About this file

This is a Past Performance Information Questionnaire template required to be submitted with quotes for a USDA FPAC Business Center solicitation for title abstract search and settlement/closing services in Oklahoma. The questionnaire requires previous clients to rate the contractor's performance on a scale of 0-4 (Unsatisfactory to Outstanding) across multiple performance areas.

The questionnaire evaluates nine key performance areas: meeting contract deliverables, timely submission of deliverables, adherence to delivery dates, reasonableness of pricing for changes/claims, quality of Performance Work Statement execution, problem-solving ability, teaming/partnering relationships, work completion, and overall management effectiveness including cooperation with technical representatives and stakeholders. Respondents must provide contact information, contract details, and signature. For any ratings below 2 (Satisfactory), an explanatory narrative must be attached. The form concludes with an overall performance assessment and whether the respondent would hire the contractor again.
