STATEMENT OF WORK
El Paso Veterans Affairs Health Care System (ELPVAHCS) Cable Distribution Service Contract
DESCRIPTION OF SERVICES:
The Contractor shall provide voice and data cable installation and follow-on services for VA sites in the VISN 17 El Paso Veterans Affairs Health Care System. The Contractor shall furnish all personnel, equipment, tools, materials, transportation, management supervision, and other items and services necessary to perform all cable installation tasks and functions.
All work performed under this contract shall come in the form of service requests by the COR.
The Contractor shall provide a quote which will be evaluated and approved by the COR. The COR in turn shall forward the quote to the Contracting Officer for their subsequent evaluation and approval. Payment shall only be made on work previously quoted, approved by the VA and completed 100% by the Contractor, including any testing and inspections required. All work performed under each option year shall not surpass the threshold limit, which in this case will be $75,000.00 per year. In the event that the threshold is reached, and the COR determines that additional work is needed, a contract modification shall be requested to the CO to increase the threshold for that respective option year.
This proposal request herein describes the complete Copper Cable and Fiber Optic Cable Distribution System and will be referred to in this document, as the "Cable Distribution System" or “Fiber Distribution System”. The statement of work also includes furnishing, installing and maintenance of the complete horizontal copper station cabling and vertical copper riser cable to support voice and data end user equipment. The Cable Distribution System architecture shall be of the latest technology design for Category (5e, 6, 6A) horizontal station cabling. The Cable Distribution System and Fiber Distribution system shall meet or exceed the manufacturer's published commercial specifications and standards specified in this document. The Cable Distribution System and Fiber Distribution system will be maintained to satisfy the digital data and voice requirements of the Department of Veterans Affairs, VISN 17 El Paso Veterans Affairs Health Care System (EPVAHCS). Contractor will not team with another Vendor or Contractor in performance of this scope of work.

The EPVAHCS reserves the right to add or delete areas of service based on service demand.
Changes in service areas shall be addressed in a written bilateral modification to this contract issued by the Contracting Officer.
Any work to be performed in Leased buildings shall be fully coordinated with the COR. Access to each Leased building will be subject to Lessor’s approval. Work schedules in Leased buildings shall be subject to COR and Leased building staff’s approval.
CABLE PLANT MANAGEMENT RECORDS:
A Cable Distribution System record shall be provided by the Contractor to show all copper and fiber cables installed and tested in accordance with contract specification.
DEMOLITION OF EXISTING COPPER AND FIBER CABLES:
The Contractor shall be responsible for removing any abandoned copper and fiber cabling encountered while performing work. The Contractor shall also be responsible for letting the COR know whenever any non-complaint copper and fiber cabling is detected. Before removal of any abandoned cabling, the Contractor should coordinate with COR and local OIT department to ensure such cabling is no longer needed. All work performed under this section shall be preceded by a quote to remove such items, approved by COR, OIT and CO.
CABLING TYPES:
Cabling tasks may include the following types:
• Voice Grade Cabling
• Material acquisition, shipment, staging and installation of Category 6A (4 Pair) and Optical
Multimode 4 (OM4) and Multimode 3 (OM3) fiber cables. These cables will be pulled, terminated, and tested. They will be machine labeled.
• Material acquisition, shipment, staging and installation of Category 5e (4 Pair). These cables will be pulled, terminated, and tested. They will be machine labeled.
• Material acquisition, shipment, staging and installation of standard copper horizontal cable to include Category 6 (4 pair) cabling. These cables will be pulled, terminated, and tested.
They will be machine labeled.
• Material acquisition, shipment, staging and installation, and termination of voice grade cables. These cables will be pulled, terminated, and tested. They will be machine labeled.
• Data Cabling. Material acquisition, shipment, staging, installation, and test of copper and Single Mode (SM) or Multimode (MM) fiber cabling in accordance with industry standards.
The offeror shall procure, install and test all data cabling to include active equipment cabling to the buildings’ point of demarcation.
