1.This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, in conjunction with FAR Part 13, Simplified Acquisition Procedures as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Requirement is being evaluated under the procedures of FAR Part 13. As such, in accordance with FAR 13.106-2(b)(3), the quotes will undergo a comparative analysis evaluation to determine which vendor provides the best value to the government in terms of technical capability and past performance, while also providing a competitive price.

2. The combined synopsis/solicitation number is 36C25624Q1468. It is issued as a Request for Quote (RFQ). The Government anticipates awarding one Firm-Fixed Price (FFP) contract resulting from this solicitation.

3. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2024-06 (eff. 08/29/2024), VAAR Update 2024-10 (eff.

04/09/2024)

4. This requirement is a total small business set-aside. The North American Industry Classification System number is 541611 with a size standard of $24.5M

B.3 STATEMENT OF WORK (SOW)

Magnet Consultant Services

1: General Information: General: This is a non-personal services contract for Magnet Consultant Services for the Department of Veterans Affairs (VA) Michael E. DeBakey VA Medical Center (VAMC) 2002 Holcombe Blvd. Houston TX 77030. The contractor shall provide all personnel, supervision, transportation, documents, other items and non-personal services necessary to provide Magnet Consultant services in accordance with this SOW. The Government shall not exercise any supervision or control over the contract service providers performing the services herein.

2. Period of Performance: The period of performance is for date of award through September 30, 2024. Estimated Base Period: Date of Award through September 30, 2025

3. Contract Type: Firm Fixed Price

4. Definitions/Acronyms: The definitions used throughout this Statement of Work (SOW) are as follows:

a. Contracting Officer (CO): The person with the authority to enter into, administer, and or terminate contracts and make related determinations on behalf of the Government.

b. Contracting Officer's Representative (COR): The Government person responsible for monitoring Contractor's performance.

c. Procedures Manual/Policies: A document written by the Contractor with VA input describing in detail the methods and standards normally used to accomplish tasks required by the contract.

d. Contractor's representative: Contractor's representative having supervisory responsibility and authority for total performance of work under the contract.

e. Quality Assurance: Those actions taken by the Government to check goods or services to determine that contractor meet the requirements of the SOW. The Government shall evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan. The plan is primarily focused on what the Government will do to ensure that the contractor is monitoring and has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).

f. Quality Control: Those actions taken by the Contractor to control the production of goods or services so that they will meet the requirement of the SOW. The contractor shall maintain effective quality control to ensure services are performed in accordance with this SOW. The contractor’s quality control is a means by which the contractor assures that their work complies with the requirements of the contract.

g. Statement of Work: (SOW).

h. Medical Facilities: Includes the Michael E. DeBakey VA Medical Center and Community Based Outpatient Clinics (CBOC)

5. DESCRIPTION OF WORK: The Contractor shall be required to provide all Contractor personnel, supervision, transportation, documents and other items/services necessary to perform the Magnet Consultant Services.

1. Contractor shall submit with technical quote the capability to provide the following services at the MEDVAMC that include but not limited to:

a. Conduct an onsite Magnet Readiness Assessment.

b. Provide leadership coaching and mentoring to build a culture of nursing excellence and support the journey to Magnet designation.

c. Review the entire Magnet document and provide feedback on content and recommendations for improvement to ensure that the domains are adequately addressed.

d. Have ongoing meetings with leadership and other identified groups to foster necessary changes as determined, to meet the desired goals.

e. Provide on-site or online workshops on the following key topics: successful Magnet writing and implementing continuous improvement at the bedside.

f. Provide data analysis support for critical Magnet data, including nurse satisfaction, patient satisfaction and nurse-sensitive patient indicators.

g. One-on-one sessions, as deemed necessary, to foster nursing excellence.

h. Meet with the Care Line Nurse Executives, at least monthly, to assess their performance needs and arrange sessions as indicated by the Associate Director of Patient Care Services (ADPCS) to facilitate Magnet goals.

i. Coach and consult Leadership in addressing the VHA Strategic Plan for Nursing Excellence.

j. Have ongoing meetings with leadership and other identified groups to foster necessary changes as determined, if indicated.

k. Provide an onsite or virtual mock site visit to prepare the nursing team for a successful visit by

American Nurses Credentialing Center (ANCC) Magnet appraisers.

l. Support the preparation of Magnet documents for submission into the ANCC, Application

Document and Appraisal Management (ADAM) platform. Provide a secure web-based solution to present Magnet documents to staff nurses.

m. Overtime will not be authorized to finalize the requirements.

n. Attire. While in the performance of their duties, the contractor shall be required to wear appropriate professional business attire with a neat and clean image.

