This solicitation is issued as unrestricted. The vendor must be registered for NAICS 811219 in
SAM to be eligible for award.

Late quotes or documentation will not be accepted. All questions, comments, or inquiries are to be submitted in writing to the Point of Contact, referenced above, prior to the closing of the solicitation.
I. Basis of Award
The Government intends to award a contract Brand Name or Equal resulting from this solicitation to the responsible offeror whose quote conforming to the solicitation meets or exceeds the requirement at the lowest price, Lowest Price Technically Acceptable (LPTA).
The quotes will be evaluated based on the evaluation factors listed below. The technical acceptability will be evaluated on an "acceptable" or "unacceptable" basis. The lowest price contractor will first be evaluated. If determined to be technically “unacceptable,” the next lowest price response will be evaluated, and so on, until an offer is determined to be “acceptable.” The following areas will be evaluated:

Factor 1: Technical Capability – The contractor shall provide a written statement addressing each of the following items. The written statement must describe each point and must provide enough detail to determine minimum qualifications:
a. Provide a written plan detailing your firm’s experience and capability of performing work similar to what is being required in the Statement of Work. No more than three (3) full pages.
b. Letter of Authorization: A letter of authorization is required from the Original Equipment
Manufacturer (OEM) stating that contractor is an authorized to maintain equipment had has been trained by the OEM for scheduled maintenance services and repairs.
Factor 2: Price – In this factor, price will be evaluated for reasonableness, completeness, and fairness by comparing the proposed prices with the Independent Government Estimate and prices posed by other offerors. This evaluation will not obligate the Government to exercise any option.
The base year price, each option year price, and total price will be evaluated.
B.3 STATEMENT OF WORK
STATEMENT OF WORK
DESCRIPTION OF WORK:
The North Texas Veterans Health Care System requires a contractor to provide a full-service contract for Aladdin food service carts located at the Dallas and Bonham locations. The contractor shall provide all labor, parts, to include carts and transport carts with expertise, travel, supervision, and other applicable items throughout the life of the contract in order to fulfill this requirement.
REQUIREMENTS:
• Contractor shall supply all labor, expertise, parts, and preventative maintenance services.
• Contractor shall warrant all services and repair work.
• Contractor shall be responsible and incur all freights costs incurred for the shipment of replacement parts.
• Contractor shall be responsible for employing technically qualified personnel to perform the work specified in this Statement of Work.
• Contractor shall maintain the personnel, organization and administrative control necessary to ensure that the work delivered meets the contract specifications and requirements.
• Contractor shall maintain a record of all maintenance and repairs.
• Contractor shall carry out the training of customers in use of scheduled equipment
ADDITIONAL REQUIREMENTS:
• Contractor shall provide scheduled Periodic Maintenance and Extended Services.
• Contractor shall provide the services of a fully trained factory representative for the purpose of performing detailed periodic maintenance, calibration and service of all Temp-Rite rethermalization equipment.
• Contractor shall ensure that equipment will be inspected semi-annually for all visual and apparent problems, tested totally, and/or calibrated in accordance with Aladdin Recommended Frequency Schedules.
• Contractor shall ensure that Periodic Maintenance Services will be performed semi-annually with up to two (2) days on each of these scheduled calls.
• Contractor shall respond to unscheduled service calls to include urgent repairs, maintenance, or emergencies within a 24-hour period with communication on when a technician will be on site for repairs.
• Contractor shall provide in writing, what services were performed for repairs, any parts that may need to be ordered for completion of the repair, and a timeline for completion of the repair.
• Contractor shall provide notification when a technician will arrive on site for repairs, regular maintenance, and periodic maintenance. If the Contractor requires entrance to either site before and/or after hours for unscheduled repairs and maintenance, notification/communication to the Contractor Officer Representative (COR) and Point of Contact(s), will be required from the Contractor prior to coming on site.
Emergency repair calls that may be required to be made if any equipment becomes inoperable impacting patient care needs, shall be responded to via phone call to the Point of Contact listed for that facility immediately upon receipt of notification. As part of the communication the Contractor shall notify the facility that a technician is in route to be on site for repair needs. A detailed description of what the issue is, repairs performed, and any pending resolution, i.e., further work needed, parts to be ordered, etc.
PREVENTIVE MAINTENANCE:
• Contractor shall ensure that maintenance on the Convect-Rit equipment is performed on a regular basis to keep each unit operating properly.
• Contractor shall ensure that all maintenance and services are carried out in accordance with the OEM.
• Contractor shall on a monthly basis check and adjust the Docking Station micro limit switches as required.
• Contractor shall on a monthly basis check and tighten all adjustment bolts both at the wall and on the floor.
• Contractor shall biannually remove the black plastic top cover of the Docking Station.
Contractor shall vacuum and clean the condensing unit coil to remove dust and dirt.
• Contractor shall abide by all instructions for cleaning and maintenance services outlined in the Original Equipment Manufacturer (OEM) Installation and Service Manuel.