• Cable Television (CATV) and Close Circuit Television (CCTV) Cabling. Material acquisition, shipment, staging, installation, and termination of Category (5e, 6, 6A), and Radio Grade-6 (RG-6) cables. These cables will be pulled from and to specified locations in the cable run sheets. All cables will be machine labeled and tested in accordance with specified industry standards. Agency site personnel will designate specific locations within the Data Closets to ensure sufficient cable length to terminate to service providers feed at time of installation.
• Cable tray cable support devices and systems.
• Racks and cabinets for new hardware .
• New switches as determined by COR and local OIT service
• Power requirements – Dedicated circuits.
• Grounding and bonding.
• Any type of connectors, wall plates, jumpers, required to terminate.
• Backboards.
• Inside Plant (ISP) riser and horizontal pathways
• Satellite Cabling. Material acquisition, shipment, staging and installation of LMR-400FR and Belden 83709 (or equivalent) cable. These cables will be pulled from site designated rack location in communications server room via conduit to closet in annex. The offeror shall provide LMR-400DB cable and perform all cable terminations and testing.
• General Cabling. Material acquisition, shipment, staging and installation of general cables. These cables will be pulled to and from specified locations only. They will be machine labeled and left for termination by others.
CABLE DISTRIBUTION SYSTEM; COPPER (TWISTED PAIR):
The Contractor shall provide a new Cable Distribution System conforming to Building Industry Consulting Service International (BICSI) Standards, EIA/TIA 568A & 569 and ICEA Publications S-80-576-1988 (Ref. B1.6) as to size, color code, and insulation. All cable distribution installation shall be fully coordinated with the VA’s Contracting Officers Technical Representative (COR) prior to start of installation.
The Contractor is responsible for complete knowledge of the space and cable pathways (equipment rooms, telephone closets, conduits, wireways, etc.) of the locations listed above. The Contractor shall design new pathway (cable tray, J-Hooks, etc.) if required to complete work and if provided, install the Cable Distribution System using the pathways (conduit, etc.) provided in the area of the VA Medical Center (VAMC). Cable trays currently in use that are not full will be utilized for new cable. Cables shall be supported with J-hooks at randomized distances between four (4) to five (5) feet whenever there is an absence of a cable tray. All cables installed in pathways and spaces will conform to the ANSI/TIA/EIA-569-A standard. Firewall penetrations will be filled using fire stopping material, type approved by VAMC Chief, Engineering. Which is 3M Fire barrier CP 25WB+Caulk and 3M Fire Barrier Moldable Putty+. Installer shall be a 3M certified installer. The VA, upon written request, will authorize the Contractor to provide a quote to increase number or size of pathways only in the event the total pathways provided to any given area are inadequate.
Voice cable and shall conform to the requirements of ICEA Publications S-80-576-1988 (Ref.
B1.6) as to size, color code, and insulation.
All material, installed by the Contractor, shall be new and thoroughly tested. Any station cables testing with bad pairs will be reinstalled.
Conductors shall be cabled to insure against induction in voice/data circuits. Crosstalk attenuation within the cable system shall be in excess of 80dB throughout the frequency range (UL, ISO 2000).
The Cable Distribution System shall be labeled in accordance with ANSI/TIA/EIA 606;
Administration Standard for the Telecommunications Infrastructure of Commercial Buildings dated 2/1993, or the existing labeling scheme used at the site. The cable distribution will also be fully tested. All cables will be tested with a TIA Level III or better tester. The cable records shall identify each cable as labeled, used cable pairs, and bad cable pairs. All cables including connectors will be tested to ensure proper installation practices were observed and the installations meet TIA/EIA TSB-36, Additional Cable Specifications for Unshielded Twisted Pair Cables, and the TIA/EIA TSB-40 Additional Transmission Specifications for Unshielded Twisted Pair Connecting Hardware. These measurements shall be entered as part of the cable records.