2. The contractor shall submit with technical quote the following that include but not limited to:

a. Demonstrate they have the ability to read and interpret the Magnet accreditation requirements and standards.

b. Have a minimum five (5) years of experience in the role of a Magnet reviewer, or coach in the area of Nursing Excellence.

c. Possess a history of participating in the Magnet writing experience for large Medical Centers (submitted on Corporate Experience form).

d. Provide verification of participating with institutions that successfully obtained at least ten (10) recent Magnet Re-designations (submitted on Corporate Experience form and Past Performance).

e. Have the ability to provide recommendations on programs to better prepare Nurse Managers to be high performing managers.

f. Possess the skills to lead Nurse Care Line Executives in transformational and servant leadership.

g. Possess the ability to effectively communicate in a manner to solicit, motivate and evoke the cooperation of the staff to make recommended changes as reported.

h. Possess a positive, self-directed and motivated attitude and work ethic to interface well with a variety of people.

i. Meet with selected groups to influence the Magnet culture is in effect, if indicated

j. Documented evidence of leading and coaching a diverse staff to attainment of successful leadership execution.

6. CONTRACTOR'S PERSONNEL:

a. Contractor shall assign a designated point of contact for the purposes of maintaining communication with the COR to ensure contractor personnel is incompliance with all provisions of this contract, attend/schedule meetings when required, submit schedules and schedule training.

b. Contractor shall submit with technical quote resumes and qualifications for contractor personnel.

c. Contractor Personnel rosters – Contractor shall provide rosters of all regular and alternate Contractor personnel to the COR prior to actual performance. Contractor personnel not listed shall not be allowed to work in this medical center.

7. SAFETY: Safety precautions shall include, but are not limited to the following requirements:

a) Occupational Safety and Health Act (OSHA)

b) Applicable State and Local Safety Operation Procedures

c) VA Safety Practices and Policies, VAMC standard operating procedures, and any other Federal, State, and Local regulations pertaining to the equipment listed for this contract.

d) The Joint Commission, formerly, Joint Commission Accreditation of Healthcare Organizations (JCAHO).

8. QUALITY ASSURANCE: The Government shall monitor the Contractor's performance under this contract.

9. HOURS OF OPERATION: Magnet Consultant services shall be provided Monday through Friday (excluding Federal holidays) 8:00 a.m. – 4:30 p.m

10. Government personnel observed Holidays: The Government hereby provides notice and Contractor hereby acknowledges receipt that Government personnel observe the listed days as holidays:

10.1 If the holiday falls on a Sunday, the following Monday will be observed as a national holiday.

When a holiday falls on a Saturday, the preceding Friday is observed as a National holiday by U.S. government agencies, and any other day specifically declared by the President of the United States.

10.1.1. In addition to the days designated as holidays, the government observes the following days:

10.1.1.1 Any other day designated by Federal Statute.

10.1.2 Any other day designated by Executive Order.

10.1.3 Any other day designated by the President’s Proclamation

10.2 It is understood and agreed between the government and the Contractor that observance of such days by the government personnel shall not otherwise be a reason for an additional period of performance, or entitlement of compensation. In the event the Contractor’s personnel work during the holiday, they may be reimbursed by the Contractor, however, no form of holiday or other premium compensation shall be reimbursed either as a direct or indirect cost, other than Contractor normal compensation for the time worked.

11. The following laws, regulations, policies, and procedures, including all subsequent changes or updates apply:

FAR Federal Acquisition Regulation VAAR Veterans Administration Acquisition Regulation Standards All applicable Federal, State, Local and Industry: Laws, Policies and Standards as applicable to the proper performance afore and hereafter described.

12. SCHEDULE OF INSURANCE COVERAGE:

A. Workers' Compensation and Employee Liability.

(1) Contractor is required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in states with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.

B. General Liability - $500,000 per occurrence.

13. DEPARTMENT OF LABOR WAGE RATES: Contractor shall submit any wage increase request to Contracting Officer, to include description of labor category from the Department of Labor with a breakout of employees and amount.

