(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the
Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor
Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the
Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. (a) This solicitation includes VAAR 852.219-10 VA Notice of Total Service-Disabled Veteran-

(b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. (a) Pursuant to 38 U.S.C. 8127(k)(2), the offeror certifies that—
(1) If awarded a contract (see FAR 2.101 definition), it will comply with the limitations on subcontracting requirement as provided in the solicitation and the resultant contract, as follows:

In the case of a contract for services (except construction), the contractor will not pay more than 50% of the amount paid by the government to it to firms that are not VIP-listed SDVOSBs as set forth in 852.219-10 or VOSBs as set forth in 852.219-11. GREASE TRAP PUMPING AND CLEANING SERVICES
MAINTENANCE AND SERVICE
STATEMENT OF WORK
BACKGROUND
Southern Arizona VA Health Care System (SAVAHCS) requires a qualified contractor to perform maintenance, service, pumping, hydro jetting service and cleaning of Grease Trap collection tank located outside on the east side of building 3 by Dietetics Services at the Tucson campus. Provide high pressure hydro jetting service, as well as removing debris from the two main sewage ejection lift stations. The SAVAHCS campus is located at 3601 South 6th Avenue
Tucson, Arizona 85723.
This contract will be for a Base Year and Four (4) Option Years.
The purpose of this contract is to secure materials and the qualified technical labor that is required for the successful maintenance and service to include monthly pumping and cleaning of existing Grease Trap Collection Tank at the SAVAHCS campus. Skim debris off the surface of the two-sewage ejection lift stations quarterly. Emergency repair services will also be provided for under this contract.
WORK TO BE PERFORMED UNDER THIS CONTRACT
Maintain the Grease Trap for Building 3 Dietetics, perform high pressure hydro jetting service to clear sewer pipes to the mains at B3 Dietetic & Canteen & Canteen Service. Clean sewage ejection lift stations (B40 & B80 area):
A. Contractor shall provide all labor, material, equipment, and supervision necessary to pump and clean the three-chamber grease collection tank at building 3 by the main kitchen. The contract will perform preventive maintenance inspection on equipment/systems as provided in the contract agreement. Written preventive maintenance (PM) procedures will be submitted on approval to the Maintenance and
Operation Section within the first 30 days of the contract. PMs will be accomplished in accordance with the approval of written procedures. The contract will provide monthly pumping and cleaning of the main kitchen three chamber grease collection underground tank.
B. Contractor shall furnish all labor, equipment, materials, tools, supervision, and all necessary items to perform high pressure hydro jetting service to clear sewer pipes and remove debris, sludge, and accumulation of grease in the Dietetic and Canteen grease lines on a quarterly schedule. Service to be completed in the first two weeks of
November, February, May, and August. Contractor shall coordinate jetting schedule with
COR, prior to beginning work.
C. Contractor to provide all labor, equipment (includes pump truck) and incidentals necessary to remove debris out of the main sewage ejection lift station located on the east side of building 40 on quarterly schedule. Approximately 1500 gallons of wastewater will need to be removed to adequately clean the sump. Services are to be performed between the hours of 5:00 AM and 7:00 AM. Services are to be completed in the first week of November and May months. Contractor shall coordinate pumping schedule with COR, prior to beginning work.
D. Contractor to provide all labor, equipment (includes pump truck) and incidentals necessary to remove debris out of the main sewage ejection lift station located between building 80 and 81 on the east side of building 80 on quarterly schedule.
Approximately 1500 gallons of wastewater will need to be removed to adequately clean the sump. Services are to be performed between the hours of 5:00 AM and 7:00 AM.
Services are to be completed in the first week of November and May months. Contractor shall coordinate pumping schedule with COR, prior to beginning work.
Contractor shall perform these commercial services IAW with all federal, state, county, local, and VA requirements.

