About this file

This is a Representations and Certifications form (Exhibit "D") for a real property lease with the federal government.

The form establishes the lessor's legal authority to lease property and requires submission of taxpayer identification information (TIN) for debt collection and reporting purposes. Lessors must represent their business structure and provide a DUNS number for System Award Management (SAM) database registration. For small businesses, the form requires representations regarding small business status, including designations as small disadvantaged businesses, women-owned small businesses, veteran-owned small businesses, service-disabled veteran-owned small businesses, and HUB Zone small businesses. The NAICS code for this acquisition is 531190 with a small business size standard of $17.5 million in gross receipts. For leases exceeding $10,000, lessors must represent compliance with Equal Employment Opportunity requirements and previous contract compliance reporting. The form mandates that contractors maintain current SAM database registration with accurate DUNS numbers throughout contract performance and final payment, with the government proceeding to award the next qualified offeror if registration is not completed within the prescribed timeframe. Contractors bear responsibility for data accuracy in SAM and must update their information annually to ensure compliance.
