PREPARATION OF PROPOSALS

This is a competitive Request for Proposal (RFP) that represents the best value to the Government conducted under Federal Acquisition Regulation (FAR) Subpart 15.1 using the tradeoff process and evaluated in accordance with RFO Subpart 15.3, Source Selection. This section describes the method for evaluating and selecting offers for this procurement.

The Government will not pay for any costs incurred in the preparation and submission of proposals.

PROPOSAL EVALUATION FACTORS

This is a competitive procurement following RFO 6.2 Full and Open Competition After Exclusion of Sources, and is set aside for verified SDVOSB’s, using the Tradeoff Process identified in RFO 15.101-1. Award of project contract will be made based on evaluation factors, with the combined technical factors being significantly more important than the price. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, will be most advantageous to the Government, price and other factors considered. Offerors must provide all requested information for each factor. Proposals must separately address each element of each factor as applicable to be considered responsive to this RFP. Factor headings should be highlighted in yellow or typed in bold text for identification. It is the contractor’s responsibility to ensure the submitted proposal is in the proper format and that the provided information is clearly understood and free of ambiguities.

Offerors proposals should consist of, and will be evaluated based on five (5) factors:

technical approach, infection control plan, past performance, key project personnel, and price proposals, as described below.

FACTOR 1 – TECHNICAL APPROACH:

A. Submittal Requirements:

a. The government will evaluate the following items this factor: Methodology, Project Schedule, and Relevant Projects.

b. Methodology:

i. This factor will be used to assess the Offeror’s understanding of the scope of work required for the project as well as type of problems to be encountered. Provide a written construction plan (12pt font, no more than six pages but no less than one page in length)

ii. At a minimum, the construction plan must address/discuss the following:

1. Describe in detail how the offeror will complete the SPS

Equipment Site Prep Project (see iii below)

2. How work will be done for each phase.

3. Any potential challenges faced and the solutions.

4. How the offeror will minimize impact of construction to Sterile

Processing Procedures

5. How the offeror will coordinate with the equipment vendors listed in the project SOW

iii. Offerors are specifically instructed to provide clear, unambiguous, and detailed information. Confusing, vague, or conflicting representations of Technical Approach to demonstrate technical competence, including proposals that simply restate the published requirements, will not be rated as highly as proposals that provide clear and detailed information.

1. For each phase (1-5) what is the contractor’s plan for achieving the architectural, mechanical, electrical, structural, fire protection, plumbing, and technology portions of the construction phasing notes in the bid documents.

c. Project Schedule:

i. Provide a proposed schedule with starting and completion times of major elements of working beginning with the Notice To Proceed.

Offeror must submit a project schedule (no page limit, utilizing a standard project schedule software) that meets the 330 calendar-day time frame for project completion. For purposes of the proposal submittal, assume a Notice to Proceed (NTP) date of September 3, 2026.

ii. At the minimum, the project schedule must include the following:

1. Submittals

2. Identification of Long Lead Items

3. Work by Division

4. Phasing

5. Commissioning

d. Relevant Projects:

i. The offeror shall provide relevant example projects. Relevant projects shall not exceed two pages each. The offeror shall provide a minimum of two relevant example projects, with a maximum of three projects in total. If the Offeror provides more than three, the

Government will only evaluate the first three listed.

ii. Provide documentation on similar previous completed project.

These projects are to be similar in regards to any kind of construction/renovation in a sterile processing services department, construction/renovation in critical hospital areas (i.e. Intensive Care

Units, Operating Rooms, etc.), or equipment site prep projects.

iii. At a minimum this documentation should include:

1. Project Name & Number

2. Customer Name (Company)

3. Current point of contact including telephone and email information

4. Project Award Amount

5. Project amount at completion or at the existing time if unfinished (if the final amount is the same as the award amount, state so)

a. Unfinished example projects must be at a minimum of

75% completion (based on the construction schedule) to be considered for evaluation purposes.

6. Explanation of what the scope of those projects were along with how the work was performed

7. Relevance to this project (618-CSI-240 SPS Equipment Site

Prep)

B. Evaluation Approach:

a. The government will evaluate the extent to which the Offeror's

Methodology, Project Schedule, and Relevant Projects; includes all of the requirements above, demonstrates if the offeror understands the scope of this project, and shows how qualified the offeror is to perform construction for this project.

b. Additional consideration will be given for the following:

i. If the example project is a VA project (Private and Public sector example projects are allowed, but will be rated normally)

ii. If any of the three projects provided include indoor excavation.