All cable records will be maintained in the Telephone Switch Room and all future changes (used pair, failed pair, etc.) shall be posted in these records as change occurs. Any station cables testing with bad pairs will be reinstalled. All cables, including connectors, shall be tested to ensure that proper installation practices were observed, and that the installation meets the requirements of EIA/TIA. These measurements shall be entered as part of the cable records. All cable records must be available at acceptance testing and thereafter maintained in the Telephone Switch Room.
If temporary cable and wire pairs are used, they shall be installed so as to present no pedestrian safety hazard and the Contractor shall be responsible for all work associated with this temporary installation and for removal when no longer necessary.
The Contractor shall ground all metallic cable sheaths, etc. (e.g., risers, underground, station wiring).
Conductors shall be cabled to ensure against induction in voice/data circuits. Cross talk attenuation within the cable system shall be in excess of 80dB throughout the frequency range.
All outside cable shall be shielded, 24AWG solid conductors, solid PIC insulation, and filled core (flex gel) (waterproof) REA LISTED PE 39 or PE 89 Code. The outside cable plant cable shall be grounded with 6-gauge green ground cable. Access to the manholes will be coordinated with the VAMC (Quality Management-Safety). This will allow time to air and test the manhole environment to ensure a safe entry.
All Telecommunications riser cables shall be 24AWG solid, thermoplastic insulated conductors and will run from the facility demarcation point or MDF/IDF to the termination point or IDF. Riser cable will be terminated on 110 type terminals on both ends.
The Contractor shall engineer and install riser cable to provide full service to all voice cable pairs identified in each telecommunication closet/cabinet. A minimum of two pair per telephone shall be provided and cross connected from the telecommunication closet to the Main Distribution Frame (MDF), unless the site requirements differ and, in that case, the existing site requirement will be used.
All cabling shall meet the EIA/TIA-568 Commercial Building Wiring Standards including the TSB- 40 update.
TELECOMMUNICATIONS OUTLET CONNECTORS AND WORK AREA CABLE
TERMINATIONS:
Cable installations will be installed as one of the following to be determined by the COR at time of installation: A new ‘Cable Drop’ consists of two Category (5e, 6, 6A) blue cables for data and one Category (5e, 6, 6A) white cable for voice. A new ‘Voice Drop’ consists of one Category (5e, 6, 6A) white cable for voice. A new ‘Data Drop’ consists of two Category (5e, 6, 6A) blue cables for data.
All new voice and data telecommunications outlets (Jacks) shall be Category (5e, 6, 6A) compliant eight position RJ-45 non-keyed (EIA/TIA 568A). Four unshielded twisted pair 24 AWG Work Area (WA) wires shall be installed from each jack (in accordance with EIA/TIA standards 568 "T568A" and 606) to the telecommunication room (IDF) and shall be of a type designed to support Category (5e, 6, 6A) data communications (not less than 100 MHz /100 Mbps). Data cables shall be terminated on Category (5e, 6, 6A) RJ-45, 8 wire 24-port / 48-port patch panels, Voice cables shall be terminated on Category (5e, 6, 6A) compliant 110 type modular blocks, which are dedicated to voice applications.
All new outlets shall have a minimum of two (2) data cables and one (1) voice cable at each faceplate unless otherwise specified by the COR. The faceplate shall be ivory in color and capable of handling a minimum four (4) connectors, including ST fiber optic and FDDI connectors. The data connectors shall be blue (top left) and orange (top right) as mounted in the faceplate. Voice connectors shall be white with one connector terminated with the white/blue & white/orange pairs (bottom left) and one connector terminated with the white/green & white/brown pairs (bottom right) as mounted in the faceplate. All connectors shall meet the TSB40 Category (5e, 6, 6A) standard.
Once terminated, the cable sheath is clamped which provides for strain relief. After termination, wire colors should be visible for verification of correct installation. The front of the connectors shall be flush with the faceplate installation. Any space on the faceplate not containing a connector will be filled with blank. A modular furniture insert shall be available that uses the same connector being used in faceplates. A surface mount box using same connectors is required.
