About this file

This is a Past Performance Questionnaire (PPQ) template used to evaluate contractor performance on federal contracts. The form is completed jointly by contractors and their clients to document and assess work quality, schedule adherence, cost management, and overall project execution.

The questionnaire is divided into two main sections: contractor-completed blocks (1-4) capturing firm information, contract details, and project descriptions; and client-completed blocks (5-8) documenting client contact information and role in the project. The evaluation criteria are organized into seven performance categories: Quality (technical data preparation, quality standards compliance, problem resolution, quality control); Schedule/Timeliness (delivery schedule compliance, resource utilization); Customer Satisfaction (end-user satisfaction, contractor cooperation, business-like conduct); Management/Personnel/Labor (on-site management, workforce qualification, government property control, personnel expertise, small business utilization, multi-project management, change management, overall program effectiveness); Cost/Financial Management (price term compliance, innovative cost-reduction methods, invoice accuracy, accounting system adequacy, contract termination history, financial problems); Safety/Security (safety environment maintenance, security compliance); and General (emergency response, contractual compliance, future work willingness, overall rating). Each criterion uses a six-point rating scale: Exceptional (E), Very Good (VG), Satisfactory (S), Marginal (M), Unsatisfactory (U), and Not Applicable (N). The questionnaire concludes with a narrative section requiring brief comments on strengths, weaknesses, and deficiencies. The form specifies that clients must submit completed questionnaires directly to the offeror, who will then submit to Virginia procurement with proposals, and may be duplicated for future solicitations.