FACTOR 2 – INFECTION CONTROL PLAN:

A. Submittal Requirements

a. The offeror shall provide a narrative (12pt font, no more than six pages but no less than one page in length) explaining what measures the firm will use to comply with the facility’s needed infection control standards per the provided VA ICRA 1.2 document.

b. The narrative shall include, but is not limited to:

i. Use of infection control barriers for each construction phase.

ii. Method of using Protective Personnel Equipment (PPE) as staff traverse through both the sterile and decontamination sides of the

Sterile Processing Department.

1. Note: PPE requirements for the sterile and decontamination sides of the Sterile Processing Department are not the same.

iii. Method to ventilate the space to keep the construction site under negative pressure for each phase of construction.

iv. Verification that at least one member of the key construction personnel has completion of a certified ICRA training/course (ex:

ICRA Best Practices in Health Care Construction provided by the

MN Carpenter’s Union Local 322) or equivalent.

1. See Factor 4 – Key Personnel Experience below (Section

B.b)

B. Evaluation Approach:

a. The Government will evaluate the extent to which the offeror’s Infection

Control Plan includes all the requirements above.

b. A narrative that simply restates the information given in the provided VA

ICRA 1.2 document will result in an unacceptable for this factor.

FACTOR 3 – PAST PERFORMANCE:

A. Submittal Requirements:

a. The offeror shall at a minimum provide a past performance evaluation for each of the projects listed under Factor 1 (Technical Approach). If the evaluation is available in CPARS, it shall be submitted with the proposal.

If the evaluation is not available in CPARS, then the Offeror shall submit a

Past Performance Questionnaire (PPQ) (reference Attachment H) for each project referenced for Past Performance. The Offeror shall notify their client that the PPQ shall be submitted via email to the Government’s point of contact, Dean.Flanders@va.gov. If the Offeror is unable to obtain a completed PPQ from a client for a project before proposal closing date, the Offeror shall complete and submit with the proposal the first page of the PPQ, which will provide contract and client information for the respective project. The Government may make reasonable attempts to contact the client noted for that project(s) to obtain the PPQ information.

Although the Government may make attempts, the Government is not responsible for ensuring that complete PPQ information is provided. The

Government will evaluate past performance even if complete information isn’t provided. Offerors should follow-up with clients/references to help ensure timely submittal of questionnaires. The page limit does not apply for the CPARS and PPQs

b. Note: For the factor 1 projects that are unfinished, please provide any interim CPARs.

B. Evaluation Approach

a. Past performance will be evaluated on the quality of the Offeror’s past performance with respect to the projects provided. The currency and relevance of the information, source of the information, context of the data, and general trends in contractor’s performance shall be considered.

When evaluating, quality aspects that may be considered are timeliness of contract completion, adherence to contract schedules and timely submission of data deliverables, the contractor’s ability to comply with the terms and conditions of the contract, the overall quality of the work performed on the contract, the Offeror’s managerial performance, and if the reference would enter a contract with the contractor again.

Contractors without relevant past performance or for whom past performance information is not available, will be given a neutral rating.

Past performance evaluations may also be conducted using information obtained from CPARS. Other sources may include, but are not limited to, inquiries of owner representatives, Federal Awardee Performance, and

Integrity Information System (FAPIIS), Electronic Subcontract Reporting

System (eSRS), and any other known sources not provided by the Offeror

FACTOR 4 - KEY PERSONNEL EXPERIENCE:

A. Submittal Requirements

a. The Offeror shall provide a resume for each of the following key construction personnel: Project Manager, Superintendent, Site Safety and

Health Officer (SSHO), and Quality Control Systems Manger as well as any known major subcontractors. For each resume, include company name; name of individual; relevant listing of construction experience;

qualifications such as specialized training, education, experience, etc. For any subcontractors identified, provide a resume for that company, identify any key personnel that will be assigned to the contract resulting from this solicitation, and the relevant construction experience of both the company and any key personnel that were identified

b. Resumes cannot exceed two pages for each member of the key construction personnel and major subcontractors.

c. Due to the complexity of this project, the SSHO, Superintendent, Project

Manager, and Quality Control Systems Manager CANNOT be the same person. None of these positions can be dual-hatted either.

