About this file
This document is a Request for Proposal (RFP) for Cranial Prosthesis and Services issued by the Department of Veterans Affairs (VA) Washington DC VA Medical Center. The solicitation seeks multiple vendors to provide cranial prostheses and related fitting services for eligible veterans, with a contract period from January 19, 2026, through January 18, 2031, including a base year and four one-year option periods. The procurement is a multi-award Indefinite Delivery Indefinite Quantity (IDIQ) contract with a total minimum order of $5,000 per contractor and a maximum order of $1,800,000 for all contractors combined.
The solicitation is set aside for Service-Disabled Veteran-Owned Small Businesses (SDVOSB) and requires vendors to provide human hair and synthetic cranial prostheses up to 18 inches in length, with various color and texture options. Vendors must hold current cosmetology licenses in Maryland, DC, or Virginia, demonstrate specialized training in hair and scalp disorders, and provide services in facilities that support veteran privacy and dignity. The evaluation will consider factors such as licensure, professional qualifications, facility capabilities, product range, and an oral presentation demonstrating fitting and styling processes. Proposals are due by January 5, 2026, at 4:00 pm EST, with the contract anticipated to be awarded on January 19, 2026.

B.3 PERFORMANCE WORK STATEMENT (PWS)
WASHINGTON DC VA MEDICAL CENTER
CRANIAL PROSTHESIS, FITTING AND MEASURING SERVICES
1. SCOPE
The Department of Veterans Affairs (VA), Washington DC VA Medical Center (DC), has a large population of eligible veterans requiring assistance with the provision of Cranial Prostheses in addition to fitting and measuring services. It is DC’s intention to solicit and award multiple contracts to at least 3, but no more than 5 qualified vendors on a competitive basis under the authority of Title 38 U.S.C. Section 8123. Anticipated contract period is January 19, 2026, through January 18, 2031, with four (4) one (1) year follow-on option periods.
The Cranial Prostheses and related services are to be provided to veterans enrolled in the following facility.
Washington DC VA Medical Center 50 Irving Street, NW Washington, DC 20422
2. BACKGROUND
The Prosthetics and Sensory Aids Service (PSAS) is responsible for providing eligible Veterans with cranial prosthesis when a VA clinician has determined that the cranial prosthesis is medically necessary and serves as a direct and active component of the veteran’s treatment or rehabilitation.
Cranial prosthesis (medical-grade wigs) is provided to veterans who experience hair loss due to medical conditions or treatments such as chemotherapy, alopecia or severe burns. When clinically prescribed, the cranial prosthesis plays a key role in the veteran’s overall treatment plan, contributing to mental health, confidence, and quality of life. This service is authorized under 38 U.S.C. §§ 1701(6)(F) and 1710(a), and outlined in 38 CFR §§ 17.3200–17.325 as part of the VA’s responsibility to furnish prosthetic items that are medically necessary and not solely for comfort or convenience.
To fulfill this mission and meet growing demands, the Washington DC Prosthetics Department seeks to establish a multi-award contract with qualified vendors who can fabricate and provide cranial prostheses that meet medical-grade standards, accommodate diverse cosmetic and cultural needs, and comply with clinical specifications outlined by prescribing providers. The purpose of this contract is to ensure timely, equitable, and cost-effective access to these critical devices for all eligible Veterans.
3. DEFINITIONS
A. GOOD STANDING: A cranial prosthesis vendor that maintains timely invoicing, professional conduct, required licensure, insurance, and certifications, as well as no patient complaints, demonstrates good ethics, and consistently conforms with purchase order agreements.
B. PURCHASE CARD/VISA LEVEL II: Enhanced data is the additional information that accompanies charge card transactions. Enhanced data is required for processing financial transactions or for settlement of that transaction. Level II provides additional summary data that accompanies Level I (minimum amount of data required to clear and settle a financial transaction) through the clearing and settlement process.
5. MULTIPLE AWARDS

The Contracting Officer contemplates a multi-award Indefinite Delivery Indefinite Quantity (IDIQ) firm fixed price contract(s) pursuant to this solicitation for commercial items, Prosthetic appliances and related services. The contract will be in service of the DC facility, in which purchase orders may be placed toward any of the contract(s) awarded under this solicitation.
B. The contract will contain one (1) 12 - month base year and four (4) 12 - month renewable option year periods.