14. INVOICING/PAYMENTS

All invoices shall be submitted in arrears, properly prepared in accordance with FAR 52.212-4, contain sufficient details, and match with the service tickets for the work rendered.

a. Monthly Invoices

These invoices shall be sent in arrears at the beginning of each month following the month in which the services were rendered and billed for. At a minimum, all invoices shall include the following details:

Description of the services rendered

Billing period in which the services were rendered

Correct purchase order number which will be issued by the Contracting Officer after the contract is awarded. Invoices without correct purchase order number shall be rejected and returned to the Contractor.

15. Contractor personnel shall not be considered government employees for any purpose and that contractor will have sole responsibility for the manner and method of performing the tasks outlined in the proposed contract.

16. Contract Modification: The Contracting Officer is the only one with authority to modify the terms of the contract and may do so via a bi-laterally executed written modification in accordance with FAR 52.212-4.

17: Information:

a. Estimated leaders with direct reports are twelve (12) Nurse Executive Leaders.

b. Estimated physicians employed by the organization are approximately three hundred (300) paid physicians and one hundred (100) – two hundred (200) Wound, Ostomy & Continence

(WOC).

c. Estimated advanced practice practitioners (APP) employed by the organization are eighty (86) advanced practice registered nurses (APRN).

INSTRUCTION TO OFFEROR(S):

This solicitation is issued as a Total Small Business Set-Aside under Federal Acquisition Regulation 12 in conjunction with FAR Part 13, Simplified Acquisition Procedures as supplemented with additional information. This solicitation is to establish a Firm Fixed Price contract.

SUBMISSION OF QUOTE INSTRUCTIONS:

Quotation shall be received no later than 11:59p.m. Central Standard Time on September 19, 2024. Quote shall be submitted electronically through email addressed to Jeneice Matthews, Contract Specialist at jeneice.matthews@va.gov .

The Contractor shall be registered in www.sam.gov to receive government contracts to be considered for award and all quoters shall be verifiable in the U.S. Small Business Administration (SBA) database to receive credit as a Small Business at the closing of the solicitation due date and time, and at the time of award.

Offeror shall complete and submit the following:

The Contractor shall submit a price for all Contract Line-Item Number (CLIN) of the solicitation.

Provide all CLIN items listed in this solicitation that are annotated in the Price/Cost Schedule. See FAR Clause 52.212-2 Addendum for basis for award. The price quote shall be legible, specific, and complete in every detail.

Note: Pricing shall be all inclusive, for example, shipping cost/price must be rolled into the line item of the product and not separated as a separate line item.

2. ADDENDUM FAR 52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL

SERVICES (NOV 2021)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:

(1) Technical

(2) Past Performance

(3) Price

The Contractor shall be registered in SAM.gov https://sam.gov to receive government contracts to be considered for award.

The Contractor shall be certified with SBA to receive credit as a Small Business.

The Government will determine the merits of each offer on the basis of (1) its acceptability, and (2) its price reasonableness.

Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process outlined in FAR 13.106-2 (b) (3), where quotations will be compared to one another to determine which provides the best benefit to the Government.

Factor 1 Technical

SUB FACTOR 1 – Technical/Management Approach

A. Contractor shall submit with technical quote evidence and capability to provide the services per the Statement of Work (SOW) in section 5.1.

SUB FACTOR 2 – Contractor Personnel A. Contractor shall submit with technical quote resumes and qualifications for contractor personnel per the Statement of Work (SOW) in section 6.

SUB FACTOR 3 – Relevant Experience

A. Contractor shall submit with technical quote evidence the contractor possess the relevant experience per the Statement of Work (SOW) in section 5.2.

See Attachment 3, Relevant Experience Form.

Factor 2 Past Performance

(A) In this factor, the Government will evaluate be evaluated for responsibility purposes and demonstrated past performance of work of a similar nature, frequency, quality of performance and relevance to this procurement. Contact persons provided for project submitted under Factor #1 Relevant Experience above may be contacted by the Evaluation Panel. The offeror’s past performance on similar project, as described below, will be considered.

(B) Contractor shall submit with technical quote past performance that demonstrates services provided.

(C) The government will use information sources such as PPIRS and CPARS and other Federal government offices and commercial sources to assess performance.

Note: Offerors with no relevant past performance history will not be evaluated favorably or unfavorably on past performance and will be given a neutral rating.

Factor 3 Price

(a) In this factor, price will be evaluated for reasonableness, completeness, and fairness by comparing the proposed prices with the Independent Government Estimate and quotes posed by other offers.

(b) Deleted.

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:

"The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."

"The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"