B. Evaluation Approach

a. The Government will evaluate the extent to which the submitted resumes demonstrate key personnel with the capabilities and qualifications to provide the required planning, implementation, and completion of the project. Submission of the VETS-4212 reporting information can be done electronically (https://www.dol.gov/vets/vets4212). Award cannot be made unless the awardee has filed their VETS-4212 report. Therefore, all offerors are encouraged to file every year. The Contracting Officer may request a copy of the email confirmation of receipt notification prior to awarding the project.

SYSTEM FOR AWARD MANAGEMENT (SAM)

Federal Acquisition Regulations require that federal contractors register in the System for Award Management (SAM) database (https://sam.gov) and enter all mandatory information into the system. Offerors are required to be registered in SAM at the time an offer is submitted in order to comply with the annual representations and certifications requirements (see FAR 52.204-7).

REVIEW REQUIRED REGISTRATION WITH CONTRACTOR PERFORMANCE

ASSESSMENT REPORTING SYSTEM (CPARS)

As prescribed in Federal Acquisition Regulation (FAR) Part 42.1102(e), the Department of Veterans Affairs (VA) evaluates contractor past performance on all construction contracts that exceed $900,000 and shares those evaluations with other Federal https://www.dol.gov/vets/vets4212

Government contract specialists and procurement officials. The FAR requires the contractor be provided an opportunity to comment on past performance evaluations prior to each report closing. To fulfill this requirement, VA uses an online database, CPARS, which is maintained by the Naval Sea Logistics Center in Portsmouth, Virginia.

CPARS is available to all Federal agencies and is the system used to collect and retrieve performance assessment reports used in source selection determinations.

CPARS also includes access to the federal awardee performance and integrity information system (FAPIIS). FAPIIS is a web-enabled application accessed via CPARS for contractor responsibility determination information.

Each contractor whose contract award is estimated to exceed $900,000 is required to register within the CPARS database (https://cpars.gov) Help registering can be obtained by contacting Customer Support Desk @ DSN: 684-1690 or COMM: 207- 438-1690.

Registration should occur no later than thirty days after contract award and must be kept current should there be any change to the contractor’s registered representative.

For contracts with a period of one year or less, the contracting officer will perform a single evaluation when the contract is complete. For contracts exceeding one year, the contracting officer will evaluate the contractor’s performance annually. Interim reports will be filed each year until the last year of the contract, when the final report will be completed. The report shall be assigned in CPARS to the contractor’s designated representative for comment. The contractor’s representative will have thirty days to submit any comments and re-assign the report to the VA contracting officer.

Failure to have a current registration in the CPARS database, or to re-assign the report to the VA contracting officer within those thirty days, will result in the Government’s evaluation being placed on file in the database with a statement that the contractor failed to respond.

BACKGROUND INVESTIGATIONS AND SPECIAL AGREEMENT CHECKS

All contractor employees are subject to the same level of investigation as VA employees who have access to VA Sensitive Information. The level of background investigation is commensurate with the level of access needed to perform all work as identified in the solicitation documents. This requirement is applicable to all subcontractor personnel requiring the same access. As per VA Directive 0710, costs of background investigations will be borne by the contractor. Please be advised the awardee will need to coordinate with the VA Medical Center concerning badging requirements.

HHS/OIG EXCLUSIONS

To ensure the individuals providing services under the contract have not engaged in fraud or abuse regarding Sections 1128 and 1128A of the Social Security Act regarding federal health care programs, the contractor is required to check the Exclusions Database – U.S. Department of Health and Human Services (HHS), Office of Inspector General (OIG), List of Excluded Individuals/Entities on the website (https://exclusions.oig.hhs.gov) for each person providing services under this contract.

Further the Contractor is required to certify in its proposal that all persons listed in the contractor’s proposal have been compared against the OIG list and are NOT listed.

During the performance of this contract the contractor is prohibited from using any individual or business listed on the List of Excluded Individuals/Entities.