B. All Contractor personnel are required to check into the facility Police & Security office at each visit to pick up a “contractor” badge prior to contacting the COR. To facilitate visits by vendors, appointments should be scheduled in advance with the respective service. Vendor appointments may be cancelled at any time due to the needs of the organization. Vendors who have previously scheduled appointments may not initiate requests for impromptu meetings with other VA staff whom they may happen to encounter during their scheduled visit.
Upon arrival at the Medical Center, vendors will report directly to Police & Security Office. Each vendor must present his/her driver’s license upon arrival and will be logged-in to be issued a vendor’s badge. Products that are not commercial items are considered experimental, and purchase of the experimental products shall not be authorized.
B. Educational and Training Requirements
The Contractor and all Contractor employees must hold a current valid Cosmetology License in either Washington, D.C., Maryland, or Virgina. The Contractor is responsible for ensuring that all personnel assigned to this contract are fully trained, competent, and qualified to perform the required services.

This training shall include an understanding of basic dermatological concepts related to the scalp and skin, such as inflammation, infection, irritation, sensitivity, and common dermatologic contraindications to cosmetic or prosthetic services.
The Cosmetologist shall demonstrate an ability to recognize when hair loss or scalp conditions may require referral to a medical provider (e.g., dermatologist or primary care clinician) and shall agree to refer Veterans back to their health care team when signs of infection, active inflammatory scalp disease, suspicious lesions, or other concerning findings are observed. Throughout the duration of this contract, notification of any facilities which are added, changed or closed must immediately be conveyed to both the Contracting Officer and the COR to obtain approval before it may be used for this contract. Any person renting a booth within the approved Contractor location will not be permitted to perform services on Veteran beneficiaries.
Booth rentals, chair rentals, or space renting, by whatever name called, is recognized as an independent business, and as such, each booth, chair, or space must have a salon or shop owner's license, a manager's license, and, if applicable, the appropriate business licenses or registrations. No person(s) shall operate utilizing another person(s) licenses and/or registrations. The space where the Veteran is being serviced must be isolated to ensure the Veteran's privacy.

The pricing paid shall be mutually agreed upon in writing and shall not exceed the contract price of the cranial prosthesis. If the cranial prosthesis can be used for another beneficiary, the Contractor must only charge for resizing and fitting fees as listed in the contract.

Expedited delivery terms and conditions, when required, will be determined by the Contracting Officer on a case-by-case basis and stated in the individual task or delivery order. The vendor shall validate and confirm the expedited delivery request within 48 hours of receipt. If expedited delivery cannot be fulfilled, the vendor shall submit justification to the COR and offer standard delivery options or propose an alternative delivery date. Finalized delivery dates will be determined on a case-by-case basis.
I. Veteran beneficiaries will be measured, fitted, and will accept delivery of their cranial prosthesis at the contractor's facility. For the purposes of this contract, "measured" means obtaining all head measurements required to fabricate or customize the prosthesis, and "fitted" means customizing and adjusting the prosthesis to ensure proper comfort, appearance, and stability. These activities are integral components of cranial prosthesis fabrication and are included in the unit price; they are not separately billable services. Delivery shall not occur until measurement and fitting have been completed and the Veteran confirms satisfactory fit. Contractors shall not use VA Medical Center space or resources to perform any services listed in this contract.
J. The contractor warrants the cranial prosthesis provided against defective material and/or workmanship for a minimum of 90 calendar days from the date of acceptance. The contractor agrees to furnish without additional cost to the government, all labor and materials necessary to correct defects that were detected during the guarantee period. These conditions do not apply to adjustments incidental to the device adjustments required by physical change of the wearer, or where there is evidence of deliberate misuse or alteration by anyone other than the contractor.
K. After 90 calendar days, any repairs needed to the cranial prosthesis will be determined by the Contractor within 5 business days of receipt of cranial prosthesis. If repairable, contractor fees incurred to make changes to the cranial prosthesis will be itemized and submitted to the VA Medical Center Prosthetic Representative, COR or designee for approval. Contractor must provide an estimated delivery time for repairs to the VA Medical Center. Repairs should be accomplished and cranial prosthesis delivered to the Veteran within 45 calendar days. The contractor shall notify the VA Prosthetic Representative, COR or designee and show cause in the event s/he is unable to deliver the required items in accordance with the Government's required delivery date. These instances will be addressed accordingly on a case-by-case basis.