At the telecommunication room (IDF) the cable for the voice portion of each connector shall be terminated on Category (5e, 6, 6A) compliant 110 type modular blocks, which are dedicated to voice applications. Wire Management shall be provided by the Contractor for distribution management. Labeling and color-coding shall be in compliance with EIA/TIA 606.
At the telecommunication room (IDF), the cable for the data portion of each connector shall be terminated on appropriately sized Category (5e, 6, 6A) compliant RJ-45, 8 wire 24-port / 48-port patch panels. Cable Management shall be provided by the Contractor for field distribution/patch cord management. Labeling and color-coding shall be in compliance with EIA/TIA 606 and or the individual sites requirements.
Each wall telephone instrument shall be installed on a 630A4 type stainless steel wall jack assembly in accordance with ADA standards. At the telecommunication room, all four pair shall be terminated on Category (5e, 6, 6A) compliant 110 type modular blocks, which are dedicated to voice applications.
In areas where there are no existing telephone outlets installed and the new installation point is on a hollow wall, the Contractor shall provide and install a flush mounted box with the appropriate type of drop. For surface mounted installations, the Contractor will provide wire mold and outlet box. Wherever possible cable molding is to be screwed into the wall. Modular free-standing cubicle designed furniture will be wired by the Contractor.
The Contractor shall provide the appropriate modular connector and cover plate for each location identified on station wiring prints.
Where Patient Bedside Private Units (PBPUs) exist which contains a single gang box to be used for telephone and data jack installation, the Contractor will be responsible for the necessary disassembly and reassembly of each PBPU to the extent necessary to pull wire from above the ceiling to the box reserved for telephone and data. A cover provided by the vendor and approved for use by the VAMC shall be used to finish the jack installation.
The Contractor shall provide the appropriate modular jack and cover plate. Blank cover plates will be utilized to cover any holes in the walls where the pathways were not utilized.
All cable terminations shall meet the EIA/TIA-568 Commercial Building Wiring Standards including the TSB-40 update.
The Contractor is advised that some installations may take place to accommodate personnel working in modular furnishings. The VA will set up a meeting between the cabling Contractor, the furniture Contractor and the VA.
MAIN DISTRIBUTION FRAME (MDF) and INTERMEDIATE DISTRIBUTION FRAME (IDF):
Gas tube protection devices, or equivalent, shall be provided on all circuits and cable pairs serving building intermediate distribution frames (IDF’s) located in buildings other than the building in which the EPABX is located or in any area served by an exposed distribution system (conduit, aerial, etc.). The Contractor shall install gas protection devices at the nearest point of entrance in the buildings where the protection is required and on the same circuits on the MDF in the telephone switch room.
In case of buildings which are either directly attached or connected by enclosed walkways, gas tube protection at the IDF will not be required as long as the cable pathways provided do not exit the connected buildings at any point. Gas tube protection is required on all cable pairs, which interconnect separate buildings.
All MDF’s and IDF’s shall be wired in accordance with BICSI, and other applicable standards. All cable will be tested using a device that will measure wire mapping, return loss, DC loop resistance, Attenuation, Near End crosstalk, length, etc.; ensure that all cables installed meet the EIA/TIA specs for all UTP cables using the criteria listed above. The results of the testing will be provided to the COR.
Any problem with the cable system must be isolated and repaired by the Contractor. The Contractor will trouble shoot the riser cable, station cable, jack (insert), splice, etc. to determine the best method of repair. If problems are found, the Contractor shall contact the site telecommunications manager and continue corrective measures until the problem is resolved.
