS) subscriptions, maintenance, and support. All performance shall be completed by June 30, 2027. Each task order issued will delineate the specific period of performance and/or delivery schedule(s).
3. Description of the Supplies or Services: VA Veterans Health Administration (VHA), National Spinal Cord Injuries/Dysfunction (SCI/D) Office has a requirement for 30 GTS- VP RPM SaaS subscription licenses to provide a broad range of clinical and Veteran needs. Specifically, this comprehensive RPM SaaS subscription will allow VA SCI/D providers to compliment traditional care with this virtual modality, providing early interventions enable providers to consider a vast number of options in the Veteran’s treatment plan and allow for medical grade continuity of care, no matter where the SCI/D Veteran resides to allow multiple VA facilities to utilize this product throughout the VA enterprise. More specifically, this action will provide for the procurement of brand name GTS-VP RPM SaaS subscriptions, maintenance support to expand upon provider options currently available for Veteran’s treatment plan. VA intends on leveraging this SaaS subscription to provide specialized care for Veterans that are spread out geographically and dispersed nationally. VA requires a FedRAMP authorized, device-agnostic SaaS to compliment traditional care and ensure that an array of existing VA owned hardware can assist VA providers in monitoring Veteran’s chronic conditions such as diabetes, obesity, cardiovascular disorders and other conditions. Due to moderate Federal Risk and Authorization Management Program (FedRAMP) approval requirement, the Contractor shall operate with Veteran-owned data (home to VA), as well as VA-owned data (VA to VA). This means the brand name GTS VirtualHealth RPM tool can be accessed from VA computers and existing Veteran owned devices in a remote Community Based Outpatient Clinic (CBOC) or VA clinic, while allowing an additional option in the treatment of SCI/D Veterans in the VA SCI/D System of Care.
To ensure VA establishes a procurement vehicle flexible enough to allow stakeholders throughout the enterprise to fulfil their needs throughout the Period of Performance (PoP) optional tasks are included to allow VA to not to exceed a total of 5,300 SaaS subscriptions throughout the life of the contract. There are approximately 25,000 SCI/D Veterans throughout the country. After extensive analysis, VA technical experts have estimated that approximately 5,300 SCI/D Veterans could be treated throughout the country during this estimated period of performance. The optional tasks shall be prorated based on the number of months remaining in the period that it was exercised therefore all subscription shall be co-terminus, ending on the same date for the base or
Remote Patient Monitoring Control Number VA-23-00034182 option periods. Following this approach would allow one contract vehicle to serve VHA requirements while allowing the VA to get to the product “faster” and eliminating the SaaS backlog to get other tools authorized considering the backlog is expected to grow once the enterprise contract is awarded. The SaaS subscription tool will allow for a variety of care options that will enable providers to consider a vast number of options in the Veteran’s treatment plan. Maintenance and support of the brand name GTS-VP RPM Tool are required. Specifically, maintenance includes access to the SaaS tool capabilities, patient and care provider applications, and functionalities which includes updates to the SaaS subscription, digital User Guides, to ensure updates to mobile applications are up to date and helpdesk support. The period of performance for the proposed action shall consist of a 12-month base period and three 12-month option periods for continued access to SaaS and two optional tasks for additional license subscriptions and specialized training. Based on deployment efforts of similar size and scope, VA requires a base period and three 12-month option periods to allow SCI/D to fully implement the program to eligible SCI/D patients. All optional tasks shall be prorated based on the number of months remaining in the period that it was exercised therefore all subscription shall be co-terminus, ending on the same date for the Base or Option Periods.
4. Statutory Authority: The statutory authority permitting other than full and open competition is 41 U.S.C. 3304(a)(1) as implemented by the Federal Acquisition Regulation (FAR) 6.302-1 entitled, “Only One Responsible Source and No Other Supplies or Services Will Satisfy Agency Requirements.”
5. Rationale Supporting Use of Authority Cited Above: Based on extensive market research, as described in section eight of this justification it was determined that limited competition among resellers is available for the aforementioned requirement. This procurement will allow use of the RPM solution to enable VA to expand upon provider options currently available for Veteran’s treatment plan throughout the entire VA enterprise.
Only the required GTS-VP RPM SaaS is capable of meetings VA’s requirements for a single platform through which Veterans may be treated for a range of conditions and/or disease states using Bluetooth enabled remote medical devices. The GTS- VP RPM SaaS uniquely meets critical VA requirements including the ability to collect accurate vitals, communicate via secure messages, video capability, self-survey and has the ability to provide patient education via a patient facing application. By utilizing the self-survey Veterans will have the ability to provide their own self-assessments based on their specific ailment to ensure that there is continuous two-way communication which results in better ongoing patient engagement. Only the required GTS-VP RPM SaaS can meet VA’s functional requirements above and meet the requirement to be device agnostic. This is a critical requirement as it will allow VA the ability to integrate with an array of existing tools in place throughout the enterprise for continuous patient monitoring. Being that the RPM SaaS is device agnostic VA will have the ability to integrate existing and future tools such as heart monitors, blood pressure monitoring tools and glucose monitoring tools to ensure that VA is able to remain up to date and avoid vendor lock in when it comes to hardware in the future. Additionally, the brand name GTS-VP RPM SaaS is the only SaaS that currently has the capability to integrate with VA’s existing authentication method to allow users to long into the system by using VA’s Single Sign-On (SSO) with Personal Identity Verification (PIV) cards, a critical VA requirement.
Failure to procure the required brand name GTS-VP RPM SaaS tool maintenance and support would not allow SCI physicians to monitor patients remotely or provide for remote management of patient’s vitals for chronic health conditions and allow for accurate monitoring. Without this required functionality, VA would put the health of its Veterans in jeopardy.

In accordance with FAR 6.302- 1(c)(1)(ii)(c) and FAR 5.102(a)(6) this justification shall be posted with the solicitation.
Furthermore, the proposed action will be synopsized on the Contracting Opportunities Page in accordance with FAR 5.201. Any proposals that are received shall be evaluated in accordance with the RFQ.