2. Disposal: The contractor shall transport and dispose of all excavated, or otherwise removed liquid and/ or semi-solid, or solid waste materials fully in accordance with all applicable Federal Environmental Protection Agency (EPA), county, state, and local liquid and solids waste pollution abatement Statutes, Executive Orders, Resolutions, Ordinances, Codes, criteria, and requirements.
3. Possible Liquid 'Waste Exception: if the contractor uses equipment that filters the grease and other waste from the wastewater pumped from the grease traps, the filtered water may be returned to the grease trap. The solid waste may then be disposed of as stated in
Paragraph 3. The filtration process exception shall be allowed for equipment that is EPA approved only, and this exception shall not relieve the contractor of obligations to remove waste materials from the walls and floors of the grease traps.
4. Reporting procedures: All contractor personnel must report to the Engineering
Department, Plumbing and Mechanical office prior to performing any work and to sign out after completion of services. The contractor shall furnish a detailed field report, or equivalent showing work completed before leaving the facility. If contractor in on station prior to VA work shift, contractor shall report to Utilities Operations for temporary badges and leave field service report with plant operator when returning temporary badges.
5. Limitations: Contractor shall not replace any parts or perform any service that will result in additional charges without prior approval from the COR at the specific site.
6. Utilities: Water and electricity are available at the facility and may be utilized by the contractor at no cost in quantities necessary to perform the work herein. The contractor shall be responsible for all temporary connections necessary for operations and for removal prior to final acceptance of work.
7. OSHA, DOT, NFPA: The contractor shall comply with the standards in the Occupational
Safety and Health Act (OSHA) U.S. Department of Transportation (DOT) and National
Fire Protection Association (NFPA).
8. The contractor is responsible for conduct of employees while on VA grounds including any accidents while performing the service. Contractor is responsible for ensuring employees are wearing VA Contractor badges in a visible location while performing contracted duties on VA property.
9. Work Hours, Emergency Services and Federal Holidays:
Normal Work Hours: 6:00 AM through 3:30 PM mountain standard time Monday through
Friday.
Emergency Services: Contractor shall be available 24 hours 7 days a week; 365 days to respond to emergency maintenance as needed. Response time for emergency service for blocked or plugged grease lines shall be no more than two (2) hours after the emergency call is placed and communicated. The contractor must provide an emergency telephone number for purposes of accepting service calls 24 hours/ 7 days a week. The contractor must acknowledge receipt of notification by calling who COR within two (2) hours of the originating call. Contractor is responsible for clearing and removing blockage and restoring to full performance.
Federal Holidays:
The eleven holidays observed by the Federal Government are:

Columbus Day Thanksgiving
Christmas Day or any other day specifically declared by the President of the United States to be a federal holiday.
Contractor Failure to Perform: if the contractor fails to perform the required services within the specified limitations, the Government may:
• Accept the service at a later date.
• Refuse to accept the service at a later date, in which case the government will bear no cost and make no payment for missed service.
• Procure the missed service under terms and conditions deemed appropriate by the
Contracting officer from an alternative, responsible firm. In such case, the Contractor shall be liable to the Government for any excess cost incurred and to continue performance as required thereafter.
General requirements:
• Contractor's employees: Vehicle operations shall observe and abide by all SAVAHCS policies, signage, speed limits, and pedestrian rights-of-way, emergency vehicle right-of-way and designated roadways to collection sites.
• Interruption of work: Contractor's personnel shall be required to interrupt the performance of the services of this contract at any time to prevent the interference of operations of the Medical Center, to include the right-of-way passage for fire systems, emergency vehicles and utility services.
• Contractor shall be responsible to ensure Contractor employees providing work on this contract are fully trained, qualified, and completely competent to perform the required work.
• The C & A requirements do not apply, and that a Security Accreditation Package is not required.
• The Contractor shall not perform work that is outside the SOW unless directed in writing by the COR. Any work performed by the Contractor outside the SOW without authorization from the COR or Contracting Officer will be at the Contractor's own risk and at no cost to the SAVAHCS.
Potential vendors maybe required to make a site visit.

The C&A requirements do not apply. A Security Accreditation Package is not required.

The Offeror shall not complete the representation at paragraph (d)(1) of this provision if the
Offeror has represented that it ‘‘does not provide covered telecommunications equipment or services as a part of its offered products or services to the Government in the performance of any contract, subcontract, or other contractual instrument’’ in paragraph (c)(1) in the provision at
52.204–26, Covered Telecommunications Equipment or Services—Representation, or in paragraph (v)(2)(i) of the provision at 52.212–3, Offeror Representations and Certifications–
Commercial Products and Commercial Services.
