About this file
This is a Single Source Justification form for a federal procurement under Simplified Acquisition Procedures for purchases exceeding the Micro-Purchase Threshold but not exceeding the Simplified Acquisition Threshold.
The Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office 16, is procuring four (4) Verathon GlideScope Core 15 FHD airway visualization systems and one hundred forty-four (144) OEM extended monitor warranties for the G.V. "Sonny" Montgomery VA Medical Center in Jackson, Mississippi. These systems support anesthesia, surgical, and critical care operations by providing high-definition multimodal visualization of the airway during intubation and diagnostic assessments. The justification establishes that the GlideScope Core 15 possesses unique technical and clinical characteristics that limit availability to a single source: the system provides multimodal imaging with picture-in-picture capability for simultaneous video laryngoscope and bronchoscope use, critical to the facility's clinical workflow. The facility's surgical and sterile processing staff have been trained exclusively on Verathon systems and OEM-validated reprocessing procedures, and the Clinical Products Review Committee has approved use of this specific system. The contracting officer conducted extensive market research across VetCert, DSBS, SAM.gov, AbilityOne, GSA eLibrary, ECMS, and other procurement systems, identifying 978 SDVOSBs/VOSBs under relevant NAICS codes but zero firms other than the named SDVOSB distributor capable of supplying the GlideScope Core 15. OEM confirmation explicitly states that Verathon has only one authorized SDVOSB distributor to the Department of Veterans Affairs, supporting a brand-name SDVOSB sole-source procurement consistent with 38 U.S.C. 8127(b) and VAAR 819.7008 and 819.7009.

VHAPG Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Single Source Justification for MPT up to SAT
Revision: 02 Effective Date: 12/30/2025 Page 1 of 3 Acquisition ID#
DEPARTMENT OF VETERANS AFFAIRS
Justification for Single Source Awards IAW FAR 13.106-1 For
Over Micro-Purchase Threshold but Not Exceeding the Simplified Acquisition Threshold
(Brand Name)
1. Contracting Activity:
Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office 16, in support of Department of Veterans Affairs, VISN 16, V.
“Sonny” Montgomery VA Medical Center, 1500 E. Woodrow Wilson Drive, Jackson, MS 39216
2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
The G.V. “Sonny” Montgomery VA Medical Center requires four (4) Verathon GlideScope Core 15 FHD airway visualization systems and one hundred forty-four
(144) OEM extended monitor warranties to support anesthesia, surgical, and critical care operations. The GlideScope Core 15 is a high-definition multimodal visualization platform used to obtain accurate and reliable visualization of the airway during intubation and diagnostic airway assessments. These systems are used daily within surgical suites and anesthesia care areas, where they directly affect patient safety and procedural success.
The estimated total value of this requirement is
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
The Verathon GlideScope Core 15 possesses unique technical and clinical characteristics that limit availability to a single brand and a single source. The system provides multimodal imaging with picture-in-picture capability, allowing simultaneous use of both the video laryngoscope and bronchoscope — a capability critical to the facility’s current clinical workflow. These specific instruments have been standardized at the G.V. “Sonny” Montgomery VAMC, and both surgical and sterile processing staff have been trained exclusively on Verathon systems and OEM-validated reprocessing procedures. Use of any alternative product would require significant retraining, introduce risk in sterile processing compliance, and disrupt established clinical protocols.
Effective Date: 12/30/2025 Page 2 of 3 Acquisition ID#
The Clinical Products Review Committee (CPRC) has approved the use of this exact system, confirming that deviations from the approved brand would create delays in patient care and impose additional training and compliance costs. The OEM, Verathon, confirmed in writing that distribution of the GlideScope Core 15 is restricted and that is the sole SDVOSB authorized distributor of this product line to the Department of Veterans Affairs.
Because Verathon restricts distribution to a single SDVOSB channel and because only the GlideScope Core 15 meets the clinical, technical, and reprocessing standards currently in use at the facility, no other products can satisfy the Government’s minimum needs. Pursuant to 38 U.S.C. 8127(b), and implemented under VAAR 819.7008 and 819.7009, an SDVOSB sole-source award is appropriate when a single verified SDVOSB is capable of meeting a brand-name requirement.
4. Description of market research conducted and results or statement why it was not conducted:
In accordance with FAR Part 10, VAAR 810, and VAAM M810.002-70, extensive market research was conducted across all applicable Government and commercial databases. The Contracting Officer and Contract Specialist performed searches within VetCert, DSBS, SAM.gov, AbilityOne, GSA eLibrary, ECMS, and other procurement systems.
Although 978 SDVOSBs/VOSBs were listed under NAICS 339112, the searches identified zero firms (other than ) authorized or capable of supplying the GlideScope Core 15 system. All mandatory sources were reviewed and found to have no ability to provide Verathon equipment.
DSBS returned zero capable small businesses, including zero SDVOSBs, VOSBs, 8(a), HUBZone, or WOSBs with the ability to provide anesthesiology GlideScope equipment.
Further, OEM confirmation explicitly states that Verathon has only one authorized SDVOSB distributor:
Because GlideScope is an OEM-controlled medical device requiring strict adherence to FDA, OEM, and VA reprocessing standards — and only one SDVOSB is legally authorized to sell to VA — no alternatives exist that meet the Government’s minimum needs. Therefore, the market research supports a brand-name, SDVOSB sole-source procurement consistent with 38 U.S.C. 8127(b).

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR 13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