CABLE DISTRIBUTION SYSTEM; FIBER (FIBER OPTICS):
The Contractor shall install fiber optic cable to meet Fiber Distributed Data Interface (FDDI) bandwidth requirements and multi-mode wavelength of 850/1300 Nanometers. The fiber optic cable shall be OS2, OM3 & OM4. The fiber optic cable shall contain a minimum of 12 strands unless otherwise specified and shall not exceed 2000 meters (6560 feet) in a single run. All fiber optic cable will be installed in Contractor provided "inner duct". Loose tube cable, which decouples the individual fibers from the environment, and shall be installed for all out door runs or for any area which includes outdoor run. Tight-buffered fiber cable shall be used for indoor runs. If any underground conduit becomes at least 75% filled, a spare inner-duct, with pull string, shall be installed.
In the computer room all fiber optic cable shall be installed in a rack mounted fiber optic cable distribution component /splice case in accordance with BICSI standards. "LC and SC" connectors will be installed on each strand.
Fiber optic cables shall terminate in IDF’s at a wall or rack mounted interconnect panel.
All strands will be tested and cable transmission performance specifications shall be in accordance with EIA/TIA standards. Attenuation shall be measured in accordance with EIA fiber optic test procedures EIA/TIA-455-46-61, or -53. Information transmission capacity shall be measured in accordance with EIA/TIA 455-51, or -30.The Contractor will provide written test results.
CONTRACTOR’S PERSONNEL:
Contractor will assign a Project Manager (PM) in writing, who will be the key point of contact for VA when communication with Contractor is necessary throughout the performance of this contract. All proposed substitutions are to be submitted to the Contracting Officer in writing, at least 30 calendar days in advance of the proposed effective date. Contractor will not team with another Vendor or Contractor in performance of this scope of work. Work will be accomplished Monday through Friday 8:00 a.m. to 4:30 p.m. excluding Federal holidays. Work performed outside these hours must be approved and coordinated with COR. Contracting personnel working on-site at any VA location shall adhere to the rules and regulations in place at the specific VA location.
Contractor's employees are required to wear VA’s visitor’s tag or badge while on the premises of the VA. Police Services, 3rd floor, Room A308 will provide Contractor's employee with a visitor’s badge. It is the responsibility of the Contractor to park in the appropriate designated parking areas.
Information on parking is available from the VA Police Section. Parking at the South parking lot is prohibited since this is a patient reserved area. The VA will not invalidate or make reimbursement for parking violations of the Contractor or Contractors' employees under any conditions. The COR will not be responsible for monitoring employee attendance on the job. The VA’s Safety Service shall brief Contractor's employees in emergency procedures, such as Fire Safety. Possession of weapons is prohibited. Enclosed containers, including personnel items, shall be subject to search.
Violations of VA regulations may result in citation answerable in the United Sates (Federal) District Court, not a local district, state, or municipal court.
NO SMOKING POLICY:
All Contractor personnel are subject to and must comply with all Federal, State, and Local Policies, Rule and Regulations. As an example, the South Texas Veterans Health Care System (STVHCS) is a non-smoking facility except in designated areas. Executive Order No. 13058 Protecting Federal Employees and the Public From Exposure To Tobacco Smoke in the Federal Workplace Applies. STRICT ADHERENCE TO FEDERAL, STATE, AND LOCAL POLICIES, RULES, AND REGULATIONS WILL BE ENFORCED. VIOLATORS MAY BE SUBJECT TO
FINES.
If work is to be done outside of the Department of the Veterans Affairs, i.e., Lease Building, smoking is strictly prohibited in the building and on the rooftop. The Contracting Officer may remove Contractors not adhering to these rules.
The Contractor shall at all times keep the work area, including storage areas, free from accumulations of waste materials. Before completing the work, the Contractor shall remove from the work and premises any rubbish, tools, scaffolding, equipment, and materials that are not the property of the Government. Upon completing the work, the Contractor shall leave the work area in a clean, neat, and orderly condition satisfactory to the COR.
Contractor shall obtain any and all required licenses required to perform the required work.
Contractor shall comply with all local building and fire codes. Where cable and wire penetrate through fire/smoke partitions, firewalls, or floors the Contractor shall provide and install fire stopping material, type approved by VAMC Chief, Engineering. Which is 3M Fire barrier CP 25WB+Caulk and 3M Fire Barrier Moldable Putty+. Installer shall be a 3M certified installer.
