STATEMENT OF WORK (SOW)

Stryker 1788 upgrade

1. Point of Contact: TBD

2. Contract Title. (Provide a short, descriptive title of the work to be performed) New Stryker Video System 1788 upgrade

3. Background. Each Operating Room (OR) at the Boise VA is equipped with the 1488 Stryker video system. This system was no longer supported by Stryker due to its age. We use this video system for approximately 60% of our surgical procedures. We have had to use third party repair services to keep the system serviceable.

The OR lights in the each OR 1-4 are also in the same situation. They are older models and are no longer supported by the manufacturer. Parts for repair are no longer available for our biomed department.

The equipment booms in ORs 1 and 2 are also older models which were not updated in the last equipment update and need to be upgraded due to age and failure.

4. Scope. The plan has identified the hardware for the ORs as overhead surgical lights in all 4 ORs and Equipment Booms in ORs rooms 1 and 2. This equipment is mounted to the ceiling and will be placed on a capital purchase. All of the upgraded video equipment will be placed on a lease to alleviate a full purchase in the future and will make it much easier to upgrade with a new lease.

5. Specific Tasks.

Upgrade the current Operating room video equipment (Stryker 1488) with the newest 1788 model. The equipment listed below is to be leased. The remaining equipment for the system has been purchased by the Boise VA Medical Center. All equipment is to be installed together as the system must interconnect, and therefore installation must be coordinated with Stryker.

6. Performance Monitoring nonapplicable

7. Security Requirements.

No security issues all stored PPI will be maintained locally by VA staff.

8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). None

9. Other Pertinent Information or Special Considerations. None

a. Identification of Possible Follow-on Work. The Stryker system in place will have all the same connections and access which is managed by our local onsite IT department.

b. Identification of Potential Conflicts of Interest (COI). No conflict of interest noted.

e. Inspection and Acceptance Criteria.

We currently have a Stryker system in place, and this would be an upgrade of the current system.

10. Risk Control.

The installation of the system will take approximately one week, and the rooms will be terminally cleaned prior to any patient usage. No infection control issues will be present.

11. Place of Performance.

The work will be completed on the Boise VA Medical Center campus 500 W Fort Street, Building 85 3rd floor Boise, ID 83702

OR1 room – 316, OR2 room – 318, OR3 room – 360, OR4 room – 362

12. Period of Performance. The system is beginning to fail and the soonest time the upgrade could be completed would be the best for our veteran care. Installation is expected to take one week.