PARTS:
Contractor shall furnish and replace all parts to restore the equipment to a serviceable condition at no additional cost to the government. This includes, but is not limited to:
labor, materials, parts, test equipment, tools, facilities, shipping and incidentals. All provided parts will be OEM parts or parts manufactured to OEM specifications. Parts shall be new and compatible with existing equipment. All replaced parts shall become property of the contractor for them to dispose or recycle as they see fit. The contractor will provide a written description of all the work to per performed on each equipment item. All part replacement will be accomplished during regular business hours. If the Contractor requires entrance to either site before and/or after regular business hours of operation for repairs, maintenance, notification/communication will be required from the Contractor prior to coming on site.

GOVERNMENT BADGES:
The Contracting Officer Representative (COR) or VAMC Representative will coordinate the issuance of required access badges to Contractor personnel. Contractor personnel shall wear laminated identification badges provided by the Government always when performing. Badges shall be worn on the outer garment, attached to the outer shirt or jacket pocket by a button or clip or worn around the neck secured by an appropriate identification card lanyard.
Government issued identification badge remains the property of the Government and shall be returned to the issuing office or other designated Government personnel upon transfer of Contractor employees from the contract.
SAFETY/SECURITY REQUIREMENTS:
The Contractor shall comply with all applicable federal, state and local laws and ensure performance is secure while protecting material, equipment, and actor supplies from damage and loss. Government security personnel shall have the express right to inspect for security violations at any time during the term of the contract. All information generated in performance of this contract becomes property of the Government and must be surrendered upon request and/or completion of the contract. The Contractor must protect against the destruction and/or misplacement of all information produced under this contract, unless directed under local policy to destroy. This includes both hard and soft copies of data.
The Contractor shall not release any information (including photographs, files, public announcements, statements, denials, or confirmations) on any part of the subject matter of this contract or any phase of any program hereunder without the prior written approval of the
COR.
CONDUCT OF PERSONNEL:
Contractor personnel shall conduct themselves in a professional manner (i.e.
timeliness, communication; spoken and written etc.). The Contracting Officer may require the Contractor to remove from the job site any employee working under this contract for reasons of suspected misconduct, a suspected security breach, or suspected to be under the influence of alcohol, drugs, or any other incapacitating agent. Contractor employees shall be subject to dismissal from the premises upon determination by the Contracting Officer that such action is necessary in the interests of the Government. The hospital director or designated representative has the authority to bar individuals from the Government facility. The removal from the job site or dismissal from the premises shall not relieve the Contractor of the requirement to provide sufficient personnel to perform the services as required by this performance work statement. The Government shall not reimburse the Contractor for travel and other expenses associated with the removal of personnel. Contractor personnel are not authorized to carry or possess personal weapons to include, but not limited to, firearms and knives with a blade length in excess of three inches, while assigned under this contract.
CONTRACTOR ATTIRE AND APPEARANCE:
Contractor personnel shall present a professional appearance commensurate with standards delineated for Government civilian personnel acting in similar capacities.
The Government reserves the right to review Contractor personnel qualifications.
Upon request, resumes shall be provided to the Contracting Officer prior to clearance approval, for review and acceptance by the COR. The intent is verification purpose and not for the Government to become the hiring authority.

(d) Comptroller General Examination of Record. (1) The Comptroller General of the United States, or an authorized representative of the
Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor
Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the
Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(b) The Contractor is advised that in performing contract administration functions, the

Note: Under Executive Order (EO) 13658 an hourly minimum wage of $10.80 for calendar year 2020 applies to all contracts subject to the Service Contract Act for which the contract is awarded (and any solicitation was issued) on or after January 1, 2015. If this contract is covered by the EO the contractor must pay all workers in any classification listed on this wage determination at least $10.80 per hour (or the applicable wage rate listed on this wage determination if it is higher) for all hours spent performing on the contract in calendar year 2020.
The EO minimum wage rate will be adjusted annually. Additional information on contractor requirements and worker protections under the EO is available at www.dol.gov/whd/govcontracts.