Contractor shall be responsible for the removal and replacement of ceiling tiles during installation of the Cable Distribution System. The Contractor shall be responsible for replacement and installation of any ceiling tiles they damage with a matching tile.
The Contractor shall comply with the fire safety code of the facility(s) as follows: the following must take place. When a cable pull is taking place in a corridor and the installer can’t see the other end of the installation. A second installer must be present under that open ceiling tile.
The Contractor shall not use gasoline, benzene, alcohol, naphtha, carbon tetrachloride, or turpentine for cleaning any part of the equipment. Flammable materials shall be kept in suitable places outside the building as determined by the COR. OSHA safety standards and local VAMC safety standards shall prevail.
The Contractor shall be responsible for requesting access to all communication closets in order to accomplish the work. The telecommunication closets will be secured at all times. The PABX switch room access rules of behavior will be signed by all those that have a must need for access to the telephone switch rooms at our various locations.
CONTRACTOR RESPONSIBILITIES:
The Contractor shall be responsible for any and all liability actions or suits when the Contractor is at fault or negligent in the performance of maintenance activities. Pecuniary liability is also included for any and all damages to VA buildings and equipment caused by telephone system maintenance activities. The Contractor shall comply with all local building and fire codes in addition to VA policy and procedures at the site.
The Contractor shall be responsible for protecting the personnel furnishing services under this contract.
The parties agree that such personnel shall not be considered VA employees for any purpose and shall be considered employees of the Contractor.
Contractor is responsible for conducting a site visit to inspect the site where the work will be performed to evaluate, identify equipment and cable needs. Site visits may be arranged during normal duty hours by contacting the COR.
QUALITY CONTROL:
Contractor shall develop and maintain a quality control program to ensure services are performed in accordance with commonly accepted commercial practices. Develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The Contractor will provide a named individual to serve as the POC for quality control issues/complaints.
The Government and the Contractor shall jointly cooperate in the development and implementation of a Quality Assurance Plan. The QAP shall document the process that will verify and validate quality assurance in compliance with the contract requirements and ensure these requirements meet the Government’s expectations. This QAP shall consist of a Government approved QA process, periodic quality briefings within the Interim Baseline Review (IBR) process, and Government surveillance.
The Contractor shall develop a QAP that provides for the monitoring of the quality of the services, work products, and deliverables required under the contract. The Contractor shall develop the QAP based on accepted industry standards such as the CMMI SM, ISO 9000 IEEE 12207, or other accepted industry QA standards. The QAP shall detail the processes, procedures, and metrics for assuring quality. The QAP shall also include, at a minimum, a description of the following key activities:
• Establishment of capable processes;
• Monitoring and control of critical processes and product variation.
• Establishment of mechanisms for feedback of field performance.
• Implementation of an effective root cause analysis and corrective action system; and
• Continuous process improvement.
A draft QAP shall be submitted with the proposal for Government review. If the contract DO is awarded, the final QAP shall be due fifteen (15) calendar days after the PoP start date. Once approved, the QAP becomes final and unless modified, governs all QA procedures for the balance of the contract performance.
During performance of the contract, the Contractor shall maintain a QA inspection system that contains procedures and processes that are consistent with the monitoring and control processes identified in the approved QAP.
The final, approved QAP shall provide for the Contractor to conduct periodic briefings to the Government. The QA briefings shall address any and all quality issues uncovered by the Contractor since the previous briefing. Prior to the briefing the Contractor shall document the root cause of the problem and provide a description of the corrective action taken (or being taken).
The corrective action shall be consistent with the Contractor’s continued process improvement and field feedback mechanisms identified in the approved QAP.
The Contractor shall maintain records documenting all corrective actions and process improvements undertaken during contract performance. The records shall validate the Contractor’s compliance with the processes contained in the QAP. The Contractor shall make the records available to the Government during contract performance and for a period of five (5) years after final payment under the contract.