The Contractor shall correct all deficiencies within 15 calendar days of written notification from the COR or designee without additional cost to the government. Advanced payment will not be authorized.
If a repair service is required, the VA Medical Center Prosthetic Representative or designee will be notified by submitting a quote via facsimile or email itemizing the services needed and estimated costs should the cost exceed the amount listed in price schedule. All repairs will need to be approved prior to proceeding by the COR or designee.
The vendor shall provide only those wig products and related services authorized and funded by the Department of Veterans Affairs which are listed in this contract.
Veterans shall not be permitted to upgrade, exchange, or supplement the authorized wig product(s) or services at their own expense.
The vendor shall not solicit, market, or offer any additional products or services (whether related or unrelated to the authorized procurement) to VA beneficiaries for an additional fee. Any attempt to solicit or accept direct payment from veterans for upgraded items or additional services not approved by the VA may result in termination of the agreement and potential disqualification from future VA contracts.
10. SERVICES NOT COVERED UNDER THIS CONTRACT
A. Per VHA Cranial Prosthesis Policy PSAS can provide a wig to male and female Veterans who have alopecia, (hair loss or baldness) either primary or chemotherapy induced. Wigs can be synthetic or of natural hair. Prosthetics do not provide for the maintenance of the cranial prosthesis which includes hair accessories (brushes, combs, cranial prothesis holder), cleaning, trimming, dying, or coloring the hair. Because natural hair wigs and their maintenance are more expensive than synthetic hair wigs, Veterans should be advised of the additional expense. With proper maintenance and depending on the length of time worn, a wig may be used for a extended amount of time.
B. Initial cranial prosthesis fittings include basic customization of the unit (cut, shaping, and minor thinning), adjustment of the cap for comfort and security, and finishing styling such as part placement and light heat-styling so the wig resembles the patient’s desired everyday look.
More advanced or frequent restyling (e.g., major cut changes, color work, or repeated curls/sets) shall be treated as separate follow-on services which are not covered under this contract.
Styling (as described above) associated with the initial fitting is included as part of the fabrication and delivery of the cranial prosthesis. Any ongoing or repeat styling beyond the initial appointment will not be provided, such as:
• Laser Caps
• Scalp Treatments
• Hair color, cut and re-style
• Non-commercial, experimental, or non-FDA approved treatments or products
• Styling products

Only the Contracting Officer is authorized to make commitments or issue changes, which will affect price, quantity, or quality of performance of this contract. In the event the contractor effects any such change at the direction of any person other than the contracting officer, the change shall be considered to have been made without authority, and no adjustment will be made in the contract price to cover any increase in costs incurred as a result thereof.
B. Contractors Remittance Address: All payment by the Government to the Contractor will be made in accordance with Federal Acquisition Regulation (FAR) Clause 52.232-34, Payment by Electronic Funds Transfer – Other than Central Contractor Registration (31 U.S.C. 3332):

A. Invoices submitted for payment shall include the contract number, purchase/delivery order number, beneficiary’s name; detail of work accomplished (itemized invoice). The mandatory and only method of payment will be the Government Purchase Card as prescribed by national directive and the Contractor must be capable of accepting it for payment. The contractor must be able to process it at Level II using the appropriate field for the obligation number. If this involves additional machinery or computers, this will be at the contractor’s expense. Each invoice must be submitted to the VA Medical Center Prosthetic and Sensory Aids Service (PSAS) within five (5) days of prosthesis delivery to the Veteran. Both the signed delivery acceptance form and the order form will be submitted with the invoice. Payment of invoices may be delayed if the appropriate invoices and supporting documentation as specified in the contract are not completed and submitted as required.
B. All VAs using Government Purchase Card/VISA, a credit card receipt with the invoice and signed acceptance receipt (see 9.B above) must still be forwarded to the appropriate VA facility as soon as the transaction has been completed in accordance with the guidelines and time frames in utilizing the Government Purchase/VISA Credit Card. A copy of the Veteran signed and dated form of delivery acceptance accompanying a copy of the charge that is made for the prosthesis shall be emailed to the prosthetic service at the Healthcare Center. (No 3rd party billing)