QUALITY ASSURANCE:
To be determined by the QAP decided by the Contractor and the government.
INSURANCE:
Contractor shall provide evidence of insurance prior to commencing work on the government installation. Submit to the attention of the contracting officer:
PAYMENT:
Services shall be paid only for work performed and completed. Government approval of invoices may be withheld, or payment reduced, until all due deliverables have been received.
NONPERSONAL SERVICES:
A nonpersonal services contract is defined in FAR 37 as a contract under which the personnel rendering services are not subject; either by the contract’s terms or by the manner of its administration, to the supervision and control usually prevailing in relationships between the Government and its employees.
The Contractor will perform independently and not as an agent of the government. The ordering activity may evaluate the quality of professional and administrative services provided but retains no control over professional aspects of the services rendered.
COMPUTER SECURITY REQUIREMENT:
“Contract personnel performing work under this contract shall satisfy all requirements for appropriate security eligibility in dealing with access to sensitive information and information systems belonging to or being used on behalf of the Department of Veterans Affairs. The cost of the background investigations is borne by the Contractor. Currently, the VA will pay for the investigations and the Contractor will then reimburse the VA. To satisfy the requirements of the Department of Veterans Affairs a Minimum Background Investigation shall be conducted prior to performing work under this contract. The level of access and the individual’s capability to perform work under this contract will be the determining factor in deciding if a higher investigative requirement is needed. The Contractor shall ensure that those requirements are fully satisfied within 30 days of initiation of such investigations. The investigative history for contract personnel working under this contract must be maintained in the databases of either the Office of Personnel Management (OPM) or the Defense Industrial Security Clearance Organization (DISCO). Should the Contractor use a vendor other than OPM or Defense Security Service (DSS) to conduct investigations, the investigative company must be certified by OPM/DSS to conduct Contractor investigations.”
ACCREDITATION:
The C&A requirements do not apply and a Security Accreditation Package is not required.
INFORMATION SYSTEMS OFFICER/INFORMATION PROTECTION:
The Contractor will not have access to VA Desktop computers nor will they have access to online resources belonging to the government while conducting services.
PRIVACY OFFICER:
The Contractor will/will not have access to Patient Health Information (PHI) nor will they have the capability of accessing patient information during the services provided to the VA.
PERSONAL SCREENING:
The Contractor staff and Subcontractors must complete background screening before access is permitted to VA systems.
TRAINING:
Contractor personnel performing the same duties as employees shall be required to attend the same security training.
RECORDS:
“Contractor personnel who obtain access to hardware or media which may manipulate or store any sensitive information protected under 38 USC 4132 or 3305, as defined by the Department of Veterans Affairs, must not access information unless absolutely necessary to perform their contractual duties. Disclosure of any sensitive data obtained during the performance of the particular contractual duty is prohibited. Violation of these statutory provisions may involve imposition of criminal penalties.”
SYSTEM SECURITY:
“The Contractor shall provide VA with full assurance that security measures have been implemented which are consistent with all CHA, VA, and other federal standards and guidelines.”
PROCEDURES FOR USER ACCESS:
“Access requirements to VA information systems by Contractors and Contractor personnel shall meet or exceed those requirements established for VA employees as described in VA’s AIS Security Directive and the accompanying AIS Security Handbooks.”
FINAL ACCEPTANCE:
The VA and the Contractor shall jointly conduct a Final Acceptance Test. This test shall be conducted 5 days following notification by the Contractor of completion of any addition or new Cable Distribution System called for in this document. In addition to compliance with the technical characteristics and quantities of equipment specified in this document, a provision is added that all punch list items noted during the initial inspection, when the Contractor notifies the VA of completion of the installation, must be resolved before the conclusion of the Final Acceptance Test. Upon completion of the punch-list item(s), the Contractor will be deemed to be in compliance with this statement of work.
At the conclusion of the Acceptance Test, the VA and the Contractor shall jointly agree to the results of the test, and reschedule testing on deficiencies and shortages, if any. When the test shows the system performs in accordance with the VA statement of work, and all punch-list items have been completed, the Cable Distribution System will be considered to have achieved compliance with the requirements of the statement of work. If any retesting is needed to reach agreement on the results of the tests or to establish compliance with this statement of work, such retesting will be done at the Contractor's expense.

Permit for Interstitial Entry
• All general Contractors and sub-Contractors must obtain a permit for daily entry into interstitial space from Safety Department. Contractors are required to obtain a permit for interstitial entry for installation projects only. A permit for daily interstitial hospital maintenance, investigations, inspections or repair will note be required. Interstitial doors must be closed at all times (including fire wall hatch doors) and absolutely no food, drinks, or smoking will be permitted.
• Before entering interstitial space, all personnel must complete Safety training. Fall training will be conduction by the EPVAHCS Safety Department.
• Contractors and sub-Contractor will not route wire or cable through wire chases or fire smoke walls without written authorization from the COR. The COR will record location of all penetrations of wire chases, smoke partition and fire wall. COR will ensure penetrations are patched with a NFPA approved fire rated application. Contractor’s personnel must notify COR of wire routing, smoke partition, fire wall, and chase penetration placements.

NARA Records Management Language for Contracts
1. Contractor shall comply with all applicable records management laws and regulations, as well as National Archives and Records Administration (NARA) records policies, including but not limited to the Federal Records Act (44 U.S.C. chs. 21, 29, 31, 33), NARA regulations at 36 CFR Chapter XII Subchapter B, and those policies associated with the safeguarding of records covered by the Privacy Act of 1974 (5 U.S.C. 552a). These policies include the preservation of all records, regardless of form or characteristics, mode of transmission, or state of completion.
2. In accordance with 36 CFR 1222.32, all data created for Government use and delivered to, or falling under the legal control of, the Government are Federal records subject to the provisions of 44 U.S.C. chapters 21, 29, 31, and 33, the Freedom of Information Act (FOIA) (5 U.S.C. 552), as amended, and the Privacy Act of 1974 (5 U.S.C. 552a), as amended and must be managed and scheduled for disposition only as permitted by statute or regulation.
3. In accordance with 36 CFR 1222.32, Contractor shall maintain all records created for Government use or created in the course of performing the contract and/or delivered to, or under the legal control of the Government and must be managed in accordance with Federal law.
Electronic records and associated metadata must be accompanied by sufficient technical documentation to permit understanding and use of the records and data.
4. El Paso Veteran Affairs Health Care System (EPVAHCS) and its Contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Records may not be removed from the legal custody of EPVAHCS or destroyed except for in accordance with the provisions of the agency records schedules and with the written concurrence of the Head of the Contracting Activity. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701.
In the event of any unlawful or accidental removal, defacing, alteration, or destruction of records, Contractor must report to EPVAHCS. The agency must report promptly to NARA in accordance with 36 CFR 1230.
5. The Contractor shall immediately notify the appropriate Contracting Officer upon discovery of any inadvertent or unauthorized disclosures of information, data, documentary materials, records or equipment. Disclosure of non-public information is limited to authorized personnel with a need-to-know as described in the [contract vehicle]. The Contractor shall ensure that the appropriate personnel, administrative, technical, and physical safeguards are established to ensure the security and confidentiality of this information, data, documentary material, records and/or equipment is properly protected. The Contractor shall not remove material from Government facilities or systems, or facilities or systems operated or maintained on the Government’s behalf, without the express written permission of the Head of the Contracting Activity. When information, data, documentary material, records and/or equipment is no longer required, it shall be returned to EPVAHCS control or the Contractor must hold it until otherwise directed. Items returned to the Government shall be hand carried, mailed, emailed, or securely electronically transmitted to the Contracting Officer or address prescribed in the [contract vehicle]. Destruction of records is EXPRESSLY PROHIBITED unless in accordance with Paragraph (4).
6. The Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-Contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, contracts.
